[Rupture of the sinus of Valsalva aneurysm complicated by infective endocarditis -- diagnosis difficulties].

Aneurysm of the sinus of Valsalva is a rare congenital heart disease. It constitutes about 0.14%-1.5% of congenital heart surgery cases. Aneurysm of the sinus of Valsalva can be asymptomatic for many years. Only rupture of the sinus of Valsalva aneurysm leads to hemodynamic disturbances which in turn causes rapid deterioration of patient clinical condition. Some of the congenital heart diseases can be complicated by of infective endocarditis. This case report concerns the rupture of the sinus of Valsalva aneurysm in a 45 year old woman, which caused infective endocarditis on the tricuspid valv

[Ruptured Aneurysm of the Sinus of Valsalva Accompanied with a Bicuspid Aortic Valve in an Elderly Man;Report of a Case].

The combination of ruptured aneurysm of the sinus of Valsalva and a bicuspid aortic valve is very rare in an elderly person. A 71-year-old man with ruptured aneurysm of the sinus of Valsalva and a bicuspid aortic valve had undergone an operation. He was admitted to his other hospital because of heart failure. He was transferred to our hospital to undergo treatment for ruptured aneurysm of sinus of Valsalva. At our hospital, echocardiography findings showed ruptured aneurysm of the sinus of Valsalva, a ventricular septal defect (VSD), and severe aortic regurgitation with moderate stenosis of th

Rupture of a dissecting aneurysm into the superior vena cava in Marfan's syndrome.

Aneurysmal dilatation of the aorta with subsequent dissection or rupture occurs frequently in patients with Marfan's syndrome. These complications are among the major causes of death. We report the case of a 51-year-old man with annulo-aortic ectasia in Marfan's syndrome. Acute aortic dissection and rupture into the superior vena cava occurred 8 years after aortic valve replacement. The preoperative diagnosis was made by right heart catheterization and computed tomography. A markedly increased left-to-right shunt occurred with rapid enlargement of the fistula due to the fragility of the aortic

Mitral valve chord rupture in a pregnant patient with uncontrolled hyperthyroidism.

SUMMARY: Graves' disease can have multiple cardiac manifestations. A rare complication is that of severe mitral regurgitation secondary to mitral valve chordae rupture, due to both compromise of valve integrity by deposition of glycosaminoglycans and the hemodynamic stresses of thyrotoxicosis. Pregnancy, with its related hemodynamic changes, is another setting in which mitral valve chordae rupture has occasionally been documented. We present a unique case of a 36-year-old female with uncontrolled Graves' disease who presented during pregnancy at 13 weeks gestation with atrial flutter and featu

[Use of 2-dimensional echocardiography in the diagnosis of interventricular septum rupture following myocardial infarction].

Interventricular septal rupture is a rare complication of the acute myocardial infarction. We report a case with rupture 4 weeks after diaphragmatic wall necrosis. The patient developed heart failure, precordial murmur and clinical signs of left-to-right shunt. The septal rupture was detected by two-dimensional echocardiogram and confirmed by catheterization. The surgical correction of the defect, was successful.

[Forensic expertise of thoracic aorta, heart and pericardial injuries in car-occupant fatalities].

INTRODUCTION: Forensic expertise has not specified with certainty any specific injury among fatally injured frontal car-occupants in frontal car collisions. OBJECTIVE: To establish if blunt injuries of thoracic aorta, heart, and pericardium could be used as predictors where the fatally injured car-occupant was at the moment of car-collision. METHODS: A retrospective autopsy study was performed. The subjects were fatally injured car-drivers, front-seat and rear-seat passengers in head-on car collisions. In each of them we analyzed the injuries of thoracic aorta, heart and pericardium. RESULT

The prevalence and natural history of aortic aneurysms in heart and abdominal organ transplant patients.

OBJECTIVE: The purpose of this study was to document the prevalence and clinical features of aortic aneurysms in heart and abdominal transplant patients. METHODS: We undertook a retrospective review of 1557 patients who had heart, liver, or kidney transplantation between January 1, 1987, and December 31, 2000. Aortic aneurysms were identified by computed tomographic scan, ultrasound scan, or at the time of surgery for rupture. An aortic diameter of 3.5 cm was used as the threshold for the definition of aneurysmal disease. We compared dichotomous variables with Fisher's exact test and continuo

Contained myocardial rupture: a variant linking complete and incomplete rupture.

Myocardial rupture is an uncommon complication of myocardial infarction, often with devastating haemodynamic consequences. Although rupture is usually fatal, when patients do survive, the majority present with a pseudoaneurysm in which the rupture is sealed by a haematoma on the epicardial surface of the heart. Cases in which all myocardial layers are dissected except the epicardium or visceral pericardium have been included under this subheading. The authors describe such a case and suggest the pathological description of a "contained myocardial rupture". This link between complete and incomp

Ectopia cordis and cleft sternum: evidence for mechanical teratogenesis following rupture of the chorion or yolk sac.

