Mitochondrial fusion and fission regulate mitochondrial morphology and homeostasis, both of which are essential for maintaining cellular health. Free mitochondria and mitochondrial-containing extracellular vesicles have emerged as key mediators of pathological processes. Conditioning regimens for allogeneic hematopoietic cell transplantation (HCT) damage and lead to impaired mitochondrial function, including biogenesis and respiration, as well as elevated reactive oxygen species (ROS), which contribute to the development of inflammatory conditions as well as activation of antigen presenting ce

Staphylococcus aureus is a facultative intracellular pathogen that persists within both professional and non-professional phagocytes, contributing to chronic and antibiotic-resistant infections. Autophagy, particularly xenophagy, serves as a central cell-autonomous defense pathway that can capture intracellular S. aureus and deliver it to lysosomes for degradation. However, the bacterium has evolved multiple strategies to subvert xenophagic clearance, including manipulation of bacteria-containing autophagosome maturation, blockade of autophagosome and lysosome fusion, and co-option of autophag

Tuberculosis (TB), caused by Mycobacterium tuberculosis (Mtb), remains the leading cause of death from a single infectious agent worldwide, with 10.7 million new cases and 1.23 million deaths reported globally in 2024. Although conventional chemotherapy cures most drug-susceptible TB, the rising burden of multidrug-resistant TB (MDR-TB), prolonged treatment regimens, and suboptimal patient adherence continue to undermine control efforts. Against this background, immunotherapeutic approaches have gained renewed interest as rational complements to chemotherapy, building on decades of mechanistic

Severe fever with thrombocytopenia syndrome virus (SFTSV), an emerging tick-borne bunyavirus with a case fatality rate of up to 30%, poses a growing threat to global public health. No approved specific antivirals are currently available. This perspective critically evaluates recent advances in the development of small-molecule anti-SFTSV agents, organized through a dual framework encompassing both viral life cycle targets and host-dependent pathways. We highlight direct-acting antivirals targeting key viral components, including viral entry glycoproteins (Gc/Gn proteins), cap-snatching endonuc

Chronic chikungunya arthritis (CCA) is a major long-term sequela of Chikungunya virus (CHIKV) infection. Its pathogenesis involves persistent viral reservoirs in joint tissues, metabolic reprogramming of myeloid cells, dysregulated Treg/Th17 immunity, and autoimmune-like tissue injury. This review integrates recent advances in virology, immunometabolism, and clinical pathology to propose a four-stage pathological cascade: "viral persistence", "myeloid metabolic dysregulation", "Treg/Th17 imbalance", and "autoimmunity-like injury". Key cytokine networks and molecular interactions at each stage

Respiratory infections remain a major global health threat, and recent epidemics have shown that treating the pathogen alone is not enough. In severe influenza, COVID-19, RSV, and bacterial pneumonia, lung failure often results less from microbial load and more from the host's overactive immune response. Two key processes, neutrophil extracellular traps (NETs) and trained immunity, sit at the center of this shift toward host-focused intervention. Although both are innate defenses, they are usually discussed in isolation: NETs in the context of acute inflammation and thrombosis, and trained imm

Diabetic foot infections (DFIs) are characterized by persistent bacterial biofilms and impaired host immune defenses. Within the diabetic microenvironment, peripheral neuropathy disrupts neuro-immune feedback loops, contributing to the arrest of resident macrophages in a pro-inflammatory M1 phenotype. This phenotypic arrest impairs efferocytosis and sustains chronic inflammation, which conventional antimicrobial therapies frequently fail to resolve. Electroacupuncture (EA) serves as a neuromodulatory intervention capable of reversing this immune dysfunction. By stimulating specific somato-auto

Multidrug-resistant (MDR) strains of Klebsiella pneumoniae present an acute threat as they continue to disseminate globally. Phage therapy has shown promise as a powerful approach to combat MDR infections, but narrow phage host ranges make development of broad acting therapeutics more challenging. The goal of this effort was to use in vitro directed evolution (the "Appelmans protocol") to isolate K. pneumoniae phages with broader host ranges for improved therapeutic cocktails. Five myophages in the genus Jiaodavirus (family Straboviridae) with complementary activity were mixed and passaged aga

Tuberculosis (TB), caused by Mycobacterium tuberculosis, reclaimed its status as the deadliest single-pathogen infectious disease globally in 2023, accounting for 10.8 million new cases and 1.25 million deaths in 2023. The escalating crisis of drug-resistant TB, suboptimal BCG vaccine efficacy in adults, and persistent treatment gaps for vulnerable populations collectively drive an urgent shift toward mechanism-driven, precision-oriented therapeutic strategies. This review presents a critically integrated appraisal of anti-TB research advances spanning 2020 to 2025, drawing on structural biolog

Psoriasis is a chronic, immune-mediated inflammatory disorder with systemic implications beyond its cutaneous manifestations. Its pathogenesis involves a complex interplay between genetic predisposition, environmental triggers, and immune dysregulation. The sustained crosstalk between dendritic cells (DCs) and T cells, which orchestrates the initiation and perpetuation of psoriatic inflammation. Plasmacytoid DCs, activated by nucleic acid-LL37 complexes, produce interferon-α that promotes myeloid DC maturation. These DCs secrete IL-12, IL-23, and TNF-α, driving differentiation of naïve T cells

