Case Report: Necrotizing pulmonary destruction and fatal gangrene: the imperative of early recognition.

INTRODUCTION: Pulmonary gangrene is a catastrophic end-stage manifestation of necrotizing pulmonary infection, often arising on the basis of unrecognized and chronically survived pulmonary thromboembolism. Large-vessel thrombosis leads to ischemia and infarction of pulmonary parenchyma, creating an ideal substrate for secondary infection with subsequent rapid tissue devitalization. Impaired perfusion prevents adequate antibiotic delivery, facilitating explosive progression of necrosis. CASE DESCRIPTION: We report a fatal case of pulmonary gangrene in a 59-year-old man with a history of chroni

Indwelling pleural catheter enabling safe air travel in malignant pleural effusion with pneumothorax ex vacuo.

Pneumothorax of any cause typically represents a contraindication to air travel. Pneumothorax ex vacuo is often encountered in clinical practice due to non-expandable lung, a common finding in malignant pleural effusion (MPE). The presence of intrapleural air may prevent people diagnosed with terminal illness from participating in air travel. We present the case of a man with pleural mesothelioma who developed a large left hydropneumothorax following initial drainage of MPE. An indwelling pleural catheter (IPC) was placed on subsequent presentation. As the patient wished to undertake a domesti

Anaphylactic Shock Following Spontaneous Rupture of Pulmonary Hydatid Cyst: A Case Report.

We present a case of a 31-year-old rural Iranian man with no prior medical history who developed sudden-onset dyspnea, generalized urticaria, and hypotension. Based on clinical findings and unstable vital signs, anaphylactic shock was suspected and immediate treatment with oxygen, intravenous fluids, and intramuscular epinephrine was initiated. Following stabilization, a spiral chest CT scan revealed a large hydatid cyst with lower lobe collapse, inner membrane detachment near the pericardium, pericardial effusion, and hydropneumothorax. The patient was referred for thoracic surgery, which was

Pneumothorax ex-vacuo or "trapped lung" in the setting of hepatic hydrothorax.

BACKGROUND: Hepatic hydrothorax is a major pulmonary complication of liver disease occurring in up to 5-10% of patients with cirrhosis. CASE PRESENTATION: We report four observations of the development of pneumothorax ex-vacuo or trapped lung in the setting of hepatic hydrothorax. The diagnosis of trapped lung was made based on the presence of a hydropneumothorax after evacuation of a longstanding hepatic hydrothorax with failure of the lung to re-expand after chest tube placement in three of the four cases. Two patients underwent surgical decortication with one subsequent death from post-ope

Incarcerated diaphragmatic hernia as a cause of acute abdomen.

Diaphragmatic hernias are usually congenital, and they usually occur in the neonatal group. They do occur in the adult population, albeit very rarely. We present a case of an adult male patient with complications as a result of a congenital diaphragmatic hernia. The patient presented with atypical abdominal pain and respiratory distress. Radiological investigations confirmed a left hydropneumothorax as well as large bowel obstruction secondary to herniation of the omentum and transverse colon through a congenital defect in the left hemi-diaphragm. He had a laparotomy, which confirmed the radio

[Pulmonary infarction as a cause of pneumothorax: report of two cases (author's transl)].

Pulmonary infarction is a very uncommon cause of pneumothorax. The authors report two patients with pneumothorax arising as a complication of pulmonary infarction. One was a 72-year-old man who had hemoptysis, pleural effusion, and alveolar condensation. Four days later he developed a hydropneumothorax and pulmonary cavitation. He died of heart failure. The pulmonary infarction was not septic in this case. The other patient was a 12-year-old boy who suffered a septic embolism with cavitation as a result of an infected wound. He later developed a tension pneumothorax and died in a state of shoc

A Rare Case of Multi-Organ Complications Due to Disseminated Citrobacter koseri Infection Originating From a Renal Abscess.

Citrobacter koseri is a gram-negative bacillus that can cause severe infections in immunocompromised individuals. While it is a rare urinary tract pathogen, its potential for dissemination and severe complications demands attention. We report the case of a 44-year-old male with poorly controlled type 2 diabetes mellitus who developed a disseminated C. koseri infection originating from a perirenal abscess. The infection extended locally, resulting in septic thrombosis of the left renal vein, and spread distantly, causing septic pulmonary embolism. The patient underwent multiple surgical interve

Diaphragmatic Musculophrenic Vessel Injury Following Ultrasound-Guided Pigtail Catheter Insertion: A Rare Case.

Pigtail catheters are widely used as a minimally invasive substitute to large-bore chest tubes for the management of pleural effusion. Although it is considered safe, serious complications may occur. We report a rare case of diaphragmatic musculophrenic artery injury following ultrasound-guided pigtail catheter insertion. A 39-year-old female with multiple comorbidities was admitted with an impression of pleural effusion. She underwent ultrasound-guided pigtail catheter insertion, with drainage of hemorrhagic exudative fluid. Within one hour, she developed severe dyspnea and chest pain. Imagin

Parvimonas micra: A Rare Cause of Pleural Empyema With COVID-19 Co-infection.

Parvimonas micra, an oral anaerobe and a known gastrointestinal microbiota, has also been found to be enriched in mucosal tissues of the colon. Our patient presented with chest pain, productive cough, and hypoxia. He was diagnosed with COVID-19 pneumonia with a suspected superimposed bacterial infection. After the initiation of treatment, the patient developed a right hydropneumothorax/loculated pleural effusion on X-ray. Bedside drainage was done, and cross-sectional imaging showed findings of pleural empyema. Cultures obtained after bedside drainage grew P. micra. The patient underwent right

Oesophageal rupture: a tough diagnosis to swallow.

