Transcatheter occlusion of an anomalous origin of left coronary artery from pulmonary artery in an adult as an alternative to surgery.

Anomalous origin of the left coronary artery from the pulmonary artery causes heart failure and death in infancy. In rare adult survivors with well-developed collaterals, surgical left coronary ligation to arrest steal is often combined with bypass grafting. Transcatheter left coronary artery closure in a symptomatic adult as an alternative to surgical ligation resulted in complete resolution of inducible ischaemia on myocardial perfusion imaging.

[Severe left ventricular dysfunction caused by multivessel coronaropathy in a 27-year-old man at low risk of cardiovascular complications from atherosclerosis].

When chronic heart failure develops in a young adult without history of ischemic heart disease, differential diagnosis between coronary artery disease and idiopathic dilated cardiomyopathy may be difficult. We describe a case of chronic heart failure in a 27-year-old male, apparently due to a dilated cardiomyopathy on the basis of clinical and instrumental data. Subsequent coronary angiography showed a severe tri-vascular coronary artery disease. After coronary bypass surgery, clinical and echocardiographic examinations showed relevant improvement in left ventricular global and regional motion

[The heterotopic heart transplantation: first clinical experiences (author's transl)].

Ten patients, all men, have been operated of heterotopic heart transplantation since the end of 1974. Ages ranged from 59 to 24 years with an average of 38 years. Six of them suffered from ischaemic heart disease, two from cardiomiopathy and two from rheumatic heart disease. All of them presented end stage cardiac disease. The first two patients underwent to left ventricular bypass only, in the remaining eight a right and left bypass was performed. At the present time, June 1977, 6 of the original 10 patients are alive all of them in functional class I. Five of them have resumed a full time jo

A Novel Modified Off-Pump Linear Closure Technique of Left Ventricular Aneurysm - A Case Report.

Left ventricular aneurysm is an important mechanical complication of myocardial infarction, and its reported incidence after myocardial infarction varies between 10 and 35%. Left ventricular aneurysms in patients with angina pectoris, congestive heart failure, malignant ventricular arrhythmias, and systemic embolization should be surgically repaired. In this paper, we present a novel modified off-pump linear closure technique performed by using a simple Foley catheter for hemostasis on beating heart without cardiopulmonary bypass for the surgical treatment of left ventricular aneurysm. To the

[Redo coronary artery bypass grafting after pneumonectomy].

A 77-year-old man who had undergone left pneumonectomy 16 years and quadruple coronary artery bypass grafting (CABG) 4 years previously presented with unstable angina pectoris. Coronary angiography revealed severe stenosis of the left main trunk and the proximal left anterior descending artery(LAD), and the severely narrowed left internal thoracic artery (LITA)-LAD graft. Computed tomography(CT) presented a marked shift of the heart and great vessels into the left hemithorax with a hyperinflated right lung crossing the midline. The LAD would not seem to be accessible by median sternotomy. Off-

Primary in-series palliation of hypoplastic left heart syndrome with mechanical lung assist in neonatal pigs.

Less than optimal outcomes for hypoplastic left heart syndrome may be related to unstable in-parallel circulation. Mechanical lung assist by (1) superior vena cava to right atrium pump with oxygenation (oxygenator assist), or (2) superior vena cava to pulmonary artery pump (pump assist) may permit successful neonatal in-series palliation. Nineteen 15-day-old piglets underwent single ventricle and bidirectional cavopulmonary shunt (BCPC) creation without mechanical lung assist (n = 8), with pump assist (n = 5), or with oxygenator assist (n = 6). Baseline hemodynamic measurements were not differ

Transcatheter aortic valve implantation through a diseased left common carotid artery: combined approach with endarterectomy and left carotid-subclavian bypass.

We report a case of transcatheter aortic valve implantation (TAVI) with the self-expanding Medtronic CoreValve bioprosthesis (Medtronic, Minneapolis, MI) through a diseased left common carotid (LCC) artery. This 81-year-old male patient presented with heart failure due to a severe degenerative aortic valve stenosis. Comorbidities included diabetes, hypertension, and dyslipidemia as well as peripheral and coronary artery disease, resulting in a logistic EuroScore II of 25.9%. Consequently, he was rejected to undergo surgery and a transcatheter approach was planned. Due to severe peripheral vasc

Effect of left ventricular septal myectomy on concurrent mitral regurgitation.

To assess the impact of left ventricular septal myectomy (Morrow procedure) performed for obstructive hypertrophic subaortic stenosis on co-existing mitral regurgitation, we examined the preoperative and postoperative left ventriculograms of 35 patients (13 of them women) who underwent left ventricular septal myectomy alone or with concomitant operation. The mean age was 45 +/- 3 years (range, 19 to 74 years). Patients underwent left ventriculography at an average of 15 +/- 3 months postoperatively (range, 1 to 78 months). Severity of mitral regurgitation was evaluated by two teams of reviewer

Combined aortic valve replacement and extra-anatomic aorta ascending-descending bypass for recurrent aortic coarctation.