Immune checkpoint inhibitors (ICIs) have transformed cancer therapy by releasing inhibitory pathways that restrain antitumor T-cell responses. Their success has renewed interest in checkpoint blockade for infectious diseases, particularly chronic viral infections, sepsis-associated immunosuppression, and refractory opportunistic infections. In these settings, immune checkpoints may contribute to pathogen persistence by limiting effector function, but they also restrain tissue injury, cytokine excess, and immune-mediated organ damage. This dual role makes translation fundamentally different fro

Inflammasome activation and pyroptosis are central to innate immune responses and play either protective or detrimental roles in host defense. Inhibiting these processes holds significant therapeutic promise for diseases exacerbated by inflammasome activation. However, clinically targeting this pathway remains challenging, as blocking pyroptosis may trigger alternative cell death pathways. In this mini-review, we summarize bacterial and viral infections in which inflammasome activation and pyroptosis are detrimental to host defense, explore the mechanisms underlying cell death switching, and d

Background: Hepatitis B and C viral infections remain a significant global health challenge, despite the implementation of an effective direct-acting antiviral (DAAs) and nucleos(t)ide analogues (NAs). Current HBV therapy is not curative as stopping therapy usually leads to active disease in most patients requiring long-term treatment. Although current HCV-DAAs are highly effective they fall short due to arising drug-resistance and have limited ability to avert re-infections. Furthermore, current HCV DAA treatments lead to the reactivation of occult HBV infection, compromising the effectivenes

Severe Mycoplasma pneumoniae pneumonia (SMPP) presents a major clinical challenge, increasingly complicated by macrolide-resistant strains (MRMP) and viral co-infections. The progression of SMPP is fundamentally driven by intricate immune-mediated mechanisms rather than direct pathogen-induced injury. Key drivers include innate immune hyperactivation, such as NLRP3 inflammasome-mediated pyroptosis, and adaptive immune dysregulation, characterized by the hyperactivation of pathogenic Th17 cells. Furthermore, systemic azithromycin administration can disrupt the gut-lung axis by depleting short-c

BCG, a first-generation live vaccine, is being reconsidered as an immunological platform. Interest in its heterologous protection intensified during the pandemic. However, large-scale clinical trials revealed inconsistencies in the efficacy of native BCG. This review argues that BCG's main value lies in its potential as a modifiable vector platform and in its ability to reveal tractable molecular pathways for therapeutic design. This review summarizes the molecular basis of BCG-induced trained immunity (TRIM), focusing on PRR-driven signaling, metabolic rewiring, and epigenetic remodeling in i

Neutrophil Extracellular Traps (NETs) are web-like structures composed of DNA fibers, histones, and granular proteins released by the neutrophils activated upon various stimuli. Recognized as a host immune defense mechanism, NETs also play a significant role in several diseases, especially in bacterial infections. Research indicates that excessive NET formation contributes to various pathological outcomes, including multiple organ dysfunction. Once activated, NETs generally remain in a hyperinflammatory phenotype. In this review, we delineate how bacterial pathogens both induce excessive NET f

In this research, we validate that inhibitors of host-cell-Rac1 can provide a novel strategy for antimalarial therapy. Host-directed therapies represent an emerging strategy to overcome antimalarial drug resistance by targeting host factors essential for Plasmodium survival. The small GTPase Rac1, the only Rac present in mature human erythrocytes, has been previously identified as a key host factor supporting Plasmodium falciparum intraerythrocytic development. Building on the known Rac1 inhibitor EHop-016, which had shown good antiplasmodial activity in vitro in a previous study, we designed

BACKGROUND: Tuberculosis (TB) remains a major global health threat, contributing substantially to high morbidity and mortality rates. This underscores the urgent need for more effective interventions. Recent studies highlight the potential of host-directed therapy approaches to enhance immune defences against TB. Atorvastatin, recognized for both its lipid-lowering properties and its immunomodulatory effects, has emerged as a compelling candidate for host-directed therapy against TB. Here, we investigated the ex vivo efficacy of atorvastatin in inducing immunomodulatory activities (phagosome m

The interplay between host and parasite determines parasite burden and disease outcome. Parasite exploits host signaling pathways like p38-MAPK for its survival and pathogenesis. Here we have used NR-7h, a proteolysis-targeting chimera (PROTAC) targeting human p38-MAPK to assess p38-MAPK's role in Leishmania donovani and Plasmodium falciparum infection in their respective host cells. NR-7h degrades host p38-MAPK in a time- and dose-dependent manner. Degradation of host p38-MAPK by NR-7h reduces parasite load in host cells dose-dependently, implicating the role of p38-MAPK in parasite survival.

Tumor heterogeneity induces resistance of programmed cell death (PCD), together with a highly immunosuppressive tumor microenvironment (TME), collectively constraining the efficacy of cancer therapies. Herein, a Janus TiO2-CoOX@PEG nanoplatform featuring a Z-scheme heterojunction was developed as a novel sonosensitizer, enabling efficient ultrasound-induced charge separation to boost sonodynamic therapy (SDT) activity. Moreover, the obtained TiO2-CoOX@PEG was endowed with multienzyme activity, including glutathione (GSH) oxidase- and catalase-like activities, which could regulate TME (e.g., re
