We report the cases of six patients with AIDS in whom reactive hemophagocytic syndrome (RHPS) secondary to disseminated histoplasmosis was diagnosed. RHPS was diagnosed by established criteria, including fever (duration of > or = 7 days, with peak temperatures of > 38.5 degrees C), unexplained thrombocytopenia with anemia and/or neutropenia, and bone marrow biopsy findings of hemophagocytic histiocytosis. Disseminated Histoplasma capsulatum infection was diagnosed on the basis of the results of cultures of the bone marrow sample. The serum lactate dehydrogenase (LDH) level was elevated (> 1,00

INTRODUCTION: Rheumatoid arthritis (RA) is a chronic systemic autoimmune disease, which is mainly characterized by joint swelling, pressure pain and joint destruction. Some patients may suffer from a variety of serious complications, which require prompt diagnosis and treatment. Otherwise, the patient condition may deteriorate rapidly, leading to premature death.
OBJECTIVE: We reported a case of RA combined with hyperferritinemic syndrome and capillary leak syndrome (CLS) that was successfully treated with tocilizumab (TCZ), with the aim of improving diagnostic ideas for clinicians and conseq

Hemophagocytic lymphohistiocytosis (HLH) is a fatal disease characterized by a highly inflammatory state due to the abnormal activation of T lymphocytes and macrophages. Miliary tuberculosis (MTB) is a rare cause of HLH and its clinical appearances occasionally resembles that of intravascular lymphoma (IVL). A 76-year-old woman presented with persistent fever and fatigue. Abnormal laboratory findings showing thrombocytopenia (13,000/μL), hypofibrinogenemia (101 mg/dL), hyperferritinemia (2,312 ng/mL), and markedly elevated soluble interleukin-2 receptor (sIL-2R) level (32,200 U/mL), in additio

While it took decades to arrive to a conclusion that ferritin is more than an indicator of iron storage level, it took a short period of time through the COVID-19 pandemic to wonder what the reason behind high levels of ferritin in patients with severe COVID-19 might be. Unsurprisingly, acute phase reactant was not a satisfactory explanation. Moreover, the behavior of ferritin in patients with severe COVID-19 and the subsequent high mortality rates in patients with high ferritin levels necessitated further investigations to understand the role of ferritin in the diseases. Ferritin was initiall

Hemophagocytic lymphohistiocytosis (HLH) is a rare but life-threatening hyperinflammatory syndrome characterized by dysregulated immune activation and cytokine storm, frequently triggered by infections, autoimmune disorders, and malignancies. Among adults, lymphoma-associated HLH represents one of the most aggressive subtypes and carries significant mortality if not recognized early. We report the case of a 24-year-old male who initially presented with constitutional symptoms, progressive jaundice, and biochemical features suggestive of acute hepatitis. Initial evaluation focused on hepatic an

Adult-onset Still's disease (AOSD) is a rare systemic autoinflammatory disorder characterized by fever, rash, arthritis, and hyperferritinemia, often presenting as a diagnostic challenge due to its nonspecific manifestations. We report the case of a 35-year-old previously healthy male who presented with a 20-day history of pruritic erythematous skin lesions, high-grade fever, severe odynophagia, and symmetric inflammatory polyarthritis. Laboratory evaluation revealed leukocytosis with neutrophilia, elevated acute-phase reactants, liver enzyme abnormalities, and hyperferritinemia (1,050 ng/mL).

Hemophagocytic lymphohistiocytosis (HLH) is a rare, life-threatening hyperinflammatory syndrome triggered by viral infections and malignancies, as well as tick-borne pathogens such as Babesia microti. Babesiosis-associated HLH is uncommon and can be difficult to recognize, particularly in immunocompetent individuals, due to clinical overlap with sepsis. We report a case of a 32-year-old previously healthy male presenting with fever, cytopenias, transaminitis, and hyperferritinemia, found to have babesiosis with low-level parasitemia (0.01%). Despite appropriate antimicrobial therapy with atova

BACKGROUND: Ferritin and prolactin have been associated with active autoimmune diseases as systemic lupus erythematosus and autoantibody production, but have been little studied in viral infections that present autoimmunity.
OBJECTIVE: To investigate the association of these two autoimmune mediators with the presence of cryoglobulinaemia and non-organ-specific autoantibodies (RF, SMA, β2GPI IgA antibody and ANA) in Brazilian individuals chronically infected with hepatitis C virus (HCV).
METHODS: Ninety-nine patients were evaluated. Ferritin and prolactin levels were determined by chemilumine

