The nephrotic syndrome persists unchanged after the initial course of steroid therapy in 15 to 20% of patients. In such cases, the severity of the condition lies essentially in the risk of developing end-stage renal failure. This occurs in one third to one half of the cases. Furthermore, some of these children are at risk for recurrence of their original disease in the transplanted kidney. The course of steroid resistant nephrotic syndrome (SRNS) is usually punctuated by the need for numerous hospitalizations for mobilization of edema and treatment of infection. The treatment of patients with

Medicinal plants have long been recognized for their therapeutic benefits, particularly for their bioactive compounds that enhance quality of life. These plants and their derived compounds possess numerous pharmacological properties, including the modulation of immune system components. Immune responses are crucial for resolving oral infections caused by pathogens such as bacteria, fungi, and viruses, as well as for preventing or managing oral cancer. A robust immune system plays a central role in defending against these stimuli, making it essential to explore and promote the use of medicinal

BACKGROUND: Triptolide (TP), a potent bioactive compound from the Chinese herb Tripterygium wilfordii, demonstrates strong anti-inflammatory and immunosuppressive effects. Evidence indicates that TP modulates macrophage polarization and regulates inflammatory responses through multiple signaling pathways, exerting significant immunomodulatory effects in various diseases.
OBJECTIVE: This review summarizes the immunomodulatory mechanisms of TP on macrophage function, with a focus on its therapeutic potential in diverse diseases. We also explore strategies to mitigate TP's toxicity, including fo

Viral diseases, whether pandemic, endemic, or epidemic, are a leading cause of global mortality and disability. Consequently, developing effective viral inhibitors is a critical public health priority. Beyond antiviral drugs, a promising therapeutic strategy involves using immunomodulators, which are antiviral agents that enhance the host's immune system against infection. Phytochemicals (PCHs) derived from plants exhibit diverse bioactive properties, including significant antioxidant and immunomodulatory effects. Notably, PCHs have attracted considerable attention due to their broad-spectrum

Drug delivery strategies for local immunomodulation hold tremendous promise compared to current clinical gold-standard systemic immunosuppression as they could improve the benefit to risk ratio of life-saving or life-enhancing transplants. Such strategies have facilitated prolonged graft survival in animal models at lower drug doses while minimizing off-target effects. Despite the promising outcomes in preclinical animal studies, progression of these strategies to clinical trials has faced challenges. A comprehensive understanding of the translational barriers is a critical first step towards

Inflammatory bowel disease (IBD) is an autoimmune disorder marked by chronic inflammation affecting the intestines. Crohn's disease (CD) and ulcerative colitis (UC) fall under the IBD umbrella, necessitating diverse treatments, including steroids, immunomodulators like 6-mercaptopurine (6-MP) and azathioprine (AZA), and biological agents. The prolonged use of immunomodulators, such as AZA, is associated with an elevated risk of developing lymphomas. This case report centers on a 77-year-old gentleman regularly monitored by Gastroenterology, undergoing long-term AZA therapy for CD management. H

Modern sun-protection products reduce the risk for erythema and DNA damage, but even those products with a very high sun protection factor (SPF) and full-spectrum UVB and UVA protection may not prevent UV radiation (UVR)-induced immunomodulation. Formulating sunscreens with a high SPF, as well as a high immune protection factor, is necessary for preventing skin cancer and maintaining effective immune responses to infectious disease after sun exposure. Supplementing current sun-protection products with immunoprotective compounds may help fill the gap between erythema protection and immunoprotec

BACKGROUND: Many treatment options exist for children with inflammatory bowel disease (IBD), yet the lack of clinical guidelines for management has lead to great variation in care. The purpose of this project was to evaluate current treatment modalities in children from the Northeast US who applied to the 2010 session of Camp Oasis, a Crohn's and Colitis Foundation of America (CCFA)-sponsored camp for children ages 8-17 with medically stable IBD.
METHODS: Patient demographics, medical history, and current medications were entered into the camp database. The subjects were divided into two grou

Pancreatic ductal adenocarcinoma (PDAC) is one of the most lethal malignancies and is a rising cause of morbidity and mortality. An immunosuppressive, hostile tumor microenvironment, and KRAS-driven biology have contributed to poor outcomes in PDAC. Recent breakthroughs in targeting tumors with homologous repair deficiency, KRASG12C mutations, rare gene fusions, and other molecular abnormalities have improved outcomes in subsets of patients. KRAS inhibitors, both allele specific and pan(K)RAS, claudin-targeting biologics, and PRMT5 inhibitors have demonstrated single agent activity in pretreat

