Ortner's syndrome or cardiovocal syndrome is a rare condition referring to laryngeal recurrent nerve paralysis due to cardiovascular conditions. We report the case of a 66-year-old man, with a medical history of active smoking, who complained of hoarseness of voice secondary to vocal cord palsy. A neck and thoracic CT scan revealed severe enlargement of pulmonary main artery, which caused compression of the left recurrent laryngeal nerve in the aortopulmonary window. The prolonged course of the left laryngeal nerve makes it susceptible to injury from cardiovascular structures in the mediastinu

A 52-year-old man presented with hemoptysis of 2 weeks' duration. He had been experiencing hoarseness, right-sided pleuritic chest pain, subjective fevers, chills, night sweats, and 10 pounds weight loss for the previous 2 months. He additionally reported severe frontal headaches, nasal congestion, and intermittent epistaxis, which had been present for a year before his current presentation. He had worked in construction and denied tobacco or illicit drug use.

The Hoarseness Guideline Update provides an evidence-based approach to a patient who presents to the clinic with hoarseness. The guidelines cover management decisions in acute and chronic dysphonia for patients of all ages before and after laryngeal examination. The present review discusses the process used to develop these guidelines, including limitations of the process and each key action statement.

OBJECTIVES/HYPOTHESIS: The American Academy of Otolaryngology-Head and Neck Surgery Clinical Practice Guidelines on Hoarseness distinguishes between hoarseness, which is a symptom perceived by the patient, and dysphonia, which is a diagnosis made by the clinician. Our objective was to determine factors that are associated with patient-perceived hoarseness in spasmodic dysphonia (SD) patients.
STUDY DESIGN: Retrospective study.
METHODS: Adductor SD patients who presented for botulinum toxin injections from September 2011 to June 2012 were recruited. The main outcome variable, Voice Handicap I

Dissection of the carotid artery can occur intracranially or extracranially, although dissections tend to affect extracranial segments of the arteries much more commonly than intracranial segments. Carotid artery dissection (CAD) is most common in middle-aged women. Although not completely known, the main risk factors related to carotid artery dissection are genetic and environmental factors, traumatic events, cervical manipulation, migraine, recent infections, hyperhomocysteinemia, and hereditary connective tissue disorders. Although some cases of bilateral internal CAD have been reported, sp

Ortner's syndrome (cardiovocal syndrome) is the clinical entity of hoarseness due to left recurrent laryngeal nerve palsy caused by cardiovascular disease. We present a case of an 86-year-old woman with hoarseness as a prominent symptom due to a large painless aneurysm of the thoracic aorta, which soon ruptured. Cardiovocal syndrome can be a rare but an important and probably the only major clinical finding of a painless aortic rupture.

BACKGROUND: Hoarseness is a major symptom of laryngeal disease. Persistent hoarseness may be an early warning of such sinister lesions as cancer of the larynx and nasopharynx, hence the need for increased awareness of this symptom and its causes.
METHODS: This is a retrospective study of all hoarse adult patients seen in the Otorhinolaryngology clinics of the University College Hospital Ibadan over an 8-year period (1995-2002).
RESULTS: The total study population of 124 patients consisted of 72 (58.06%) males and 52 (41.94%) females with an overall mean age of 46.98 years and age range 16-84

Vocal cord nodules are benign laryngeal lesions seen in professional voice users. They are usually bilateral and are thought to occur due to submucosal edema and hemorrhage, with resultant fibrosis. False cord occurrences are very rare; hence we report a unique case of unilateral right false cord nodule. A healthy 16-year-old female student presented with persistent hoarseness for two years without any history of voice abuse, trauma, infection, or endotracheal intubation. Endoscopic examination showed a mass occupying the right false cord. Subsequent direct laryngoscopy revealed a friable mass