Hemophagocytic lymphohistiocytosis (HLH) is a rare, life-threatening hyperinflammatory syndrome characterized by uncontrolled immune activation and cytokine storm. Dengue, a flavivirus transmitted by Aedes mosquitoes, is usually self-limiting but may rarely trigger secondary HLH. Persistent fever accompanied by cytopenias, extreme hyperferritinemia, transaminitis, and hypofibrinogenemia should raise suspicion for this diagnosis. Although dengue is not endemic to the United Arab Emirates, a significant outbreak followed unprecedented rainfall and flooding in April 2024, resulting in a surge in

Human granulocytic anaplasmosis (HGA) is a tick-borne, infectious disease caused by Anaplasma phagocytophilum that generally presents with nonspecific symptoms such as fever, chills, headache, malaise, and myalgia. If not treated immediately, HGA can cause hemophagocytic lymphohistiocytosis (HLH), a well-documented but underrecognized sequela of severe HGA. In this article, we report a case of severe HGA with hyperferritinemia in a 74-year-old male from Central Pennsylvania who initially presented with recurrent fevers, nausea, and malaise to our emergency department and was subsequently disch

It is still debatable whether serum ferritin is a potential prognostic marker in patients with decompensated cirrhosis. In this meta-analysis, we hope to investigate the relationship between elevated serum ferritin and the risk of death in patients with decompensated cirrhosis. We systematically searched PubMed, Embase, Web of Science, Cochrane Library, CNKI, SinoMed, WAN FANG, and ClinicalTrials.gov without language restrictions from inception to 3 October 2022, and finally identified a total of eight eligible studies with 1829 patients. The pooled prevalence of elevated serum ferritin in dec

Heme oxygenase (HO)-1, a heme-degrading enzyme inducible by various stimuli, plays a key role in the regulation of inflammatory response in monocytes/macrophages. The serum HO-1 level is remarkably increased in patients with secondary hemophagocytic syndrome (HPS) or adult-onset Still's disease. We measured serum HO-1 levels in patients with a variety of hematological diseases, including secondary HPS, by means of ELISA. Serum HO-1 levels were significantly higher in 22 patients with HPS (134.7 +/- 116.2 ng/mL, P < 0.0001) at diagnosis than in 80 patients with other hematological diseases. The

Hemophagocytic lymphohistiocytosis (HLH) is an inflammatory condition that may run a rapid fatal course and calls for prompt diagnosis. Early intervention with steroids and other immunosuppressive drugs can contain the disease process and favours positive outcome. Ferritin ≥500 ng/ml is a HLH diagnostic criterion. We evaluated the diagnostic potential of admission ferritin, in children with HLH. Pediatric patients of a referral teaching hospital from Feb 2010-Oct 2013 having been investigated for ferritin on admission were included. HLH was confirmed when patients had clinical features and met

We present a diagnostically challenging case of a 51-year-old woman with systemic diffuse large B-cell lymphoma (DLBCL) that progressed to secondary central nervous system (CNS) involvement. The initial presentation was notable for a whole-body PET/CT scan showing only subtle, diffuse fluorodeoxyglucose uptake in lymph nodes and bone marrow (SUVmax <5.0), below conventional thresholds for malignancy. Four months later, the patient developed an acute encephalopathic illness accompanied by multiorgan dysfunction and a severe hyperinflammatory state consistent with hemophagocytic lymphohistiocyto

Burnout and trauma are often framed as psychosocial conditions or as dysregulation of the hypothalamic-pituitary-adrenal (HPA) axis. Yet across more than two decades of clinical observation, I have repeatedly encountered a recurring metabolic signature that does not fit existing frameworks: persistent hyperferritinemia without hemochromatosis or overt inflammation, coexisting with low dehydroepiandrosterone-sulfate (DHEA-S) and preserved but gradually declining cortisol dynamics. This constellation is frequently observed in neurodivergent individuals and their families, with early signs alread

BACKGROUND: Hemophagocytic lymphohistiocytosis (HLH) is a life-threatening hematologic disorder resulting from an ineffective and pathologic activation of the immune response system that may mimic common emergency department presentations, including sepsis, acute liver failure, disseminated intravascular coagulation, and flu-like illnesses such as coronavirus disease 2019 (COVID-19).
OBJECTIVE: This narrative review provides a summary of the disease and recommendations for the recognition and diagnostic evaluation of HLH with a focus on the emergency clinician.
DISCUSSION: Though the conditi

A 53-year-old White man underwent chest radiography as part of a diagnostic evaluation prompted by the incidental detection of hyperferritinemia (838 ng/mL; normal values [nv]: 24-300 ng/mL) during routine blood tests. It was prescribed by the general practitioner as part of the initial diagnostic workup, in the suspicion of a possible underlying pulmonary infectious/inflammatory condition.
