[Papillary muscle rupture].

Papillary muscle rupture (PMR) is a rare, but catastrophic mechanical complication of acute myocardial infarction (AMI). Patients with PMR present in acute pulmonary edema and commonly in cardiogenic shock. The absence of new heart murmur after AMI dose not exclude the diagnosis. To diagnose PMR accurately, transesophageal echocardiography is the most useful and essential, coronary angiography is also necessary for appropriate surgical treatment. Based on these results of diagnostic procedure, combined myocardial revascularization and mitral operation should be performed without delay. Althoug

Pitfalls in the diagnosis and management of papillary muscle rupture: a study of four cases and review of the literature.

Four cases of papillary muscle rupture occurring in the setting of acute myocardial infarction are presented, which illustrate the following points: the diagnosis may not be apparent at presentation, a mitral regurgitant murmur may be absent, transesophageal echocardiography may establish the diagnosis when transthoracic echocardiography does not, and appropriate surgical correction can lead to excellent functional recovery.

[Surgery for acute myocardial infarction].

We discuss the current status and outcome of surgery for acute myocardial infarction (AMI). The optimal timing of surgical revascularization following AMI is a matter of controversy. Early surgery after an AMI involves high risk If elective surgery is possible under mechanical cardiac support cardiac artery bypass grafting (CABG) can be performed with acceptable mortality rates early after AMI. On-pump beating heart revascularization is efficacious in patients in cardiogenic shock or with unstable hemodynamics early after AMI. Spontaneous closure of left ventricular free-wall rupture after acute myocardial infarction: a case report and review of the literature.

BACKGROUND: Left ventricular free-wall rupture (LVFWR) is a rare but often fatal complication of acute myocardial infarction (AMI). While typically requiring emergency surgical intervention, spontaneous healing is exceptionally uncommon. This case is noteworthy for the spontaneous resolution of a left ventricular rupture in a hemodynamically stable patient, successfully managed with multimodality imaging and selective surgical revascularization. CASE PRESENTATION: A 66-year-old white European male with a history of hypertension and smoking presented with non-ST elevation myocardial infarction

[Surgical correction of complete rupture of the anterior papillary muscle complicating acute myocardial infarction].

A 71-year-old female patient with complete post infarction rupture of the anterior papillary muscle and severe congestive heart failure is presented. The diagnosis was determined by echocardiography and cardiac catheterization. Two days after the acute episode, the patient underwent emergency mitral valve replacement with an ATS 27 M prosthetic valve. The mitral annulus was edematous and friable. Postoperatively, the patient improved dramatically. In this case, two factors were important-repairing the nonfunctioning mitral valve as soon as possible and handling the friable mitral annulus with

[A case of scheduled mitral valve replacement for mitral regurgitation caused by papillary muscle rupture after acute myocardial infarction].

Mitral regurgitation caused by papillary muscle rupture after acute myocardial infarction has poor prognosis, and it is the usual course for such cases in cardiogenic shock to be performed early mitral valve replacement. However, we recently experienced a case of successful scheduled mitral valve replacement for mitral regurgitation caused by rupture of the posterior papillary muscle after acute inferior myocardial infarction. The patient was a 63-year-old woman who admitted to a hospital with back pain and dyspnea and was diagnosed as pulmonary edema due to mitral regurgitation after acute my

[Myocardial rupture after acute myocardial infarct: 2 cases with an unusual clinical presentation].

Myocardial rupture is the second most common reason for in-hospital mortality in patients with acute myocardial infarction, accounting for 8-17% of deaths. The clinical presentation varies due to the possibility of rupture in three main locations: free left ventricular wall (85%), interventricular septum (10%), and papillary muscle (5%). Hypotension, long persisting or repeated chest pain, syncopes, new heart murmurs or weak action should draw attention to the possibility of myocardial rupture, apart from the classical sign of upper inflow congestion. In about 48% of cases immediate surgical i

[Emergency CABG and mitral valve replacement for anterolateral papillary muscle rupture after acute myocardial infarction].

A 74-year-old man developed sudden cardiogenic shock 5 days after the onset of acute myocardial infarction. Echocardiographic diagnosis was severe mitral regurgitation due to papillary muscle rupture. Despite the effort to support the hemodynamics with catecholamines and IABP, the patient's condition deteriorated rapidly, which necessitated emergency operation. Anterolateral papillary muscle was found to be totally ruptured. Coronary artery revascularization and mitral valve replacement were performed. Postoperative course was uneventful, with two days of IABP and three days of ventilatory sup

TGFBI functions similar to periostin but is uniquely dispensable during cardiac injury.

Extracellular matrix production and accumulation stabilize the heart under normal conditions as well as form a protective scar after myocardial infarction injury, although excessive extracellular matrix accumulation with long-standing heart disease is pathological. In the current study we investigate the role of the matricellular protein, transforming growth factor beta-induced (TGFBI), which is induced in various forms of heart disease. Additionally, we sought to understand whether TGFBI is functionally redundant to its closely related family member periostin, which is also induced in the dis

[Post-myocardial infarction ventricular septal defect].

