Is It Possible to Save the Deep Brain Stimulation Hardware when Presenting with Wound Dehiscence or Hardware Infection?

INTRODUCTION: Deep brain stimulation (DBS) hardware complications have been traditionally managed by removal of the entire system. Explantation of the system results in prolonged interruption to the patient's care and potential challenges when considering reimplantation of the cranial leads. The purpose of this study was to understand whether complete explantation can be avoided for patients initially presenting with wound dehiscence and/or infection of hardware. METHODS: We performed a retrospective study that included 30 cases of wound dehiscence or infection involving the DBS system. Patie

Fibular Plate Fixation and Correlated Short-term Complications.

Background. The most common method of surgical stabilization of fibular fractures is plate osteosynthesis. Despite its ubiquity, there is a dearth of large series reporting implant-related outcomes and complications. The purpose of this study was to report on short-term complications and hardware removal after plate fixation of distal fibula fractures. Methods. A retrospective chart analysis and review of radiographic images was performed of 461 ankle fractures between 2011 and 2017. In 404 cases, a fibular fracture was treated surgically; 94.1% underwent tubular and 5.9% locking plate fixatio

Implants removal in children: results of a survey among Italian orthopaedic surgeons.

In the treatment of paediatric limb disorders, the use of metal implants has been increasing over the last decades. Recent studies have addressed the decision of orthopaedic surgeons regarding the removal of implants after the treatment of fracture, and there is a growing consensus within the scientific community supporting the choice of not removing implants in children. This survey aimed to investigate the rationale behind the Italian orthopaedic community's decision regarding metal implant removal in paediatric patients. An electronic questionnaire was sent to all members of the Italian Pae

Bilateral Slipped Capital Femoral Epiphysis Fixation Failure Including the Prophylactic Side: A Case Report.

Slipped capital femoral epiphysis (SCFE) is a common problem treated by pediatric orthopedic surgeons. A 13-year-old male presented with right-sided hip pain. Both sides, symptomatic and asymptomatic, were treated with a single 7.3-mm screw. The patient returned with symptoms to the bilateral hips 16 months after the procedure. He was treated with removal of hardware and revision fixation with a good outcome. We report a rare case of fixation failure in bilateral SCFE with an excellent outcome. We highlight the importance of quick recognition of failure before displacement and a strategy for h

Extreme femoral valgus and patella dislocation following lateral plate fixation of a pediatric femur fracture.

A 15-year-old boy presented with a 35° femoral valgus deformity, leg-length discrepancy, painful retained hardware, and a lateral dislocation of the patella 4 years after undergoing lateral plate fixation of a distal femur fracture. Femoral valgus is a possible complication of lateral plate fixation in up to 30% of pediatric distal femur fractures. With this patient's unusual combination of deformities as an example, we suggest early hardware removal after fracture union to prevent the development of deformity. If plate removal is not chosen, then continued close monitoring of the patient is n

Navigated Simultaneous Lateral Minimally Invasive Tubular and Posterior Mini-Open Access for Removal and Revision of Triangular Sacroiliac Joint Implants: A Technical Note.

BACKGROUND: Sacroiliac joint (SIJ) fusion, to treat back pain caused by SIJ dysfunction, can employ open or minimally invasive surgery (MIS) techniques and either cylindrical (screw-shaped) or triangular (wedge-shaped) implants. Fusion nonunion sometimes explains recurrent SIJ pain following fusion and occasionally requires hardware revision. MIS revision minimizes patient pain, infection, and disability, but due to the triangular implant size and form factor, implant removal can present challenges for MIS access during the explantation and achieving good bony purchase for reinstrumentation. A 15-year experience with permanent pacemaker and defibrillator lead and patch extractions.

BACKGROUND: The value of extraction of infected or hazardous epicardial and endocardial pacemaker and internal cardioverter defibrillator (ICD) leads is well established. Recent reviews including all leads and patches are lacking. This review describes experience with open and percutaneous techniques, including all lead types and indications. METHODS: With Institutional Review Board approval, we reviewed charts of all adults and children undergoing extraction of permanent pacemaker and ICD leads and patches by a single operator between 1993 and 2008. RESULTS: Overall, 145 leads and 7 patches

Can low-profile double-plate osteosynthesis for olecranon fractures reduce implant removal? A retrospective multicenter study.

BACKGROUND: Because of poor soft-tissue coverage at the proximal ulna and prominent posteriorly positioned implants, hardware removal remains the most common reason for revision surgery of olecranon fractures that were operatively treated using plate osteosynthesis. We hypothesized that low-profile double-plate osteosynthesis would reduce the number of soft tissue-related hardware removals compared with single posterior plating whereas the functional results would be comparable. METHODS: This study retrospectively included patients who were treated with low-profile double-plate osteosynthesis

Types of Sliding Calcaneal Osteotomy Fixation: A Systematic Review and Meta-Analysis.

Introduction Different methods are used to fix a sliding calcaneal osteotomy for hindfoot varus and valgus deformity. However, information about the effectiveness and limitations of each method is limited. In this meta-analysis, we compare the hardware removal rate, union rate, and complications of three different methods of fixation: plate, headed screw, and headless screw. Methods A systematic review and meta-analysis of published articles were carried out, following the recommendations of the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines. Polylactic acid bioabsorbable implants of the hand: A review.