Third branchial cleft cyst is a rare congenital disease of the neck. It presents as a painless mass that develops rapidly in the neck following an infection. This is the first case report of recurrent laryngeal nerve palsy caused by a third branchial cleft cyst. A 30-year-old woman presented with a 3-month history of hoarseness as her only symptom; she had no pain, fever, dysphagia, dyspnoea, or palpable neck mass. Laryngoscopy revealed that her right vocal cord was paralyzed. Computed tomography and magnetic resonance imaging revealed a cystic mass in the right tracheoesophageal groove that w

The differential diagnosis for hoarseness is extensive and includes a multitude of etiologies that span a large geographic area from the brainstem to the mediastinum. Therefore, localizing a causative lesion can be extremely difficult for clinicians and radiologists alike. In this review, we will first discuss the normal anatomy of the larynx and its innervation via the vagus and recurrent laryngeal nerves. We will then proceed with a guided tour of the various infectious/inflammatory, neoplastic, congenital, and traumatic/iatrogenic causes of hoarseness subdivided by anatomic location (brains

A previously well 81-year-old Chinese male presented with hoarseness and low back pain for one month. Chest radiography at presentation revealed widening of the mediastinal silhouette. Nasopharyngoscopy detected left vocal cord paralysis. CT aortogram revealed a large saccular aortic arch aneurysm with a dissection flap extending distally down to the aortic bifurcation. The combination of clinical and imaging findings was consistent with cardiovocal syndrome. In view of good premorbid function, surgical repair was offered, and the patient underwent surgical repair and recovered well with no fu

A 34-year-old woman presented for evaluation of several months of a hoarse voice and dyspnea on exertion that progressed over the last 3 years. She had a clinical diagnosis of asthma that had been treated with bronchodilators and inhaled corticosteroids for a few years. She continued to use her inhalers but with minimal symptomatic improvement. The patient was a lifelong nonsmoker with no history of drug abuse. She worked as a college professor and denied any significant environmental exposures or recent travel.

Cryptococcosis commonly involves central nervous system and lungs in organ transplant recipients. Isolated laryngeal infection is extremely rare. We report a rare case of cryptococcus in a renal allograft recipient that clinically presented with hoarseness of voice and mimicked laryngeal carcinoma on examination.

OBJECTIVE: Automatic intelligibility assessment using automatic speech recognition is usually language specific. In this study, a language-independent approach is proposed. It uses models that are trained with Flemish speech, and it is applied to assess chronically hoarse German speakers. The research questions are here: is it possible to construct suitable acoustic features that generalize to other languages and a speech disorder, and is the generated model for intelligibility also suitable for specific subtypes of that disorder, i.e. functional and organic dysphonia?

Hoarseness is a common post-operative complication in patients who receive general anesthesia. In most cases, the symptoms are temporary and improve within several days. This report describes two patients with prolonged hoarseness following use of the streamlined liner of the pharyngeal airway (SLIPATM). We present the first case of a 56-year-old female patient who developed arytenoid cartilage dislocation resulting in prolonged hoarseness and dysphagia after using a SLIPA™ during a laparoscopic myomectomy. In the second case, we report on a 65-year-old male patient who was scheduled for a lap

PURPOSE: To determine factors associated with chronic hoarseness in Australian group fitness instructors (GFIs).
METHOD: A total of 361 GFIs (81 males, 280 females), aged between 18-67 years currently active in the Australian fitness industry, completed a 65-item self-completion questionnaire distributed via SurveyMonkey. Demographic, lifestyle and voice use variables thought to influence vocal health and voice production in the GFI population were examined using logistic regression analyses. GFIs' chronic hoarseness with response options "positive" and "negative" was considered as the outcom

OBJECTIVE: Laryngeal cryptococcosis is rare, with few reported cases in the literature. We present current investigation and treatment recommendations for this disease.
STUDY DESIGN: A structured literature review; additional cases and expert opinion are presented.
RESULTS: Localized laryngeal cryptococcal infection most commonly presents with persisting hoarseness. Clinical suspicion of the disease is required for accurate diagnosis, with treatment based on the patient's immune status. Early microbiological advice and adequate follow-up is recommended to ensure disease resolution.
