Subclavian catheterization for acute hemodialysis has become routine in many institutions. Immediate complications of this technique are dramatic and easily recognized, whereas late complications are less common, frequently insidious and tend to be more severe. We report the delayed occurrence of a right hemothorax following two hemodialysis treatments with a left subclavian access. Pertinent literature is reviewed.

A 19-year-old woman died when a subclavian catheter that had provided vascular access for plasmapheresis penetrated her right atrium, pericardium, and parietal pleural, causing a hemothorax. Precautions are recommended to minimize the risk of this complication in patients in whom subclavian catheters are used as a vascular access route for hemodialysis or plasmapheresis.

A case is described where a near-fatal hemothorax developed in a child (age 13 months) right after the removal of the subclavian vein catheter. The catheter was introduced a week earlier and it had functioned without disorder. The possible mechanism of the complication is discussed. Even though this kind of complication is very rare, we recommend that the patients, especially children, must be carefully watched also after removal of the subclavian vein catheter to find a possibly life-threatening complication immediately.

Idiopathic intramural hematoma of the esophagus is a rare condition associated with Mallory–Weiss syndrome or spontaneous esophageal rupture. It is usually manageable with conservative treatment but, in rare cases, may be complicated by hemothorax or progress to esophageal perforation. We report the case of a 56-year-old man with right-sided paralysis following a stroke who was taking aspirin. He presented with chest pain, a suspected mediastinal tumor, and right pleural effusion on CT. At our hospital, contrast-enhanced CT suggested a mediastinal hematoma with contrast leakage. He had no hema

INTRODUCTION: Rivaroxaban, a novel oral anticoagulant, is widely used in patients with non-valvular atrial fibrillation. Bleeding events during rivaroxaban therapy most commonly include gastrointestinal bleeding and intracranial hemorrhage, while spontaneous hemothorax is exceedingly rare. We report a case of spontaneous hemothorax occurring during rivaroxaban administration.
METHODS: We report the case of a patient presenting primarily with dyspnea. The patient had been on long-term oral rivaroxaban for atrial fibrillation. Physical examination upon admission revealed diminished breath sound

BACKGROUND: Central venous catheters play a vital role in managing critically ill patients by providing reliable access for fluid resuscitation, medication delivery, hemodialysis, and hemodynamic monitoring. While insertion-related complications such as pneumothorax and vascular injury are well described, adverse events following catheter removal are less commonly recognized in clinical practice. These post-removal complications, though rare, can be life-threatening if not promptly identified. This report presents a rare case of hemothorax occurring immediately after the removal of a subclavia

A 67-year-old male with metastatic lung cancer presented with acute shortness of breath and increasing oxygen requirements. He had a decreasing hemoglobin for which he required red blood cell transfusions. His chest X-ray showed near complete white-out of the left lung. Bedside ultrasound (Handheld Sonostar C4PL) showed a large pleural effusion with swirling echogenic material suggestive of plankton sign. The pleural effusion was aspirated and showed frank blood, after which a small-bore chest tube (SBCT) was inserted. A total of 3200 mL of blood was drained with the SBCT. There was complete c

BACKGROUND: Neurofibromatosis type 1 (NF1) is generally a benign disease but has the potential for rare and fatal complications. Vascular signs and symptoms associated with NF1 are reported in only 1-3% of patients. Pulmonary complications have been rarely described in the literature and spontaneous rupture of a major thoracic artery is, however, extremely uncommon.
CASE REPORT: We report the case of a patient with NF1 admitted to the emergency department for the management of a spontaneous life-threatening left hemothorax. The patient became hemodynamically unstable after thoracic drainage.

