National hospice for the insane and the Brazilian Neurology in the beginning of the 20th century.

The modern Brazilian Neurology was born in the campus of Praia Vermelha, Universidade Federal do Rio de Janeiro (UFRJ). The former National Hospice for the Insane (NHI) as well as some of its facilities were fundamental for the teaching about diseases of the nervous system to the students of the UFRJ's Medical School, caring for patients with neurosyphilis and epilepsy, as well as children with neurological impairment. We highlight the role of Juliano Moreira, director of the NHI (1903-1930), and his team, including Antonio Austregésilo Rodrigues Lima, the "father" of the Brazilian Neurology,

Evaluation of methadone absorption after topical administration to hospice patients.

CONTEXT: Increased use of methadone (MTD) applied topically as an extemporaneously compounded gel was documented on routine review of medication profiles of hospice patients. A literature search did not identify any published clinical studies assessing the systemic absorption or clinical efficacy of MTD administered topically to intact skin for systemic pain. OBJECTIVES: To compare serum MTD levels achieved after topical and oral administration to hospice patients. METHODS: To assess the absorption of MTD administered topically, two steady-state serum levels were determined in 10 patients ad

Hospice Palliative Care: A Model for Quality End-of-Life Care

Abstract Hospice is a program of care provided across a variety of settings, based on the understanding that dying is part of the normal life cycle. As people experience this last phase of life, hospice provides comprehensive palliative medical and supportive services, compassion, and care with the goals of comfort and quality of life closure. A hospice supports the patient through the dying process and the family through the experience of caregiving, the patient’s illness, dying, and their own bereavement. Understanding that the last phase of life is as individual as each person who experienc

Can hospice care be provided to people who live alone?

A growing number of people live alone; some in need of hospice care. Current approaches to providing care include the "patchwork" approach, institutional contracts, hospice day care, a demonstration program for people living alone, and supported housing. A brief discussion of each is presented, with a closing challenge to nursing to provide leadership and bold thinking to meet the changing need of these patients.

Hospice care for patients living alone: results of a demonstration program.

The Wissahickon Hospice initiated a demonstration program in 1988 to provide home hospice care to individuals living at home without primary care persons. This paper describes the planning process and admission requirements for the Live Alone Program and the evaluation results for 34 Live Alone patients compared to 105 regular hospice patients. Principal findings were that the hospice was able to maintain patients at home, to provide care safely to patients, and to enable home deaths for patients living alone. Patients living alone tended to require more supportive services than regular patien

Current situation and support need for non-cancer patients' admission to inpatient hospices/palliative care units in Japan: a nationwide multicenter survey.

BACKGROUND: Specialist palliative care for non-cancer patients is important; however, access to inpatient hospices/palliative care units (PCUs) for non-cancer patients in Japan may be insufficient. We aimed to explore the current situation, the support needs to accept admission of non-cancer patients, and the willingness to accept admission of non-cancer patients to inpatient hospices/PCUs in Japan. METHODS: We conducted a nationwide multicenter anonymous questionnaire survey to inpatient hospices/PCUs in Japan in January 2022. We recruited potential participants from 381 PCUs belonging to Ho

Development and Execution of a Pediatric Hospice and Palliative Medicine Fellowship in Latin America.

BACKGROUND: Pediatric hospice and palliative medicine (PHPM) is a growing subspecialty worldwide; however, formal graduate medical education pathways remain limited in low- and middle-income countries, including most of Latin America, constraining workforce development and access to specialized care. OBJECTIVE: To describe an educational innovation that provides formal fellowship training in PHPM to physicians in Latin America. METHODS: From 2023 to 2026, Unidad Nacional de Oncología Pediátrica (Guatemala), St Jude Children's Research Hospital, and the University of Utah developed and implem

A qualitative descriptive case study on the incentive strategies of hospice care volunteers.

BACKGROUND: While the operational and funding structures of hospice care vary significantly across the globe, volunteers remain an indispensable component of hospice care teams. Existing research has examined volunteers' working experiences and training models; however, less is known about incentive strategies to promote sustained engagement. OBJECTIVE: To explore the incentive strategies for hospice care volunteers through this qualitative case study in China and thus provide evidence to enhance global volunteer retention. METHODS: A descriptive qualitative case study was conducted using pa

A Pilot Curriculum on Physician-Assisted Dying for Hospice and Palliative Medicine Fellows.

INTRODUCTION: Physician-assisted dying (PAD) is legal in an increasing number of jurisdictions across the United States. Despite this, formal hospice and palliative medicine (HPM) fellowship curricula on PAD are lacking. To address this, a neutral (nonadvocacy) PAD pilot curriculum was developed and implemented in one large HPM fellowship program. METHODS: A five-lecture curriculum was developed by an interdisciplinary group using Kern's six-step method. Comfort, knowledge, and satisfaction were assessed. RESULTS: 17 fellows were eligible to complete curriculum assessments with a 76% respons

Hospice family caregivers' preparedness to provide care during the death vigil.

Family caregivers of hospice patients face numerous demands during their family member's final days of life, including escalating symptom management needs, physical strain, and intense emotional challenges. Informed by preparedness theory and the National Cancer Institute framework for patient-centered communication (adapted for family caregivers), this qualitative descriptive study explored the knowledge, skills, and presence family caregivers required to fulfill their role as their family member was actively dying, as well as their perceptions of their communication with hospice teams during

Educating the Hospice Workforce on Pediatric Concurrent Care: A National Workshop Approach.

CONTEXT: Medicaid insures most children at the end of life and mandates concurrent hospice and curative care coverage. Uptake remains inconsistent, partly due to hospice workforce unfamiliarity with the benefit and concerns about compliance. OBJECTIVES: To evaluate whether a brief virtual workshop can shift hospice workforce self-perceived knowledge, attitudes, and behavior (KAB) regarding pediatric concurrent care, and to identify workforce-informed priorities for a Concurrent Care Toolkit. METHODS: Two national one-hour virtual workshops (clinical and administrative audiences) were conduct

[Barriers to the timely initiation of hospice-palliative care in the Hungarian healthcare system].

INTRODUCTION: Timely initiation of hospice-palliative care may improve the quality of life of patients with advanced, incurable disease, support family caregivers, and reduce the use of disproportionate hospital care. In Hungary, however, access to care and the organization of patient pathways may face several challenges. OBJECTIVE: The aim of our study was to explore, based on the experiences of healthcare professionals, which factors hinder the timely initiation of hospice-palliative care in the Hungarian healthcare system. METHOD: In this qualitative study, semi-structured interviews were

The Hospice and Palliative Medicine Physicians Certification Programs across Countries/Regions.

As the demand for end-of-life care increases, the development of a well-structured training and certification system for palliative medicine specialists is becoming increasingly important. In South Korea, a certification system for palliative care physicians has been in place since 2019, managed by the Korean Society for Hospice and Palliative Care. To further develop this certification system and training process, this review aims to describe hospice and palliative medicine certification programs across eight countries/regions-the United States, the United Kingdom, Australia, Japan, Taiwan, H
