Imiquimod 5% cream for isolated lichen planus of the lip.

Lichen planus (LP) of the lips is a rare condition that is generally associated with other parts of the oral mucosa. Lip localization has an increased risk, since external trauma, smoking and ultraviolet light trigger malignant transformation. Only a few cases of isolated LP of the lips have been reported up to now, but results of larger series on oral LP suggest that it might be underestimated. Treatment of oral LP is usually difficult and lesions are generally resistant or recur, so that novel therapy alternatives are necessary. Here we report four cases of isolated LP of the lip successfull

Treatment of human papillomavirus in childhood with imiquimod 5% cream.

In children, lesions caused by the human papillomavirus (HPV) constitute a significant epidemiological issue and a therapeutic dilemma, particularly in the case of anogenital warts. The treatment of anogenital warts in children is a challenge, since standard treatments are generally painful and require the patient to be anesthetized. Imiquimod, a topical immune response modifier, constitutes an alternative therapeutic agent for the treatment of HPV. The present report describes four cases in which treatment with topically applied imiquimod 5% cream was implemented with successful results.

Complete resolution of Paget disease of the vulva with imiquimod cream.

Extra mammary Paget disease (EMPD) is a neoplastic disease of apocrine gland bearing skin. Surgical excision is the standard of care for EMPD; however, it is accompanied by recurrence in more than 60% of the patients. Recently, imiquimod cream has been reported to induce complete responses in primary or recurrent EMPD. We report on 2 women with vulva EMPD who achieved biopsy-confirmed resolution of their disease after topical application of imiquimod 5% cream.

Update on actinic keratosis in clinical trial experience with imiquimod.

Actinic keratoses (AK) is a sun induced cutaneous lesion, currently considered as a squamous cell carcinoma in situ that has the potential to progress to invasive SCC. To treat them, numerous, ablative and no ablative, therapeutic approaches exist. Among them, imiquimod, a toll like receptor agonist, recently approved to treat them in the US and Europe, has demonstrated to be effective and safe with an acceptable tolerability in the treatment of these lesions. In this article the results of several high quality RCT are analysed.

Aphthous ulcers associated with imiquimod and the treatment of actinic cheilitis.

Our case series report is the first documented depiction of the appearance of aphthous ulcers secondary to imiquimod application. This case series presentation discusses the underlying pathophysiology of aphthous ulcer development and imiquimod therapy in terms of the stimulation of proinflammatory cytokines, such as tumor necrosis factor alpha (TNF-alpha). The literature review suggests more than just a mere coincidence for the development of aphthous ulcers subsequent to the treatment of actinic cheilitis with imiquimod application.

Imiquimod 5 percent cream and the treatment of cutaneous malignancy.

Imiquimod is a novel synthetic compound that is a member of the imidazoquinolone family of drugs. This class of drugs, which also includes a more potent member, resimiquimod (R-848), is unique having the properties of topical immune response modifiers and stimulators. Imiquimod resembles a nucleoside analogue and is known for its potent induction of endogenous antiviral pro-inflammatory mediators. This article is a review of the relevant literature as it relates to the off-label applications of imiquimod 5 percent cream for treatment of cutaneous preneoplastic and neoplastic conditions includi

Bowenoid papulosis of the vulva-immunotherapeutical approach with topical imiquimod.

INTRODUCTION: Bowenoid papulosis is a characteristic lesion of the ano-genital region and represents a form of squamous cell carcinoma in situ, very often associated to the oncogenic high-risk human papilloma virus (HPV) types 16, 18, 31 and 33. Therapies applied so far, in general, show high rates of relapse, and patients complain of pruritus and pain. Imiquimod cream is a topical immune response modifier with indirect antiviral and antitumor effects through the stimulation of local cytokine production and cell-mediated immune response. CASE REPORT: In the present paper we report on the topi

[Imiquimod in the treatment of recalcitrant warts: a new therapy option?].

Recalcitrants viral warts can pose a therapeutic challenge to the treating physician. In a clinical study, we documented the effect of imiquimod 5% cream (Aldara) on recalcitrants warts in 22 patients. A complete and partial clearing of the warts was achieved in 41% and 50%, respectively. In only 9% of patients no improvement could be observed. Beside the very often observed perilesional erythema, which correlated well with good treatment response, no relevant side effects were documented. Imiquimod 5% cream (Aldara) is an efficient, easy to perform ambulatory treatment modality for recalcitra

A review of the applications of imiquimod: a novel immune response modifier.