An elderly gentleman presented with acute dyspnoea and right-sided pleuritic chest pain. Two-weeks previously an oesophageal stent had been inserted for dysphagia secondary to oesophageal carcinoma. With low PaO(2), a neutrophilic leucocytosis, raised inflammatory markers and a right-sided pleural effusion, antibiotics were prescribed for pneumonia. Computed tomographic pulmonary angiogram (CTPA) ruled out pulmonary embolus. The patient rallied transiently and his effusion improved. His respiratory distress returned 14 days later. A chest x-ray revealed a right-sided hydropneumothorax, and a c

Boerhaave's syndrome: Thoracolaparoscopic approach.

We present a case of Boerhaave's syndrome managed thoracolaparoscopically. A 45-year- old man presented with hydropneumothorax following severe retching. He was treated with Intercostal drainage insertion as the primary management and referred to a tertiary care centre. There endoscopic stapling was attempted, following which he developed a leak. He presented to us with severe sepsis and mediastinal collection on the ninth day following the perforation. We treated him with thoracoscopic mediastinal toilet, laparoscopic-assisted feeding jejunostomy and cervical oesophagostomy. The patient was m

The limited role of total parenteral nutrition in the management of pancreatic pseudocyst.

Total parenteral nutrition (TPN) for the nonoperative treatment of acute pancreatic pseudocyst has been of hypothetical benefit. We reviewed pseudocyst hospital admissions in 40 patients treated with TPN who had serial imaging studies. The mean cyst size was 7.4 cm on presentation, decreasing to 5.6 cm after nonoperative treatment with TPN (mean 32.5 days). After a nonoperative period, 68 per cent of cysts regressed, completely in 14 per cent, partially in 54 per cent. Except for a patient with cyst-related obstructive jaundice, there were no complicated pseudocysts. Only 12 (28%) patients und

Endobronchial Infection and Bacterial Lymphadenitis by Gemella morbillorum Leading to Airway Perforation and a Bronchopleural Fistula.

Introduction: Necrotizing bronchial infection with severe infectious lymphadenitis is infrequently encountered and most commonly ascribed to Aspergillus, Histoplasma, and Mycobacterium species. We present a unique cause of severe airway destruction with lymphadenitis and bronchopleural fistula formation by the bacterium Gemella morbillorum. Case: A 24-year-old man presented with acute symptoms of vomiting, fever, and shoulder pain. A CT of the chest demonstrated a large subcarinal mass encasing the central bronchi. The workup for malignant, fungal, and granulomatous etiologies was unrevealing,

Cladosporium sphaerospermum as a Rare Cause of Pneumonia.

Cladosporium sphaerospermum isa radiotrophic dematiaceous fungus that can rarely cause disease in humans such as infections of the skin, eye, upper airways, and brain. To the best of our knowledge, we present the first reported case of Cladosporium sphaerospermum-induced invasive lung infection. This case presents a 51-year-old male with a medical history significant for heavy smoking and severe alcohol abuse who was admitted for acute hypoxic respiratory failure secondary to a large exudative right pleural effusion compounded by hydropneumothorax. Despite an initial positive clinical response

Spontaneous Hemopneumothorax: A Rare Cause of Unexplained Hemodynamic Instability in a Young Patient.

Spontaneous hemopneumothorax is a rare and potentially life-threatening disorder which complicates about 1-12% of patients presenting with spontaneous pneumothorax and has a remarkable predilection for male patients. It may present with signs of hypovolemic shock without apparent cause. While there are no specific guidelines for the management of patients diagnosed with such condition, wide debate in the literature relating to patient selection for surgery remains unresolved, and recently there seems to be a trend increasingly favorable towards early surgical intervention. A late post-traumatic diaphragmatic hernia revealed during pregnancy by post-partum respiratory distress.

INTRODUCTION: Diaphragmatic hernia during pregnancy is uncommon and is usually traumatic in origin, epigastric pain, and vomiting could be the initial symptoms of herniation of gastrointestinal contents, with a risk of strangulation and ischaemia, leading to respiratory distress due to collapse of the lung. METHODS: Case report. RESULTS: A 27-year-old woman, with undiagnosed traumatic diaphragmatic hernia who presented, at 32 weeks' gestation, epigastric pain, vomiting and tachycardia, immediate post-partum course was complicated by respiratory failure. A chest X-ray showed an air fluid leve

20 mm lithium button battery causing an oesophageal perforation in a toddler: lessons in diagnosis and treatment.

Swallowed button batteries (BB) which remain lodged in the oesophagus are at risk of serious complications, particularly in young children. The authors report a 3-year-old child, who rapidly developed an oesophageal perforation, following the ingestion of a 20-mm lithium BB which was initially mistaken for a coin. A thoracotomy and T-tube management of the perforation led to a positive outcome. BBs (20 mm) in children should be removed quickly and close observation is required as the damage initiated by the battery can lead to a significant injury within a few hours.

Esophageal Perforation With Right-Sided Hemothorax in a Patient With Suspected Variceal Bleeding: A Diagnostic Challenge.

Esophageal perforation (Boerhaave syndrome) is a rare but life-threatening condition that may mimic other causes of upper gastrointestinal bleeding, making early diagnosis challenging. We report a 76-year-old woman with disseminated breast cancer who presented with massive hematemesis and profound hemodynamic instability, initially suspected to represent variceal bleeding. Emergency endoscopy achieved hemostasis at the gastroesophageal junction but did not identify a perforation. During anesthesia, point-of-care ultrasound revealed a large right-sided pleural effusion, prompting CT imaging. CT
