Custom lingual augmentation prostheses for a patient with dysarthria, dysphagia, and functional deficits following complete glossectomy: A clinical report.

This clinical report describes the treatment of a patient with dysarthria, dysphagia, and functional problems following a complete glossectomy subsequent to squamous cell carcinoma of the tongue. A conventional palatal augmentation prosthesis failed to alleviate the symptoms of dysphagia and dysarthria because of the severely restricted movement of the residual lingual tissues. Two custom lingual prostheses that were interchangeable on a single magnetic base were fabricated to assist the patient with speech and deglutition. The lingual augmentation prostheses improved patient symptoms compared

A comparison of histopathologic margin assessment after steel scalpel, monopolar electrosurgery, and ultrasonic scalpel glossectomy in a rat model.

EDUCATIONAL OBJECTIVE: At the conclusion of this presentation, the participants should be able to describe the tissue effects of steel scalpel, monopolar electrosurgery, and ultrasonic scalpel glossectomy with respect to histopathologic margin assessment in a rat model. OBJECTIVES: Histopathologic margin assessment is important in guiding treatment and determining prognosis for squamous cell cancer of the oral tongue. Energy based devices for glossectomy produce varying degrees of tissue distortion and artifact that can influence margin assessment. The purpose of this study is to compare the

Extended sigmoid-shaped free jejunal patch for reconstruction of the oral base and pharynx after total glossectomy with laryngectomy.

The procedure for reconstruction of the extensive defect after total glossectomy with total laryngectomy has not been standardised so far. We have devised a new method in which a free jejunal patch is extended to a sigmoid-shaped form specifically optimised for reconstruction of the defect that consists of the entire floor of the mouth and the antero-lateral walls of the pharyngoesophageal tract. After a jejunal segment of about 40 cm has been longitudinally opened in full length, the proximal quarter is folded parallel to the second quarter, which is then folded parallel to the third quarter

Validity of the American College of Surgeons' National Surgical Quality Improvement Program risk calculator in South Australian glossectomy patients.

BACKGROUND: Appropriate selection of tongue cancer patients considering surgery is critical in ensuring optimal outcomes. The American College of Surgeons' National Surgical Quality Improvement Program ('ACS-NSQIP') risk calculator was developed to assess patients' 30-day post-operative risk, providing surgeons with information to guide decision making. METHOD: A retrospective review of 30-day actual mortality and morbidity of tongue cancer patients was undertaken to investigate the validity of this tool for South Australian patients treated from 2005 to 2015. RESULTS: One hundred and twenty

The tongue may not move, but the voice will: preoperative AI voice cloning for identity preservation in major glossectomy - a prospective feasibility study.

BACKGROUND: Major glossectomy frequently results in profound speech impairment. We investigated whether a patient's voice could be captured preoperatively and reconstructed using artificial intelligence voice cloning for potential future use as a personalized assistive communication voice during postoperative recovery. METHODS: In this prospective single-centre feasibility study, adult patients with treatment-naïve carcinoma of the tongue planned for major glossectomy underwent a single approximately 30-60 second preoperative voice recording in a sound-treated room. Voices were cloned offline

Evaluation of a caudal midline glossectomy on tongue volume and upper airway cross-sectional areas in brachycephalic dogs: a cadaveric study.

INTRODUCTION: Relative macroglossia may contribute to brachycephalic obstructive airway syndrome, the pathologic disorder associated with respiratory dysfunction commonly seen in brachycephalic dogs. Recent studies on brachycephalic dogs have demonstrated a relative macroglossia along with reduced air volume in the upper airway compared to non-brachycephalic dogs. Tongue reduction glossectomy may be a surgical option to address upper airway obstruction secondary to macroglossia. The objective of this study was to evaluate the effects of a caudal midline glossectomy (CMG) on tongue volume and u

Functional MRI during tongue strength tasks before and after partial glossectomy: Insights into the cortical activation of tongue motor function.

AIM: Because the tongue is a midline structure, studies on the neural correlates of lateralized tongue function are challenging and remain limited. Patients with tongue cancer who undergo unilateral partial glossectomy may be a unique cohort to study tongue-associated cortical activation, particularly regarding brain hemispheric lateralization. This longitudinal functional magnetic resonance imaging (fMRI) study investigated cortical activation changes for three tongue tasks before and after left-sided partial glossectomy in patients with squamous cell carcinoma of the tongue. METHODS: Seven

Clinical, functional, and patient-reported outcomes of radial forearm versus anterolateral thigh free tissue transfer for reconstruction of glossectomy defects.

BACKGROUND: There is a lack of literature of health-related quality of life endpoints for radial forearm (RF) versus anterolateral thigh (ALT) free flap reconstruction for glossectomy defects. Our goal was to perform a comprehensive evaluation of clinical, functional, and quality of life outcomes after glossectomy reconstruction using a RF or ALT flap. METHODS: A retrospective review was performed on patients who underwent glossectomy and immediate reconstruction with RF or ALT flaps between 2016 and 2021. Outcomes of interest included readmission and reoperation rates, functional assessments

Functional and quality of life outcomes after partial glossectomy: a multi-institutional longitudinal study of the head and neck research network.

BACKGROUND: While aggressive treatment for oral cancer may optimize survival, decrements in speech and swallowing function and quality of life often result. This exploratory study investigated how patients recover their communicative function, swallowing ability, and quality of life after primary surgery [with or without adjuvant (chemo)radiation therapy] for tongue cancer over the course of the first year post-operation. METHODS: Patients treated for oral cancer at three institutions (University of Alberta Hospital, Mount Sinai Beth Israel Medical Center, and Turku University Hospital) were

Evaluation of speech intelligibility after a secondary dehiscence operation using an artificial graft in patients with speech disorders after partial glossectomy.

The purpose of this study was to evaluate speech intelligibility after a dehiscence operation using artificial grafts for patients with speech disorders after partial glossectomy that were caused by scars resulting from the primary operation. The subjects were six men and three women, who had had a partial glossectomy for tongue cancer followed by direct closure without reconstruction. They were operated on a second time operation to mobilise the residual tongue by dividing the cicatrix. An artificial graft was applied to the wound to maintain the dehiscence. Speech intelligibility was evaluat

Functional reconstruction of swallowing and articulation after total glossectomy without laryngectomy: money pouch-like reconstruction method using rectus abdominis myocutaneous flap.

When a total glossectomy is performed without a laryngectomy, the functional recovery of swallowing and articulation is extremely important in maintaining the patient's quality of life. The authors established a money pouch-like reconstruction method in which a round and raised tongue is rebuilt using a rectus abdominis myocutaneous flap. In this method, the skin island of the rectus abdominis myocutaneous flap is created about 20 percent larger than the defect in both width and length. The skin island is sutured to the defect such that the excess skin folds into the shape of a money pouch. Th