Steroids and immunosuppressants are indicated to treat systemic vasculitides. However, the therapeutic strategy is different from one disease to the other. Treatment choice should be adapted to the predictable outcome, severity, pathogenic mechanisms and patient's general condition. In polyarteritis nodosa, Churg Strauss syndrome, and microscopic polyangiitis we have demonstrated that immunosuppressants should not be systematically prescribed. Immunosuppressants should be only prescribed in the most severe patients, when factors of poor prognosis are present. In Wegener's granulomatosis, immun

The therapeutic strategy in systemic necrotizing vasculitides comprises steroids and, in severe forms, immunosuppressants like cyclophosphamide. A combination of steroids and immunosuppressants should be proposed for severe forms only. When factors of poor prognosis are absent, steroids alone can be prescribed. When vasculitides are related to viral infection, a specific approach is needed that comprises a combination of antiviral treatments and plasma exchanges. In Wegener's granulomatosis, the combination of steroids and cyclophosphamide, either oral or pulse, should be prescribed. A mainten

Recent therapeutic trials in rheumatology using different immunomodulating agents have given encouraging results. In this study an aqueous calf thymus extract (CTE) was administered to three patients, two with systemic juvenile chronic arthritis (JCA), Still's disease, who could not be weaned from steroids during 2 years of conventional therapy, and one girl with a chronic juvenile monarthritis who had responded unsatisfactorily to nonsteroidal antirheumatics for 19 months. A striking clinical improvement was observed in all three patients. Prednisone (PRED) was discontinued in one case with s

Immunomodulators and biological agents are effective for the treatment of ulcerative colitis and Crohn's disease; however, there is concern that these therapies may be associated with an increased risk of malignancy. MEDLINE, Cochrane Library, and Web of Science were searched for articles regarding these medications and their associations with hematologic malignancies and solid tumors in inflammatory bowel diseases (IBDs) and transplant, rheumatology, dermatology, and neurology patient populations. There is evidence that use of thiopurines and anti-tumor necrosis factor (TNF) agents is associa

Although the remarkable efficacy of biological therapy has resulted in significant success in inflammatory bowel disease (IBD) management, susceptibility to infections remains a concern. The biological agents include the tumor necrosis factor-alpha (TNF-alpha) inhibitors, for instance infliximab, and other immunomodulating agents, such as natalizumab. Progressive multifocal leukoencephalopathy (PML), a rare but mostly fatal opportunistic brain infection caused by reactivation of the human polyomavirus JC virus (JCV), has been found in two patients with multiple sclerosis and one patient with C

Long-term treatment with ursodeoxycholic acid (UDCA; 13-15 mg/kg/day) in patients with primary biliary cirrhosis (PBC) improves biochemical liver tests, delays histological progression and prolongs survival without liver transplantation. UDCA monotherapy appears sufficient for many patients as suggested by long-term observational data. However, the transplant-free survival rate of UDCA-treated patients remains significantly lower than that of an age- and sex-matched control population. Therefore, there is a continued need for new therapeutic options in PBC. In this article we review and discus

Non-surgical treatments for moderate to severe and active Graves' orbitopathy (systemic glucocorticoids with or without orbital radiotherapy) have limited effects on the underlying autoimmune process causing the disease. Although the clinical responses to treatment are often good, at least one third of patients with Graves' orbitopathy are eventually dissatisfied with the treatment outcome. Progress in our understanding of the autoimmune basis of Graves' orbitopathy (although still incomplete) made it possible, similar to other autoimmune disorders, to envision the use of novel immunomodulatin

Overall fifty patients with chronic hepatitis and 50 patients with concurrent pathologies were examined. The patients with chronic active hepatitis and in coexisting pathologies showed more profound depression of the immunity system as well as striking alterations on the iris of the eye. The former patient group received basic therapy, while the latter one were given immunomodulating agents against the background of basic therapy. Iridodiagnosis will, we believe, help in detecting concurrent pathologies of the hepatobiliary and urinary systems. The dynamics of the pathological signs in the iri
