Helicobacter pylori infection and gastric cancer.

Gastric cancer remains a major health burden on many societies claiming hundreds of thousands of lives every year. The discovery of Helicobacter pylori has no doubt revolutionised our understanding of this malignancy, which is now regarded as a paradigm for infection-induced chronic inflammation-mediated cancer. In this paper, we discuss the evidence for the association between H. pylori and gastric adenocarcinoma and MALT lymphoma. We also discuss the pathogenesis of these two forms of cancer and the factors that determine their outcome. There is no doubt that the knowledge accumulated over t

The health economics of Helicobacter pylori infection.

Economic considerations are becoming increasingly important as health care becomes more expensive. Evidence for effectiveness is usually gained from randomised controlled trials (RCTs) but often there is insufficient evidence of the costs of alternative strategies in trials. Often, therefore, economic models are needed to extrapolate data from a variety of sources to give an indication of which strategy is cost effective. Helicobacter pylori (H. pylori) testing and treating in a wide variety of upper gastrointestinal diseases is a good example of the application of economic analyses to health

[Helicobacter pylori infection in elderly patients].

PURPOSES: Helicobacter pylori (H. pylori) infection is a chronic gastric Gram-negative infection that increases with age. Acquired in childhood, H. pylori infection may induce a whole cascade of events leading to gastric pathologies such as peptic ulcer diseases, gastric precancerous lesions, and gastric lymphomas (MALT). The characteristics of the diagnosis and the treatment of this infection in geriatrics are thus particularly important to take into account. MAIN POINTS: 1) The incidence of gastric and duodenal ulcers and their bleeding complications is increasing in old-aged populations. C

Colonization and infection by Helicobacter pylori in humans.

When Helicobacter pylori arrives in the human stomach, it may penetrate the mucin layer and adhere to the gastric epithelial cells or it may pass through the stomach without colonizing the mucosa. In this paper, the colonization process and the ensuing immunological response will be briefly described. Urease production is necessary for H. pylori to establish a pH-neutral microenvironment around the bacteria. The flagella enable the bacteria to move and the shape of H. pylori makes it possible to penetrate the mucin layer where it comes into contact with the gastric epithelial cells. H. pylori

Gastric plasmacytoma and Helicobacter pylori infection.

A 59-year-old man with a history of melena and upper abdominal pain was referred to our hospital. An upper endoscopy was performed, and a gastric ulcer was found bordering the antrum and stomach body. Multiple biopsies from the lesion showed monoclonal plasmacytic infiltration of the mucosa, consistent with the diagnosis of plasmacytoma. Helicobacter pylori was also identified. Triple therapy failed and quadruple therapy eradicated the H. pylori, confirmed by repeated biopsies. Healing of the gastric lesion followed the treatment. Multiple biopsies from the scar and the entire stomach showed c

Helicobacter pylori infection in children: a review.

H pylori infection is highly prevalent in asymptomatic children and it varies between countries and often within a country as well. Initial infection probably occurs at an early age and prevalence increases with age. Ethnic and racial factors, socio-economic status and living conditions affect the prevalence of infection. Long term population based studies are needed to identify the exact prevalence and clinical significance in Indian children. There is strong evidence for an association between H pylori infection and antral gastritis and duodenal ulcer disease in children, but it's associatio

[Epidemiology and pathogenesis of Helicobacter pylori infection].

Since Marshall and Warren reported the first isolation of Helicobacter pylori basic and clinical research on pathogenesis and epidemiology of Helicobacter pylori infection have been tremendous. Childhood is clearly established as the period of major risk for Helicobacter pylori acquisition. The transmission pathways may be several including the oral-oral, the gastro-oral or the fecal-oral transmission route. The colonization of Helicobacter pylori occurs exclusively in the gastric mucosa or in areas of gastric metaplasia especially in the duodenum. Helicobacter pylori possesses several factors

[Duodenal ulcer and Helicobacter pylori infection].

The research accomplishment of Australian investigators Marshall and Warren, published in 1983, has entirely changed our understanding of ulcer genesis and proposed a new strategy in diagnostics and therapy of peptic ulcer disease. The key etiopathogenetic role of Helicobacter pylori has been recognised. Marshall confirmed on himself that Helicobacter pylori, as an infectious agent, fulfilled all Koch's postulates. The bacterial infection caused the mucosal inflammatory changes in his stomach which were rapidly resolved by antibiotics. During later clinical and experimental investigations the

[Eradication therapy of Helicobacter pylori infection].