In the treatment of hand fractures, metal implants are often used to allow early range of motion and a stable fixation. Although they provide adequate bone union, metal implants have been known to cause joint stiffness, painful or prominent hardware, and adhesions, often leading to another surgery for hardware removal. Bioabsorbable implants have been shown to offer comparable results for fracture fixation in the hand while removing the complications seen with retained hardware. In this article, we review biomechanical and clinical studies focused on bioabsorbable implants made of polylactic a

Treatment of superficial wound infection following open reduction and internal fixation of ankle fractures: A scoping review.

INTRODUCTION: Ankle fractures account for approximately 10% of fractures among adults and frequently require surgical intervention, with open reduction and internal fixation being the gold standard. This intervention is associated with a high complication rate. Superficial infections can easily progress due to limited soft-tissue coverage. It is therefore vital to treat early infection. This study has aimed to map the treatments of superficial infection, following open reduction and internal fixation of ankle fractures. METHODS: This scoping review was conducted in accordance with the Joanna

Removal of a retained Herbert-Whipple screw with use of the Osteochondral Autograft Transfer System (OATS) core harvester: a case report.

After a revision procedure is performed to address a full-thickness chondral defect of the knee, hardware removal can be challenging. We describe using a core harvester, the Osteochondral Autograft Transfer System (OATS; Arthrex, Naples, Fla), to remove a Herbert-Whipple screw placed earlier in a failed attempt to fix an osteochondritis dissecans lesion on the lateral aspect of the medial femoral condyle. This hardware removal technique can be performed either arthroscopically or in an open fashion and may also be used with other revision procedures (ie, revision anterior cruciate ligament rec

Should we remove flexible intramedullary nails after diaphyseal forearm pediatric fracture? A cohort study of 288 patients.

BACKGROUND AND PURPOSE: Routine removal of elastic stable intramedullary nails (ESIN) after pediatric diaphyseal forearm fractures remains common practice despite limited supporting evidence. We aimed to evaluate refracture risk following ESIN retention compared with implant removal and assessed complications and healthcare resource use. METHODS: In this retrospective cohort study, patients treated with ESIN for diaphyseal forearm fractures at a single center were included. Following a policy change, routine implant removal was discontinued in favor of retaining implants unless symptomatic. P

Does Open Reduction and Internal Fixation versus Primary Arthrodesis Improve Patient Outcomes for Lisfranc Trauma? A Systematic Review and Meta-analysis.

BACKGROUND: Although Lisfranc injuries are uncommon, representing approximately 0.2% of all fractures, they are complex and can result in persistent pain, degenerative arthritis, and loss of function. Both open reduction and internal fixation (ORIF) and primary fusion have been proposed as treatment options for these injuries, but debate remains as to which approach is better. QUESTIONS/PURPOSES: We asked whether ORIF or primary fusion led to (1) fewer reoperations for hardware removal; (2) less frequent revision surgery; (3) higher patient outcome scores; and (4) more frequent anatomic reduc

Residual mobility of instrumented and non-fused segments in thoracolumbar spine fractures.

The surgical management of thoracolumbar fractures presents potential benefits. However, the surgery solve the instability by fusion of mobile segments. We incorporate in our treatment algorithms, the use of restricted arthrodesis at injured levels, regardless of longer instrumentations, as well as the use of non-fused transitory stabilizations, based on the conviction that in non-fused segments without traumatic disc injury, mobility persists once the instrumentation is removed. The goals of this study were to compare the mobility of non-fused segments after hardware removal to a normal range

The incidence of postoperative wound healing problems following sagittal ramus osteotomies stabilized with miniplates and monocortical screws.

PURPOSE: Titanium plates and monocortical screws are commonly used to stabilize the mandible following sagittal split ramus osteotomies. Despite widespread use of this type of fixation, there is a paucity of large studies evaluating the infection rate and need for hardware removal. MATERIALS AND METHODS: This study is a retrospective cohort evaluation of 1,066 consecutive mandibular sagittal ramus osteotomies in 533 patients, performed between January 2002 and December 2003. All osteotomies were stabilized with 4-hole miniplates and 2.0 mm x 5.0 mm monocortical screws. Study variables include

An Unexpected Journey: Migration of a Scoliosis Rod to the Groin.

Rod migration is a rare but potentially serious late complication of spinal instrumentation. A case of long-distance rod migration to the groin following anterior lumbar fusion for adolescent idiopathic scoliosis is presented. A 43-year-old female, who had undergone selective anterior lumbar fusion 20 years earlier, presented with progressive lumbar pain. Initial radiographs demonstrated screw loosening and rod dislodgment. Before the planned revision surgery, the patient re-presented with pain localized to the hip and groin. Imaging revealed distal migration of the rod toward the anterior asp

Staged screw removal in external plate fixation for distal tibial fractures: a finite element study on adaptive biomechanics.

High-energy distal tibial fractures present treatment challenges due to poor soft tissue coverage and risk of delayed union. External locking plates offer stability while preserving biology, yet the optimal strategy to dynamically modulate fixation rigidity in accordance with healing progression remains unclear. We constructed finite element models of a comminuted distal tibial fracture incorporating callus at four healing stages (0, 1, 3, and 6 months). Seven screw configurations and the effect of an intact fibula were evaluated under 800 N axial load. Outcome measures included fracture site
