A Smile Reimagined: Gummy Smile Resolution with Cutting Edge Method Using Chu's Gauge and Polymethylmethacrylate (PMMA) Bone Cement.

Excessive gingival display (EGD) is one of the most common aesthetic concerns, and its correction often presents a challenge to periodontists. It has a multifactorial etiology, and this article describes a case involving hypermobile upper lip (HUL), altered passive eruption (APE), and vertical maxillary excess (VME). Upon investigation, a positive collum angle and marked subnasal skeletal depression were observed. In this context, it is noted that during a spontaneous smile, the upper lip retracts and gets lodged in this depression. The rehabilitation plan includes included aesthetic crown len

A Review of Current Techniques in Lip Reposition Surgery for Treating Excessive Gingival Display.

Excessive gingival display (EGD), commonly known as a gummy smile (GS), is a cosmetic concern that involves exposing a significant area of gum tissue during a smile, rendering it unaesthetic. Gingival exposure greater than 3 mm is deemed aesthetically displeasing and often necessitates treatment to mask the gummy smile. The causes of EGD are multifactorial, including altered passive eruption (APE), hypermobile upper lip (HUL), short lip length, increased vertical maxillary component, gingival hyperplasia, dentoalveolar extrusion, and more. As each aetiology requires different treatment modalit

Crown-root fracture with pulp exposure: a case report with 16-year follow-up.

A 12-year-old boy presented for dental care 35 days after he fell from his bicycle. Clinical and radiographic examinations revealed a longitudinal crown-root fracture with pulp exposure in the maxillary left central incisor. The radiograph also suggested necrosis of the maxillary right central incisor. Urgent treatment of the left central incisor involved gingivectomy followed by autogenous bonding of the tooth fragment with self-curing composite resin. Immediately after bonding, coronal access was prepared, chemical and mechanical preparation was completed, and a calcium hydroxide intracanal

An abbreviated history of osseous surgery.

Various forms of periodontal therapy, including surgery, have been advocated and documented in the dental literature during the last three centuries. This variety of treatment modalities has been developed to address the anatomical consequences (pocket formation and bone loss) sustained from chronic periodontal disease. The marked differences in techniques have created significant controversies between the greatest leaders in dentistry and their equally influential disciples. Nevertheless, these leaders have always shared a common goal: the preservation of the natural dentition in a harmonious

Current concepts on gingival fibromatosis-related syndromes.

Gingival fibromatosis is a rare, benign, slowly-growing fibrous overgrowth of the gingiva, with great genetic and clinical heterogeneity. Gingival fibromatosis/overgrowth can be inherited as an isolated trait (hereditary gingival fibromatosis) and/or as a component of a syndrome, or it can be drug induced. As a clinical manifestation of a syndrome, gingival fibromatosis is usually associated with generalized hypertrichosis, mental retardation, or epilepsy. Gingival fibromatosis and its related syndromes are mainly inherited in an autosomal-dominant manner, but autosomal-recessive inheritance h

An unusual case of generalized severe gingival enlargement during pregnancy.

Increased hormone levels that are present during puberty and pregnancy are associated with localized or generalized gingival enlargement. This article reviews the gingival alterations that can occur during pregnancy and describes a case of generalized severe gingival enlargement associated with pregnancy and its management. A 36-year-old woman had severe bilateral gingival enlargement of short duration. The patient denied taking any medications. The laboratory report revealed no systemic abnormalities; however, the report disclosed that she was pregnant. Surgical therapy for the gingival enlar

Re-eruption of an intruded immature permanent incisor with necrotic pulp: a case report.

Intrusive luxation is a serious dental injury that causes damage to the pulp and supporting structures of a tooth because of its dislocation into the alveolar process. This paper presents the case of the re-eruption of a severely intruded immature permanent incisor with a crown-root fracture. A 9-year-old boy was referred to the clinic 1 day after a fall. Clinical and radiographic examinations revealed intrusive luxation of the immature left permanent incisor and a crown-root fracture without pulp exposure. Palatal gingivectomy was done 2 weeks later to facilitate re-eruption. Root canal thera

Hereditary gingival hyperplasia associated with amelogenesis imperfecta: a case report.

Hereditary gingival fibromatosis (HGF) and amelogenesis imperfecta (AI) are two rare oral conditions with genetic etiologies. The case of a 17-year-old boy affected by HGF, AI, anterior open bite, and pyramidal impaction of the maxillary molars is reported. Internal bevel gingivectomies were carried out to reduce gingival overgrowth. Clinical examination of the family revealed the presence of HGF and AI in his 12-year-old sister (both in milder forms) and of HGF in his older half brother. Genetic sequencing analyses were performed to detect any of the known mutations leading to HGF and AI. His

Oral Crohn's disease without intestinal manifestations.

