The Nature of Colonial Public Healthcare: The Establishment of Sariwon Provincial Hospital.

This paper analyzes the structural limitations of the colonial public healthcare system by examining the establishment of Sariwon Provincial Hospital in Hwanghae Province under Japanese colonial rule. In the 1920s, Sariwon emerged as a central city in Hwanghae Province, driven by the development of railway transportation and commerce. However, modern medical institutions and public healthcare facilities remained severely inadequate. Under these circumstances, the local community of Sariwon initiated a movement to establish a provincial hospital. The movement, however, faced a crisis as the Gov

Improving surgical care in rural South Africa: exploring collaboration between Indigenous and formal health sectors.

INTRODUCTION: South Africans use both Indigenous and formal health care to address surgical conditions; however, these sectors are often siloed, leaving care for the individual disjointed. This study aimed to improve person-centered surgical care in rural South Africa by identifying community-prioritized surgical conditions and exploring Indigenous and formal healthcare stakeholder buy-in to collaborative solutions. METHODS: This phenomenological qualitative study used focus group discussions and a multi-stakeholder workshop. Study participants included Indigenous knowledge healers, clinic an

[Telemedicine support system in home care of patients with chronic respiratory failure: preliminary results].

We report the preliminary results of an advanced telemedicine system for the delivery of home care services. We have been trying out a system for a ternary telemedicine network for remote consultation in patients with chronic respiratory failure. The elements of this system are: the patients at home, the home doctor (and/or community hospital), and an institution with respiratory staff (Shinshu University), which were linked by ISDN lines. The system allows real-time visualization, at home, of the patients and their physiological data, which are transmitted simultaneously to both the community

Nosocomial Legionnaires' disease uncovered in a prospective pneumonia study.

Most hospitals have yet to record a case of nosocomial legionnaires' disease; the importance of isolation of Legionella pneumophila in the water system of such an institution is unclear. We undertook a prospective pneumonia study in tandem at a veterans hospital where legionnaires' disease was known to be endemic and a community teaching hospital where legionnaires' disease had never been documented. Legionella serological tests were performed on all patients with pneumonia; selective culture media and direct fluorescent antibody testing for Legionella were made readily available. Simultaneous

The Role of Narrative Medicine and Lean Management in Umbilical Cord Blood Donation: A Story of Success.

Background/Objectives: Umbilical cord blood (UCB) is a valuable source of hematopoietic stem cells used in treating blood and immune disorders. Despite its potential and the availability of public banking systems in Italy, donation rates remain low due to patient misinformation, emotional barriers, and organizational inefficiencies. This study aimed to evaluate the impact of integrating Narrative Medicine (NM) and Lean Management (LM) on UCB donation rates and operational effectiveness at the University Hospital of Alessandria. Methods: This prospective, single-center pre-post study ran from J

Crisis leadership and power dynamics in Portuguese hospitals: organisational learning from COVID-19.

PURPOSE: This paper aims to examine how power struggles, competing leadership logics and governance failures shaped organisational learning and crisis response in Portuguese public hospitals during the COVID-19 pandemic. By analysing tensions between managerial, clinical and political actors, this study explores how fragmented authority and informal leadership networks conditioned hospitals' adaptive capacity under sustained systemic pressure. DESIGN/METHODOLOGY/APPROACH: This study draws on 41 semi-structured interviews with hospital managers, clinical leaders and frontline coordinators acro

Perspectives of stroke survivors, caregivers and healthcare providers on improving access to stroke care services in Tanzania: A qualitative study.

BACKGROUND: Stroke is a leading cause of death and disability worldwide, with the greatest burden occurring in in low- and middle-income countries. In Tanzania, delayed hospital presentation, weak referral systems, high out-of-pocket costs, shortages of stroke-ready facilities, and limited rehabilitation services contribute to preventable deaths and long-term disability. Although previous qualitative studies have described barriers to accessing stroke care services, there remains limited evidence on strategies for improving access across the continuum of stroke care. Therefore, this study expl

Action learning across the decades.

Purpose The purpose of this paper is to explore how action learning concepts were used in two healthcare projects undertaken many decades apart. The specific purpose in both cases was to examine how action learning can contribute to shared learning across key stakeholders in a complex socio-technical system. In each case study, action learning supported joint design programmes and the sharing of perspectives about the complex system under investigation. Design/methodology/approach Two action learning projects are described: first, the Hospital Internal Communications (HIC) project led by Reg R

Service-oriented subscription management of medical decision data in the intensive care unit.

OBJECTIVES: This paper addresses the design of a platform for the management of medical decision data in the ICU. Whenever new medical data from laboratories or monitors is available or at fixed times, the appropriate medical support services are activated and generate a medical alert or suggestion to the bedside terminal, the physician's PDA, smart phone or mailbox. Since future ICU systems will rely ever more on medical decision support, a generic and flexible subscription platform is of high importance. METHODS: Our platform is designed based on the principles of service-oriented architect

Palliative care utilization in oncology and hemato-oncology: a systematic review of cognitive barriers and facilitators from the perspective of healthcare professionals, adult patients, and their families.

BackgroundDespite the high potential to improve the quality of life of patients and families, palliative care services face significant obstacles to their use. In countries with high-resource health systems, the nonfinancial and nonstructural obstacles to palliative care services are particularly prominent. These are the cognitive barriers -knowledge and communication barriers- to the use of palliative care. To date no systematic review has given the deserved attention to the cognitive barriers and facilitators to palliative care services utilization. This study aims to synthesize knowledge on

Opening Doors to Recovery: Recidivism and Recovery Among Persons With Serious Mental Illnesses and Repeated Hospitalizations.

OBJECTIVE: Repeated hospitalizations and arrests or incarcerations diminish the ability of individuals with serious mental illnesses to pursue recovery. Community mental health systems need new models to address recidivism as well as service fragmentation, lack of engagement by local stakeholders, and poor communication between mental health providers and the police. This study examined the initial effects on institutional recidivism and measures of recovery among persons enrolled in Opening Doors to Recovery, an intensive, team-based community support program for persons with mental illness a

Community Mental Health Policy in America: Lessons Learned.

After 1945 the prevailing consensus that persons with serious mental disorders should be treated in public hospitals began to dissolve. It was replaced by a faith in the efficacy of community care and treatment. Yet for a variety of reasons the new community care system fell far short of its initial promise. Advocates failed to understand that the problems of persons with serious and persistent disabilities were different from those people with mild and moderate disorders. The belief that residence in the community would promote adjustment and integration was illusory and did not take into acc

Policies and initiatives to facilitate timely discharge from hospitals: a comparison of six European countries.

BACKGROUND: Timely discharge of patients who are clinically ready to be discharged from hospitals to the next point of care is a common health system challenge. Ensuring safe and effective discharge holds new urgency, given the backlogs and increased waiting times for inpatient services in many countries after the COVID-19 pandemic. OBJECTIVE: The study aimed to identify policy options for addressing delayed discharges in six European countries (Denmark, France, Germany, the Netherlands, Norway, and Sweden), and to summarise available evidence on their effectiveness.
