Determinants of Functional Outcome after Pediatric Hemispherotomy.

OBJECTIVE: We aimed to evaluate determinants of functional outcome after pediatric hemispherotomy in a large and recent multicenter cohort. METHODS: We retrospectively investigated the functional outcomes of 455 children who underwent hemispherotomy at 5 epilepsy centers in 2000-2016. We identified determinants of unaided walking, voluntary grasping with the hemiplegic hand, and speaking through Bayesian multivariable regression modeling using missing data imputation. RESULTS: Seventy-five percent of children were seizure-free, and 44% stopped antiseizure medication at a 5.1-year mean follow

Cognitive Development in Pediatric Epilepsy Surgery.

Epilepsy surgery is a very effective treatment option for children and adolescents with drug-resistant structural epilepsy, resulting in seizure freedom in the majority of cases. Beyond seizure freedom, the postsurgical stabilization or even improvement of cognitive development constitutes a fundamental objective. This study aims to address key features of cognitive development in the context of pediatric epilepsy surgery. Many surgical candidates present with severe developmental delay and cognitive deficits prior to surgery. Recent studies support that global cognitive development remains st

Advanced treatments for childhood epilepsy: beyond antiseizure medications.

A substantial minority of children with epilepsy have continued seizures despite adequate trials of standard antiseizure medications. To maximize seizure control and thereby optimize their neurodevelopmental outcomes, alternate nonmedication therapies should be considered for these patients. Dietary therapies, including the ketogenic diet and its variations, have been available for years. With a recent resurgence in popularity and expansion of indications, these treatments can lead to freedom from seizures or a significantly reduced seizure burden for a large number of patients. For carefully

Approach to pediatric epilepsy surgery: State of the art, Part II: Approach to specific epilepsy syndromes and etiologies.

The second of this 2-part review depicts the specific approach to the common causes of pediatric refractory epilepsy amenable to surgery. These include tumors, malformations due to abnormal cortical development, vascular abnormalities and certain epileptic syndromes. Seizure freedom rates are high (usually 60-80%) following tailored focal resection, lesionectomy, and hemispherectomy. However, in patients in whom the epileptogenic zone overlaps with unresectable eloquent cortex, and in certain epileptic syndromes, seizure freedom may not be achievable. In such cases, palliative procedures such

Rasmussen encephalitis associated with segmental vitiligo of the scalp: clinicopathologic report.

A 3-year-old boy with segmental vitiligo, poliosis, and alopecia over the right side of the scalp developed epilepsia partialis continua involving the left side of the body and progressive atrophy of the right cerebral hemisphere. There was a right ear dysacusia and a perilimbal vitiligo associated with an area of iris depigmentation in the right eye. Pleocytosis and hypergammaglobulin were detected in cerebrospinal fluid. Because medical treatment that included phenobarbital, phenytoin, carbamazepine, oxcarbazepine, benzodiazepines, corticosteroids, gamma-globulin, and a ketogenic diet was in

Current status of epilepsy surgery.

In the United States, there are approximately 75,000 patients with medically intractable epilepsy of focal origin who are candidates for ablative surgery. Currently, about 1500 epilepsy operations are performed per year in the United States. Candidates for ablative surgery (i.e., removal of epileptogenic tissue) are those with a reasonable chance of reduction in seizure frequency and improvement of overall lifestyle. The epileptogenic tissue must be localized preoperatively by either non-invasive or invasive recordings, or by imaging studies (e.g., MRI). Nearly 60% of all patients who undergo

[A case of total hemipherectomy].

The author described a case of extensive total hemispherectomy extending to the red nucleus, gave a review of the literature of such conditions together with a brief historical review. He analysed in detail the secondary degenerative changes of the nervous tracts within the brainstem. The histological study of the remaining hemisphere, the cerebellum and the membranes revealed the following: 1) a false sub-dural membrane within the surgical space, proliferation of the arachnoidal cells and a granular ependymitis, changes which were interpreted as an attempt to fill the empty space resulting fr

Surgical management of Rasmussen's encephalitis: A systematic review and meta-analysis of long-term seizure and functional outcomes.

OBJECTIVE: Rasmussen's encephalitis (RE) is a rare, progressive inflammatory brain disease often leading to drug-resistant epilepsy and severe neurological decline. While surgery is the definitive treatment, evidence remains fragmented due to the rarity of the condition. We conducted a systematic review and meta-analysis to evaluate the efficacy of various surgical techniques and identify predictors of long-term seizure freedom. METHODS: We performed a comprehensive search of PubMed, Scopus, Web of Science, and Cochrane Library in accordance with PRISMA guidelines. We included studies reporti

[Lessons learnt from 101 hemispherotomies in children with symptomatic epilepsy. Part I: seizure outcome].

OBJECTIVE: To evaluate variables that may predict the outcome after hemispherotomy basing on a retrospective study of a large consecutive pediatric cohort of patients from a single institution. MATERIAL AND METHODS: One hundred and one patients with refractory seizures and variable decline in development (n=78) underwent hemispherotomy (med. age - 43 months, med. epilepsy history - 30 months). Developmental pathology was the anatomical substrate of disorder in 42 patients, while the infantile post-stroke scarring and gliosis was its origin in the majority of 43 cases with acquired etiology. Hemispherotomy for isolated infantile spasms following perinatal ischemic stroke.