Helicobacter pylori(H. pylori) infection is recognized to be a pathogen of various gastro-duodenal diseases. Eradication therapy of H. pylori reduces the recurrence of gastro-duodenal ulcer, and improves gastritis histologically. Recently, proton pump inhibitor(PPI) based triple therapy, that combining PPI, clarithromycin, amoxicillin is widely accepted throughout the world, and shows high eradication rate which ranged about 80-90%. In Japan, one week triple therapy is recommended for the treatment of gastro-duodenal ulcer, though it is expected the improvement of recurrent peptic ulcer. In th

[Helicobacter pylori infection as a cause of hypergastrinemia].

With the aid of the radioimmunologic technique, the content was studied of blood serum gastrin in healthy patients, those presenting with chronic gastritis under exposure to Helicobacter pylori (Hp+) and free from the infection (Hp-). The secured results suggest to us that (Hp-) patients have hypogastrinemia with antral localization of the pathological process. In (Hp+) patients, blood serum levels of gastrin are noted to be significantly higher than the analogous indices in essentially healthy subjects, (Hp-) patients presenting with antral- and pangastritis.

"Familial hyperpepsinogenemia" and Helicobacter pylori infection.

OBJECTIVE: Pepsinogen 1 (PG1) is a proenzyme precursor to pepsin, a protease secreted by the gastric chief cell. PG1 levels correlate with maximal gastric acid output. In 1979, Rotter et al. reported two pedigrees in which elevated PG1 levels and duodenal ulcers were prevalent. They proposed autosomal dominant inheritance of elevated PG1 and suggested that it was a risk factor for duodenal ulcer disease. In 1982, Helicobacter pylori (Hp) was discovered and was shown to be an important factor in peptic ulcer disease. Hp infection is also associated with increased PGI levels. We tested serum fro

Treatment of Helicobacter pylori infection in children.

The majority of Helicobacter pylori (H. pylori) infections appear to be acquired during childhood. Despite this fact, the natural history of H. pylori infection in children, such as the mode of acquisition, the clinical symptoms and signs of infection and the appropriate treatment, is poorly understood. There is no consensus regarding which children with H. pylori infection deserve treatment nor is there agreement on the appropriate treatment regimen. This stems from the lack of controlled studies into H. pylori infection during childhood. For example, there have been no controlled studies to

Immunological aspects of Helicobacter pylori infection.

Host defence against Helicobacter pylori infection is a complex system of both specific and non-specific reactions. Among the non-specific defense mechanisms acting on bacteria before they reach the mucus layer in the stomach are digestive enzymes, lyzozyme, lactoferrin and other components with antimicrobal activity. The mucus layer is the final non-specific barrier against the bacteria reaching the gastric mucosa cells. On reaching the gastric mucosa Helicobacter pylori adheres to epithelial cells and bacterial antigens, chemotaxins and other components are liberated. Helicobacter pylori ant

Diagnosis and management of Helicobacter pylori infection.

Helicobacter pylori infects more than half of the world's population, making it one of the most prevalent infections. H pylori is now accepted as the most common cause of histologic gastritis and is responsible for the majority of cases of peptic ulcer disease and gastric cancer. Approximately 1 in 6 (17%) persons with H pylori infection will develop peptic ulcer disease, and each year 1% to 2% of these will experience a major or life-threatening complication, such as bleeding, perforation, or gastric outlet obstruction. Peptic ulcer disease should no longer be regarded as a chronic, recurring

Helicobacter pylori infection and skin diseases.

There is increasing evidence for systemic effects of gastric H. pylori infection which may result in extragastrointestinal disorders. This review summarizes the available medical literature up to September 1999, identified through a MEDLINE research including own studies, regarding H. pylori and skin diseases. Due to current knowledge best evidence for a potential link of H. pylori infection exists for chronic urticaria although the data are still conflicting. Thus, the search for H. pylori should be included in the diagnostic management of chronic urticaria. With regard to other skin diseases

Helicobacter infection in man: problems to be solved.

Helicobacter pylori (Hp) infection in man has several disease outcomes, varying from asymptomatic chronic gastric inflammation to Hp-associated dyspepsia, pepic ulcer disease, gastric adenocarcinoma, and Malt lymphoma. Particularly controversial is the role of Hp infection in the genesis of chronic dyspeptic symptoms. Only a small percentage of chronic dispeptics have long-lasting remission of the complaints after cure of the infection. It is now well established that healing of the inflammation through microbial eradication cures peptic ulcer disease. The high efficacy of bismuth or PPI tripl

[Colonic microenvironment in familial helicobacter infection].

To elucidate the significance of the familial microenvironment in the genesis of Helicobacter infection, a clinical and instrumental investigation was made of 13 families selected by the probands who had digestive diseases associated with H. pylori: gastroduodenitis and duodenal ulcer disease. The occurrence of Helicobacter infection and gastritis in the family members was ascertained to be largely determined by their concurrent residence in the limited area, i.e. by the way of life. The contribution of the "family" factor in antral gastritis, fundal gastritis, and H. pylori infection was 60.0