We present case material and a literature survey to document the association between ectopia cordis and band disruption anomalies. The occurrence of thoracic ectopia cordis with a cephalic-pointing cardiac apex suggests an arrest of cardiac descent at 3 weeks of development, consistent with our finding of ectopia cordis in a 28-day human embryo. Mechanical compression secondary to rupture of the chorion and/or yolk sac at 3 weeks of gestation would interfere with normal cardiac descent and compress the chest, yielding thoracic and pulmonary hypoplasia. Congenital heart defects associated with

[Spontaneous rupture of an abdominal aortic aneurysm into the inferior vena cava--case report].

A 76-year-old man was admitted to the surgical department with acute abdominal pain and impaired sensation of the lower extremities. An aneurysm of the abdominal aorta (AAA) was diagnosed already in the past. There were no signs of cardiovascular failure. Examination (sonography, CT) did not show intraabdominal bleeding. Nevertheless AAA rupture was suspected. A decision on an urgent operation was taken. Despite permanent resuscitation the patient died on the table before the operation began. In the discussion four type of AAA rupture are mentioned: into the open abdominal cavity, into the ret

The floating heart or the heart too fat to sink: analysis of 55 necropsy patients.

Certain clinical and morphologic findings are described in 55 patients whose hearts at necropsy contained so much fat that they floated in water. The patients were 47 to 89 years old (mean 67). Symptomatic coronary heart disease was present in 28 (51%) and valvular heart disease (mitral stenosis) in 3 (5%). The heart at necropsy was enlarged (greater than 350 g for women and greater than 400 g for men) in 45 patients (82%). The mean heart weight for the 31 women was 470 g and for the 24 men, 515 g. In addition to the severe increase in fat in the atrioventricular sulci and over both ventricles

Rupture of a left sinus of Valsalva aneurysm into the pulmonary artery.

Sinus of Valsalva aneurysm is a rare congenital defect and the diagnosis is usually made after it ruptures. Very few cases of non-complicated sinus of Valsalva aneurysm have been published. The anomaly most often involves the right coronary sinus and less frequently the non-coronary sinus. A case with left sinus of Valsalva has not been reported. Since the aortic valve occupies a central position in the base of the heart, rupture of a sinus of Valsalva aneurysm can occur in any of the four heart chambers. Rupture into the pulmonary artery is very rare and is mostly associated with other congen

[Left Ventricular Rupture after Mitral Valve Replacement].

Ventricular rupture after mitral valve surgery is rare but a serious complication associated with high mortality rate. Of the 2,338 patients who underwent mitral valve surgery, 8 patients (0.7%) suffered from left ventricular rupture in our institution. All developed left ventricular rupture after mitral valve replacement and 3 patients( 37.5%) died within 30 days. To prevent left ventricular rupture, preservation of the mitral loop, appropriate valve sizing, and perioperative hemodynamic management to unload ventricular pressure are needed. Surgical repair for left ventricular rupture should

Ventricular septal perforation following acute myocardial infarction in a patient with hypothyroidism--a case report.

A 61-year-old woman with profound hypothyroidism underwent surgery for a ventricular septal perforation 15 days following an acute myocardial infarction, with no preoperative thyroid replacement. Her immediate postoperative course was complicated by a low cardiac output, respiratory suppression and high output renal failure, requiring prolonged hemodynamic and respiratory support. Thyroid replacement (50 micrograms of thyronine and 100 micrograms of thyroxine daily) was given in two installments on the 2nd day after the operation. The effect of the inotropic agents increased gradually and the

Anterolateral Papillary Muscle Rupture Revealing Infective Endocarditis.

The rupture of mitral papillary muscles is a very rare complication of infective endocarditis (IE). We report a case of anterolateral papillary muscle rupture resulting in severe mitral regurgitation due to IE in a young man without previous heart disease. The patient underwent urgent mitral valve replacement. The isolated rupture of the mitral papillary muscle complicating IE is rare. Urgent surgery should be performed is such cases.

[Elective Mitral Valve Repair for Acute Mitral Regurgitation Due to Anterior Papillary Muscle Rupture:Report of a Case].

An 83-year-old woman with congestive heart failure due to severe mitral regurgitation was referred to our department. Because acute coronary syndrome was suspected, the patient underwent emergent coronary artery angiography, which showed 75% stenosis of segment 2 and 90% stenosis of segment 11. Subsequently, segment 11 was treated by percutaneous coronary intervention. Additionally, transesophageal echocardiography findings showed a prolapse of P2 due to papillary muscle rupture. After management of heart failure, a scheduled operation was performed under the diagnosis of acute mitral regurgit

A rare cause of sudden dyspnea and unexpected death in adolescence: fistula from aortic sinus of Valsalva to right atrium.

Aneurysm of the sinus of Valsalva is a rare cardiac abnormality. Unruptured sinus of Valsalva aneurysm is usually asymptomatic, however, a ruptured aneurysm typically leads to an aortocardiac shunt and progressively worsening heart failure. We report a case of a 17-year-old male who suffered an aneurysm of the sinus of Valsalva rupture into the right atrium who manifested sudden dyspnea and acute heart failure.