BACKGROUND: Infantile spasms (IS) are a severe epileptic encephalopathy. In patients with early focal ischemia and refractory IS, a preoperative evaluation is required even if IS are the only ictal manifestation. METHODS: We report three such patients who presented with IS between 5 and 7 months of age without any other focal seizure types. Imaging exhibited a perinatal middle cerebral artery (MCA) stroke. RESULTS: All patients had hemiparesis and experienced psychomotor regression after the onset of IS. Scalp video-electroencephalogram (EEG) monitoring for presurgical assessment showed inte

Surgical strategies after failed hemispheric disconnection surgery: A systematic review and individual-patient data analysis.

PURPOSE: To analyze surgical and seizure outcomes after secondary interventions following failed hemispheric disconnection surgery (hemispherectomy/hemispherotomy). METHODS: We systematically searched five databases from January 1990 to April 2025. Eligible studies included patients of any age who underwent index hemispheric disconnection surgery (HDS), experienced seizure recurrence, and reported outcomes after secondary interventions: redo hemispheric surgery (RHS), laser interstitial thermal therapy (LITT), or vagus nerve stimulation (VNS). Studies were classified as aggregate or individua

Symptomatic epilepsy in children with poroencephalic cysts secondary to perinatal middle cerebral artery occlusion.

BACKGROUND: Perinatal cerebral artery occlusion is responsible for ischemic cerebral infarction leading to brain cavitation and gliosis; the territory of the middle cerebral artery is most frequently involved. The resulting poroencephalic cysts are frequently associated with hemiplegia and epilepsy; that can be managed medically in most cases, only 6-7% of them being refractory to medical treatment. This particular subset of congenitally hemiplegic children will be possible candidates for electrophysiological investigation and eventually for resective surgery. Whatever the kind of surgical tre

Transcranial direct current stimulation in adolescent and adult Rasmussen's encephalitis.

Rasmussen's encephalitis is a rare, progressive inflammatory disease that typically affects one cerebral hemisphere and causes intractable partial-onset seizures. Currently, the only effective therapy is hemispherectomy; however, this procedure is associated with irreversible neurological deficits. Novel therapeutic approaches to this condition are therefore necessary. One possible option that has not yet been extensively studied is electrical cathodal transcranial direct current stimulation (cTDCS). We describe the cases of two patients with atypical-onset Rasmussen's encephalitis who underwe

Mild hypothermia for refractory focal status epilepticus in an infant with hemimegalencephaly.

Hypothermia can reduce seizure frequency in animal models of status epilepticus, and its effectiveness in human status epilepticus has been reported occasionally. We report an infant with hemimegalencephaly who presented with generalized status epilepticus. After high dose intravenous drug therapy, this converted to focal status epilepticus in the right occipital region. A sudden cessation of all seizure activity was found to coincide with accidental hypothermia. After application of mild continuous hypothermia, a marked reduction of seizure frequency occurred, which allowed reduction of intra

Brain plasticity as a basis for recovery of function in humans.

One of the factors leading to the virtual neglect of the long-term potential for functional recovery following brain damage was the eclipse of plasticity concepts during the 100 years following Broca's 1861 publication on location of function. However, in the last 30 years evidence has been accumulating that demonstrates the plasticity of the brain and thus recovery potential is a subject of practical as well as theoretical interest. "Unmasking" of relatively inactive pathways, the taking over of functional representation by undamaged brain tissue, and neuronal group selection are among the me

Hemispheric polymicrogyria and neonatal seizures: a potentially life-threatening combination.

Polymicrogyria (PMG) is a heterogeneous malformation of cortical development characterized by excessive gyration and abnormal cortical lamination. Typically, bilateral forms have more severe developmental delay and early-onset epilepsy, but the full spectrum of severity remains ill-defined. We report two cases of right hemispheric PMG and neonatal-onset, drug-resistant seizures culminating in early death. Case 1 began having seizures on Day 1 of life that intensified in severity and proved resistant to numerous antiepileptic drugs. He underwent right functional hemispherectomy but died three w

Surgical treatment of epilepsy--clinical, radiological and histopathological findings in 139 children and adults.

The present study relates clinical and radiological data to histopathological diagnoses in the first 139 patients (children and adults) in the Göteborg Epilepsy Surgery series. Temporal lobe resections were most common (54.0%) followed by frontal lobe (18.0%) and multilobar resections (11.5%). All histopathological specimens were re-evaluated in connection with this study. Parenchymal malformations and atrophic-gliotic lesions were the most common histopathological findings. Microdysgenesis was more common than major malformations (24.5% versus 11.5%). When the MRI scans were blindly re-evalua

Effect of unilateral perinatal hypoxic-ischemic brain damage on the gross development of opposite cerebral hemisphere.

The effect of unilateral perinatal cerebral hypoxic-ischemic damage on the gross development of the opposite hemisphere was investigated in immature rats. Eight-day-old rats underwent unilateral common carotid artery ligation followed by exposure to hypoxia (8% oxygen) ranging from 1 to 2 h at 37 degrees C. The animals were sacrificed 3 weeks later, and brain damage was assessed histologically, by weighing the cerebral hemispheres, and by measuring hemispheric cross-sectional area and diameter as well as the thickness of neocortex. There were no significant differences in either the size or co