Imiquimod (1-(2-methylpropyl)-1 H-imidazo[4,5-c]quinolin-4-amine) and its analogues are a class of non-nucleoside imidazoquinolinamines (hetero-cyclic amine) that activate the immune system through localised induction of cytokines, such as IFN-alpha, -beta, and a number of endogenous interleukins. The exact mechanism of its actions are still unexplored, although when tested in a number of cell culture systems, imiquimod demonstrated no inherent antiviral or antiproliferative activity in vitro, whereas, due to its reported ability to produce onsite stimulation and secretion of cytokines in vari

Imiquimod for the treatment of genital warts: a quantitative systematic review.

OBJECTIVE: To review published randomised controlled trials to assess the benefit and harm of imiquimod in the treatment of external genital warts. DATA SOURCES: MEDLINE (1966 - December 2000), Cochrane Library (Issue 3, 2000) and PubMed (December 15, 2000), review articles and reference lists. REVIEW METHODS: Included studies had to be randomised trials of imiquimod, to be full published papers, and to have a comparison group. Quality of trial reporting was assessed. Relative benefit and number needed to treat were calculated for the main outcomes of wart clearance at the end of therapy, of

Bcl-3 expression in the imiquimod-induced psoriasis mouse skin.

OBJECTIVE: Psoriasis is an inflammatory process in the skin involving the loss of normal function of immune cells and keratinocytes. TNF-α is a pro-inflammatory cytokine involved in psoriasis induced by NF-κB activation. In contrast, the anti-inflammatory cytokine IL-10 can suppress macrophage TNF-α production. The non-classical NF-κB regulator, Bcl-3, can regulate IL-10 expression. Limited knowledge exists on Bcl-3 and IL-10 expression in psoriasis and their action on TNF-α. This study evaluated Bcl-3, TNF-α, and IL-10 expression levels in mouse skin in a model of imiquimod-induced psoriasis.

Stevens-Johnson induced by imiquimod 5% cream: a case report.

Imiquimod 5% cream is an approved treatment for actinic keratoses, superficial basal cell carcinomas, and anogenital warts. Severe systemic side effects associated with imiquimod 5% cream are rare, although a few cases of erythema multiforme and Stevens-Johnson syndrome have been described. We present a case of Stevens-Johnson syndrome associated with topical treatment with imiquimod of two superficial basal cell carcinomas.

Pediatric erythema multiforme induced by topical imiquimod: a case report.

Erythema multiforme (EM) is a rare immune-mediated reaction with mucocutaneous involvement. This case report describes the development of EM in a 9-year-old girl after treatment with imiquimod, a topical immunomodulator used in the management of some dermatologic conditions. Because imiquimod-related EM is rare, particularly in children, this article also reviews the potential adverse effects of this drug and the main characteristics of imiquimod-induced EM, especially in similar cases reported in the literature. Clinicians should be aware that topical imiquimod can induce EM, and this medicat

Immunological Memory in Imiquimod-Induced Murine Model of Psoriasiform Dermatitis.

Psoriasis is a common chronic inflammatory skin condition manifested by T cell responses and characterized by preferential recurrence at previously inflamed sites upon withdrawal of treatment. The site-specific disease memory in psoriasis has been linked to CD8+CD103+ tissue-resident memory T cells (Trm) in the epidermis which were previously thought to only provide "frontline" protection against pathogens and immunosurveillance during cancer development. In this study, we correlated the presence of a subset of the Trm cells which are also CD49a+ with disease severity in human psoriatic lesion

Successful treatment of chromoblastomycosis caused by Fonsecaea pedrosoi using imiquimod.

Chromoblastomycosis (CBM) is a fungal infection caused by fungi belonging to the order Chaetothyriales, and caused mainly by Fonsecaea pedrosoi. The classic treatment, based on itraconazole and/or terbinafine as well as physical approaches, is considered complex and ineffective due to the high rate of relapses. Thus, new strategies are needed to manage CBM; in this regard, the present work reports the evolution of lesions in patients successfully treated with imiquimod. Of note, classic treatment was not effective in healing the lesions of two of them, but single topical treatment with imiquim

Multicentric Anogenital Bowen's Disease Treated with Imiquimod and CO2 Laser.

A 53-year-old white woman presented to our clinic with skin lesions in the anogenital region that had persisted for 1 year. She had a past medical history of total vulvectomy for a vulvar localization of Bowen's disease. She was otherwise in good health, with no evidence of illness or immunosuppression. Physical examination revealed multiple erythematous papular lesions located in the anogenital region (Figure 1). Dermatoscopy of the anogenital papules revealed glomerular vessels on an erythematous background typical of Bowen's disease (Figure 2A). There were no palpable inguinal lymph nodes.