Post-myocardial infarction ventricular septal defect corresponds to the rupture of the ventricular septum between the healthy and infarcted parts. It is a rare complication still associated with a high mortality rate. Its diagnostic should be evoked in case of pathologic cardiac auscultation and confirmed by emergent transthoracic echocardiography. Hemodynamic stabilisation, mainly with the insertion of an intra-aortic balloon pump is the first step in the management. The subsequent modality of closure, either surgical or transcatheter, as well as the ideal timing should be discussed in the He

Surgical techniques for emergent repair of post-infarction ventricular septal defect: compare endocardial patch and infarct exclusion method with traditional method.

BACKGROUND: The traditional surgical repair of post-infarction ventricular septal defect (VSD) includes excision of necrotic myocardium and approximation of the remaining of healthy ventricular wall and septal portion. The exclusion method emphasizes no excision of infarcted myocardium, preservation of the left ventricular geometry and exclusion of infarction area. We discuss our experiences in 13 patients and compared the results obtained from 2 different surgical methods. METHODS: From July 1996 to December 2001, 13 patients with post-infarction VSD received emergent repair. Seven patients

Percutaneous extracorporeal life support for treatment of fatal mechanical complications associated with acute myocardial infarction.

BACKGROUND: Vaccine hesitancy has intensified since COVID-19. In Türkiye, a performance-based family medicine system provides free vaccines. Before May 2024, physicians could remove refusals from metrics tied to pay deductions, obscuring true rates. A new policy mandating documentation created a natural experiment. We used population-based data to quantify these previously hidden dose-specific refusal rates across the childhood schedule. METHODS: We conducted a population-based ecological analysis of monthly dose-specific refusals among children aged 0-59 months in Balıkesir Province, in 2024

[Traumatic heart rupture or infarct?].

A 70-year-old motorist driving on the wrong side of the road caused a car-to-car head-on collision and died some minutes later. The autopsy revealed a lethal rupture of the right heart ventricle. The insurance company argued that driving on the wrong side was an act of gross negligence so that the damage was not covered by the full comprehensive insurance. Surprisingly, the microscopic examination of the myocardium performed by a clinical pathologist brought to light a severe myofibrillar degeneration (MFD) which was interpreted as diagnostic of acute myocardial infarction. It was supposed tha

Cardiac Lymphoma Presenting with Recurrent STEMI.

The incidence of primary cardiac tumors is exceedingly rare, whereas secondary cardiac tumors are more common in the global population. Cardiac involvement is seen in approximately 18% of patients with non-Hodgkin's lymphoma at the time of autopsy. Clinical manifestations of cardiac involvement are subtle and often go unrecognized until advanced stages of the disease. We present a rare case of metastatic cardiac lymphoma that presented as an ST-segment elevation myocardial infarction complicated by left ventricular free wall rupture and cardiogenic shock due to transmural myocardial necrosis f

Late left ventricular pseudoaneurysm formation following subacute myocardial infarction.

We describe a patient with a subacute inferior myocardial infarction who developed a pseudo-aneurysm more than 18 days after the acute event. This is an unusual case with three different complications of a myocardial infarction: Firstly, ventricular rupture is usually the result following transmural myocardial infarction without reperfusion. However, coronary angiography confirmed reperfusion after late thrombolysis in this patient. The subacute rupture could potentially be caused or aggravated by the late thrombolysis. Secondly, this patient developed a mural apical thrombus in a non-infarcte

Transcatheter closure of residual postinfarction ventricular septal defect after dehiscence of surgical patch repair.

Ventricular septal defect (VSD) is a life-threatening complication of transmural myocardial infarction. Urgent surgical repair and concomitant revascularization are the standard of care. Percutaneous catheter-based closure techniques have been reserved for patients with a high-risk surgery or a failed surgical procedure with residual shunting. This case report demonstrates the successful transcatheter closure of residual VSD using the Amplatzer muscular VSD device (Amplatzer, Minnesota, USA) after surgical patch dehiscence for postinfarction VSD and 3-and-a-half years' post-intervention follow

Necroptosis is a key contributor to impaired cardiac repair following myocardial infarction.

Myocardial infarction (MI) is a leading cause of morbidity and death worldwide. Endothelial cells (ECs) contribute to post-MI remodeling through angiogenesis, inflammation, and endothelial-to-mesenchymal transition (EndMT). ADAM17, a membrane-bound protease, is upregulated in ischemic heart disease, but its role in endothelial function post-MI is unknown. We investigated whether loss of endothelial ADAM17 could improve post-MI recovery using male and female mice with inducible endothelial-specific ADAM17 knockdown (Adam17f/f/Cdhr5-CreERT2; Adam17EC-KD). Surprisingly, male Adam17EC-KD mice exhi