Background Thoracic aortic aneurysm rupture is often a fatal condition. Emergent thoracic endovascular aortic repair (TEVAR) has emerged as a suitable treatment option. Unfortunately, respiratory complications from hemothorax continue to be an important cause of morbidity and mortality even after successful management of the aortic rupture. We hypothesize that early hemothorax decompression after TEVAR for ruptured aneurysms decreases the rate of postoperative respiratory complications. Methods Single-center, retrospective eight-year review of ruptured thoracic aneurysms treated with TEVAR. Re

We present the case of a 30-year-old man with no past history of disease or recent trauma, who was seen in the emergency room after developing sharp pain in the left hemithorax. Chest roentgenogram showed costopherenic angle blunting and an oval mass in the left mediastinum. A computed tomographic scan showed extrapleural mass with coarse calcifications and pleural effusion, confirmed by magnetic resonance imaging. The tumor was biopsied and removed during thoracotomy. The pathology reported revealed chondrosarcoma, which is a rare cause for a spontaneous massive hemothorax. Invasion of the in

Ruptured cystic mature teratoma of anterior mediastinum is rare. This 45-year-old woman complained of sudden onset of chest pain and dyspnea after a traffic accident. Physical examination revealed decrease of breath sounds over the left chest. Chest X-ray showed pleural fluid accumulation of the left pleural cavity. Computed tomography scan of the chest revealed a heterogenous cystic mass lesion approximately 11.5 x 10 x 7 cm in size in the left anterior mediastinum and left pleural effusion. Under the impression of mediastinal tumor and traumatic hemothorax, she received excision of the media

A unique case of hemothorax caused by proven pleural involvement with benign gestational trophoblastic disease and anticoagulation is presented. The pulmonary manifestations of gestational trophoblastic disease are reviewed. It is stressed that these may be associated with histologically benign as well as frankly malignant disease, and that the clinician must be alert for them in managing all patients with known or suspected gestational trophoblastic disease.

PURPOSE: Adolescent idiopathic scoliosis (AIS) is commonly managed with posterior spinal fusion (PSF) and thoracic pedicle screw fixation. While vascular complications are typically acute, delayed hemothorax occurring years after surgery is exceedingly rare. This report describes a unique case of delayed hemothorax secondary to thoracic pedicle screw proximity to the descending thoracic aorta.
METHODS: An 18-year-old female presented with chest pain and dyspnea 5 years after T4-L3 posterior thoracic fusion for AIS. Computed Tomography Angiography (CTA) revealed a large left pleural effusion,

BACKGROUND: Pulmonary arteriovenous malformation (PAVM) is a rare pulmonary vascular anomaly. During pregnancy, physiological hemodynamic changes and hormonal fluctuations substantially increase the risk of PAVM rupture. Spontaneous hemothorax secondary to ruptured PAVM is extremely uncommon and often presents with nonspecific manifestations, frequently resulting in misdiagnosis and delayed treatment.
CASE PRESENTATION: A 33-year-old woman at 31+ 6 weeks of gestation presented to the emergency department with a 30-minute history of sudden-onset left-sided chest pain radiating to the shoulder

Blunt thoracic trauma often causes rib fractures, hemothorax, and pneumothorax. Although there is no established definition regarding the duration and management of delayed hemothorax, it commonly occurs in a few days and exhibits at least one displaced rib fracture. Moreover, delayed hemothorax rarely develops tension hemothorax. A 58-year-old male who had a motorcycle accident received conservative treatment from his orthopedic doctor. He felt a sudden severe chest pain 19 days after the accident. Contrast-enhanced computed tomography (CT) of the chest revealed multiple left-sided rib fractu

BACKGROUND: Esophageal gastrointestinal stromal tumors (GISTs) are rare tumors of the gastrointestinal interstitium, and comprise less than 0.7% of all GISTs. The presentation of esophageal GIST is relatively benign, commonly characterized by symptoms of dysphagia and gastrointestinal bleed. On the contrary, it is highly unusual for these tumors to present as surgical emergencies.
CASE PRESENTATION: Here, we describe a case of hemothorax secondary to the rupture of a massive (19 cm) esophageal GIST in a 79-year-old male. The patient presented with mild back pain, vomiting, and hypotension. A

A 44-year-old man presented with spontaneous massive hemothorax. Computed tomographic scan revealed thickened and nodular pleura with no evidence of lung tumor. The patient underwent right open total pleurectomy. Histopathologic and immunohistochemical studies of the pleura confirmed poorly differentiated sarcomatoid squamous cell carcinoma of the lung with metastasis to the pleura. The carcinoma was psuedomesothelimatous type because of its presentation as diffuse pleural thickening. This presentation of sarcomatoid cancer as pseudomesotheliomatous type with massive hemothorax is rare and can
