Preferences for harm reduction vending machine design among rural people who use drugs.

Globally, harm reduction vending machines (HRVMs) have been used for decades to dispense safe injection equipment and other supplies to people who use drugs (PWUD). However, HRVMs have only recently scaled up in the U.S. and few operate in rural settings. This study describes preferences for HRVM design among rural PWUD in two Appalachian Kentucky communities. Peer-referral and outreach were used to recruit participants (n = 731). Eligibility criteria were ≥ 18 years, residing in the target counties, and having used any drug (excluding alcohol, cannabis, and tobacco) to get high within 6 month

Public Understanding and Perception of Harm Reduction and Prevention Messaging About Fentanyl Overdoses.

BACKGROUND: Fentanyl-related opioid fatalities have risen drastically in the United States, indicating a "new wave" of the opioid crisis and highlighting the urgent need for more effective public health interventions to address its harms. Despite an increasing number of public communication campaigns focused on the general public, evidence on how people perceive fentanyl-related harm reduction strategies and prevention messaging is still nascent. METHODS: We conducted a cross-sectional survey with a national sample (N = 1,044). Fentanyl-related information seeking, risk perception, and messag

Honor Norms Predict Lower Support for Opioid Harm Reduction Policies in the U.S.

Harm reduction services are empirically based strategies aimed at counteracting drug overdoses and other drug-related harms. Despite the various harms associated with heroin use as well as other drug use, the accessibility of these services varies across the United States. In this study, we explored the role honor norms might contribute to reduced support for harm reduction programs in the United States. We conducted a web-based survey of a sample of adults living in the United States (N = 376) that assessed honor norms, controllability beliefs about heroin use disorder, and support for harm r

Individual-level harm reduction intervention coverage among people in Australia who inject drugs.

BACKGROUND: Needle and syringe programs, opioid agonist treatment (OAT), and take-home naloxone (THN) are key harm reduction interventions that reduce bloodborne virus transmission and/or overdose. In this study, we provide updated individual-level coverage of these measures among people who inject drugs in Australia. METHODS: Data from cross-sectional surveys in 2018 and 2023 were used to report the percentage who: (a) had sufficient sterile needle/syringes to cover all past month injections; (b) received the monthly equivalent of the WHO recommended 200 needle/syringes per year; (c) reused

Borne of necessity: Pharmacy-based harm reduction and express sexually transmitted infection services.

BACKGROUND: HIV, hepatitis C virus (HCV), Sexually Transmitted Infections (STIs), and substance use disorder are interrelated epidemics. Augmented services to respond to this "syndemic" are hampered by shortages of health care workers, especially in rural areas. In an Indian Health Service hospital in rural Minnesota, the pharmacy sought to integrate harm reduction and express STI services into its scope of practice. OBJECTIVES: Provide pharmacy-based harm reduction and express STI services to increase access to care for community members, especially those without a primary care provider. ME

Understanding Clinician Knowledge, Attitudes, and Practices Relating to Nonpharmaceutical Fentanyl and Harm Reduction.

BACKGROUND: Nonpharmaceutical fentanyl (NPF) is driving the national epidemic of opioid overdose deaths. Clinicians can play a role in fostering awareness of this growing risk and delivering interventions to reduce mortality. However, there is limited research assessing clinician knowledge, attitudes, and practices relating to NPF and harm reduction strategies. METHODS: A 34-question survey was designed to assess knowledge, attitudes, and practices related to NPF and harm reduction strategies of adult and pediatric hospital-based and emergency clinicians at a single academic medical center. Applying the Canadian Low-Risk Gambling Guidelines to Gambling Harm Reduction in England.

There is a need for evidence-based guidelines for gamblers who wish to reduce their risk of harm by setting self-directed limits on their gambling. Recognizing this, the Canadian Low-Risk Gambling Guidelines were developed using data from 8 countries to establish the relationship between gambling behaviour and harm. The guidelines include recommended limits on gambling spending as a percentage of income, gambling frequency, and number of types of games played. However, the developers of the LRGG's did not include UK data in their analysis. This study analyzes data from Health Survey England to

Harm reduction behaviors are associated with carrying naloxone among patients on methadone treatment.

BACKGROUND: Despite the widespread availability of naloxone, US opioid overdose rates continue to rise. The "Cascade of Care" (CoC) is a public health approach that identifies steps in achieving specific outcomes and has been used to identify gaps in naloxone carriage among individuals with opioid use disorder (OUD). We sought to apply this framework to a treatment-seeking population with OUD that may be more inclined to engage in harm reduction behaviors. METHODS: Patients were recruited from an urban methadone program to complete a survey. We assessed naloxone familiarity, availability, obt

Examining buprenorphine diversion through a harm reduction lens: an agent-based modeling study.

BACKGROUND: Recent policies have lessened restrictions around prescribing buprenorphine-naloxone (buprenorphine) for the treatment of opioid use disorder (OUD). The primary concern expressed by critics of these policies is the potential for buprenorphine diversion. However, the population-level effects of increased buprenorphine diversion are unclear. If replacing the use of heroin or fentanyl, use of diverted buprenorphine could be protective. METHODS: Our study aim was to estimate the impact of buprenorphine diversion on opioid overdose using an agent-based model calibrated to North Carolin

Electronic harm reduction interventions for drug overdose monitoring and prevention: A scoping review.

BACKGROUND: Novel strategies are required to address rising overdose deaths across the globe. We sought to identify the breadth and depth of the existing evidence around electronic harm reduction (e-harm reduction) interventions that aimed to reduce the harms associated with substance use. METHODS: We conducted a scoping review according to the PRISMA-ScR and PRISMA for Searching guidelines. A health sciences librarian systematically searched seven health databases from inception until January 20, 2023. Citation chaining and reference lists of included studies were searched to identify additi

Known fentanyl use among clients of harm reduction sites in British Columbia, Canada.

BACKGROUND: North America is in the midst of an opioid overdose epidemic and it is commonly suggested that exposure to fentanyl is unknown. Using a provincial survey of harm reduction site clients, we aimed to characterize known and unknown fentanyl use and their correlates among people who use drugs in British Columbia, Canada. METHODS: We recruited 486 clients who were >18 years old and 316 agreed to provide a urine sample for substance use testing. Reported known fentanyl use was defined as a three-level categorical variable assessing recent (i.e., in the previous three days) fentanyl expo