Crohn's disease is a granulomatous inflammatory bowel disease and was described in 1932 as a chronic granulomatous disorder of the terminal ileum and is now considered a distinct member of the inflammatory bowel disease family. It may affect any part of the gastrointestinal tract. Oral Crohn's disease has been reported frequently in the last three decades with or without intestinal manifestations. In the latter case, it is considered as one of the orofacial granulomatosis. There has been much doubt whether intestinal manifestations of Crohn's disease will eventually develop in the orofacial gr

A combined approach with passive and active repositioning of a traumatically intruded immature permanent incisor.

BACKGROUND: Severe damage to the tooth, periodontal ligament, and pulpal tissue often occurs with intrusive luxation, and outcomes are quite unpredictable because of the variable ways in which the injury may occur, which influences both the choice of treatment and prognosis. CASE REPORT: This case presents a novel method for the management of an intruded permanent maxillary central incisor with an immature apex. At first, watchful waiting for spontaneous re-eruption was the chosen treatment option. A palatal gingivectomy was performed and endodontic access was established, following which, an

A modified double pedicle graft technique and other mucogingival interceptive surgeries for the management of impacted teeth: a case series.

Maxillary canine is one of the most common teeth that are impacted. This accounts for 1-2% of all patients who attend orthodontic treatment. The key to achieve maximal eruption of these teeth is their surgical exposure and the role of periodontist in such situations is to provide a functional and satisfactory width of attached gingiva on the labial surface. There are different techniques to surgically expose the impacted teeth, namely--gingivectomy technique, apically positioned flap, closed eruption technique, modified apically positioned flap, double pedicle flap and free gingival graft. Esthetic crown lengthening in the treatment of gummy smile.

The aim of this article was to evaluate 1 year after surgery a surgical protocol that included gingivectomy and an apically positioned flap plus osseous resective surgery to correct excessive gingival display (EGD) in patients with altered passive eruption (APE) of the maxillary anterior teeth. Six female patients aged 18 to 22 years were diagnosed with APE type 1B. Surgical crown lengthening with flap surgery and bone recontouring was performed to achieve the biologic width. Photographic images were analyzed to evaluate the stable improvement of crown length before the procedure (baseline), i

Management of a Complex Crown-Root Fracture in a Single Appointment Through Root Canal Therapy and Rebonding.

A 10-year-old male presented with a mobile and fractured maxillary right central incisor following dental trauma. Clinical and radiographic examinations revealed a chisel-type crown-root fracture with supragingival labial and subgingival palatal extensions. A direct crown reattachment was planned, emphasizing biological width preservation. After removing the coronal fragment, endodontic treatment was performed using a rotary file system and was obturated with a bioceramic sealer. A fiber post was selected and integrated into the fractured fragment, which was reattached using resin cement. Addi

Management of Aberrant Frenum and Gingival Overgrowth in Orthodontic Patients: A Case Report.

The frenum, a fold of mucous membrane, connects the lip and cheek to the alveolar mucosa, gingiva, and underlying periosteum. When the frenum is positioned excessively near the gingival margin, it has the potential to compromise gingival health, impeding plaque control efforts and inducing muscular stress. A frenectomy is a commonly employed corrective measure for anomalous frenum attachments. In a recent clinical case, a 21-year-old female patient was referred from the Department of Orthodontics to the Department of Periodontics due to a papillary-type aberrant labial frenum attachment and ex

Treatment of calcium channel blocker-induced gingival overgrowth without modifying medication.

Gingival overgrowth is a common side effect of calcium channel blockers used in the treatment of cardiovascular diseases. While controversial, management includes discontinuing the calcium channel blocker. We report the case of a 66-year-old Japanese man with hypertension and type 2 diabetes mellitus who was diagnosed with severe periodontitis covering almost all the teeth. The patient had been on nifedipine (40 mg/day) and amlodipine (10 mg/day) medication for 5 years. With his physician's consent, nifedipine was discontinued during his treatment for periodontitis, which consisted of oral hyg

Plasma Cell Gingivitis Mimicking Granulomatous Disease: A Diagnostic Dilemma.

INTRODUCTION: Plasma cell gingivitis is an uncommon benign inflammatory condition of the gingiva which is considered to arise as a hypersensitivity reaction to certain allergens. The distinguishing feature of this condition is massive infiltration of plasma cells into the subepithelial connective tissue. This case report discusses a case of plasma cell gingivitis which produced a diagnostic dilemma due to its unusual histopathologic appearance. CASE PRESENTATION: A 13-year-old female patient presented with diffuse gingival enlargement in relationship to upper anterior teeth. The case was mana