Aortic recoarctation may be difficult to manage, especially in patients with the need for additional surgical interventions. We describe the case of a 45-year-old man with grown-up congenital heart disease, presenting with recoarctation after initial anatomic repair with a prosthetic graft at the age of 16 years. Further examination revealed heart failure and left ventricular dilation because of severe aortic regurgitation, owing to a bicuspid aortic valve. Through a median sternotomy and CPB, an aortic valve replacement and extra-anatomic ascending-to-descending aortic-bypass-grafting was per

How to improve flow during cardiopulmonary bypass in an acardia experimental model.

OBJECTIVES In extreme scenarios, such as hyperacute rejection of heart transplant, an urgent heart explantation might be necessary. The aim of this experimental study was to determine the feasibility and to improve the haemodynamics of a venoarterial cardiopulmonary bypass after cardiectomy. METHODS A venoarterial cardiopulmonary bypass was established in seven calves (56.4 ± 7 kg) by the transjugular insertion to the caval axis of a self-expanding cannula, with a carotid artery return. After baseline measurements (A), ventricular fibrillation was induced (B), great arteries were clamped (C),

Right ventricular infarction with tricuspid insufficiency and chronic right heart failure.

A patient with chronic right heart failure and probable tricuspid insufficiency associated with an inferior myocardial infartion is described. Angiograms demonstrated total occlusion of the right coronary artery at its origin, a patent venous bypass graft to the mid-right coronary artery and hypokinesia of the inferior wall of both the right and left vebtricles. Clinical data indicated a greater impairment of right than of left ventricular function. It is proposed that infarction of the right ventricle resulted in chronic right heart failure and tricuspid insufficiency.

Norwood operation without circulatory arrest: a new surgical technique.

Circulatory arrest (CA) is associated with potential neurologic injury. We have developed a new surgical technique to eliminate CA during the Norwood operation. A modified Blalock-Taussig shunt (BTS) was fully constructed before cannulation for cardiopulmonary bypass. The aortic cannula was inserted in the patent ductus arteriosus to allow systemic cold perfusion. When deep hypothermia was reached, the aortic cannula was redirected into the pulmonary artery (PA) confluence. Both cerebral and systemic perfusion were maintained through the right PA and BTS into the innominate artery.

Beating-heart surgical treatment of atrial fibrillation with microwave ablation.

BACKGROUND: In this feasibility study, early results are presented of our first series of patients with microwave ablation for atrial fibrillation (AF) on the beating heart. METHODS: From June 2001 until December 2001, a total of 24 patients underwent beating-heart epicardial ablation for AF. With a microwave antenna, the left and right pulmonary veins were isolated and connected to each other followed by amputation of the left atrial appendage. Subsequently, patients underwent either off-pump coronary artery bypass graft or valve surgery on pump. The mean age of the patients was 67.4 +/- 6 y

Unprotected left main stent placement in a patient with Takayasu's arteritis: an unusual solution for an unusual disease.

We describe the case of a young woman with Takayasu's arteritis that initially manifested as heart failure due to left main coronary artery stenosis. The patient's occluded subclavian artery and the active inflammatory process of Takayasu's arteritis precluded coronary artery bypass grafting with the use of arterial grafts. Therefore, a drug-eluting stent was placed in the unprotected left main artery. This procedure resulted in the resolution of symptoms, with a patent stent and no new coronary lesions observed on 3-month angiography, and normal left ventricular function on 9-month echocardio

Left thoracotomy reoperation for coronary artery disease.

Twenty patients underwent reoperative coronary artery bypass grafting (CABG) through a left thoracotomy since 1971. This was their second CABG in 16 patients, third in three patients and fourth in one patient. Surgery was performed from 1 to 16 years following the initial procedure. Demographic data showed no significant variation from patients undergoing standard reoperative CAGB in this institution. Ejection fraction varied between 30% and 73%. Cardiopulmonary bypass technic has gradually developed since 1971, using the left femoral artery and vein. For venous cannulation a 50-cm long cathet

The Case of the "Disappearing Ventricle": A Report.

Embolization of entrapped intracardiac air represents a significant risk to the patient undergoing open-heart surgery. Entrapment of as little as 0.5 mL of gas in the heart can cause temporary myocardial dysfunction, cardiac arrhythmias, and systemic emboli. In contrast, larger emboli can disrupt the evaluation of heart function by limiting visualization during echocardiography. We present the case of a 67-year-old male who presented with dizziness, nausea, and chest pain. A left heart catheterization revealed multi-vessel disease. Undergoing general anesthesia, the patient received three-vess

Hybrid Approach to a Right Aortic Arch With a Large Kommerell's Diverticulum.

A 60-year-old woman with a family history of aortic dissection in her mother was investigated for dysphagia. Her scan showed a right aortic arch with a large Kommerell's diverticulum causing esophageal compression. Her left vertebral artery, which originated abnormally low in the thorax, was embolized preoperatively to limit operative difficulties. Two weeks later, a right thoracotomy allowed the resection of the aneurysm with an associated left carotid-axillary bypass. Partial cardiopulmonary bypass in moderate hypothermia with beating-heart perfusion of the brain was conducted. Five-year fol
