Extracting Concepts Related to Homelessness from the Free Text of VA Electronic Medical Records.

Mining the free text of electronic medical records (EMR) using natural language processing (NLP) is an effective method of extracting information not always captured in administrative data. We sought to determine if concepts related to homelessness, a non-medical condition, were amenable to extraction from the EMR of Veterans Affairs (VA) medical records. As there were no off-the-shelf products, a lexicon of terms related to homelessness was created. A corpus of free text documents from outpatient encounters was reviewed to create the reference standard for NLP training and testing. V3NLP Fram

A biorepository for ophthalmic surgical specimens.

Biorepositories are collections of surgically obtained human tissues for current and future investigations of disease mechanisms, therapeutics, and diagnostics. In ophthalmology, a critical challenge is how to interface the operating room with the laboratory. To attain standards required for basic research, clinical and research teams must cooperate to collect, annotate, and store specimens that yield consistent results required for advanced molecular techniques. We developed an efficient platform for obtaining vitreous and other eye tissues from the operating room and transferring them to the

Evidence-based practice: an example related to the use of splinting in a patient with carpal tunnel syndrome.

The purpose of this article was to illustrate a limited search for evidence and its application in the context of a patient with CTS. The search was by no means exhaustive, as it was performed and interpreted in less than an hour. My conclusions are not definitive because my evidence was based almost exclusively on abstracts, and many key details about the studies are unknown. I do not know how closely my specific patient matches those in a particular study. My search allowed me, however, to make an informed response to a patient with legitimate questions. Integrating the evidence with my own

Facilitating the work of a meta-analyst.

Meta-analysis facilitates the transfer of knowledge from nurse researchers to clinicians. In this article, the benefits and criticisms of meta-analysis for nursing are identified along with the specific problems a meta-analyst may encounter in conducting a quantitative analysis and synthesis of the literature. Problems in data retrieval from the primary studies for a quantitative literature review can plague a meta-analyst. These problems can include insufficient data such as inexact p-values, incomplete descriptions of samples or experimental and control groups, and errors in tables in resear

BRISK--research-oriented storage kit for biology-related data.

MOTIVATION: In genetic science, large-scale international research collaborations represent a growing trend. These collaborations have demanding and challenging database, storage, retrieval and communication needs. These studies typically involve demographic and clinical data, in addition to the results from numerous genomic studies (omics studies) such as gene expression, eQTL, genome-wide association and methylation studies, which present numerous challenges, thus the need for data integration platforms that can handle these complex data structures. Inefficient methods of data transfer and a

The consensus-based CINEX guideline for reporting clinical information extraction studies.

OBJECTIVE: Information extraction (IE) from clinical texts has advanced rapidly with recent advances in natural language processing, particularly the advent of large language models (LLMs). However, inconsistent and incomplete reporting of methodologies limits reproducibility, comparability, and clinical translation. We aimed to develop a consensus-based reporting guideline tailored to clinical IE studies. MATERIAL AND METHODS: We developed the Clinical Information Extraction Reporting Guideline (CINEX) through a multi-phase process. The initiative was prospectively registered on the EQUATOR

Characterizing complex opioid use disorder care trajectories and outcomes following acute service utilization: A protocol for a population-based data linkage study.

Despite robust evidence that medications for opioid use disorder (MOUD) reduce overdose and mortality, substantial care gaps remain following opioid-related hospital encounters. The opioid use disorder (OUD) Cascade of Care framework conceptualizes progression from identification to treatment initiation and retention, yet limited research has examined how real-world OUD treatment trajectories unfold, particularly across treatment episodes and multiple care settings. This paper describes an NIH-funded study protocol (1R01DA061367-01A1) to conduct a longitudinal observational study using linked

SBDH-Reader: a large language model-powered method for extracting social and behavioral determinants of health from clinical notes.

OBJECTIVE: Social and behavioral determinants of health (SBDH) are increasingly recognized as essential for prognostication and informing targeted interventions. Clinical notes often contain details about SBDH in unstructured format. Conventional extraction methods for these data tend to be labor intensive, inaccurate, and/or unscalable. In this study, we aim to develop and validate a large language model (LLM)-powered method to extract structured SBDH data from clinical notes through prompt engineering. MATERIALS AND METHODS: We developed SBDH-Reader to extract 6 categories of granular SBDH

Language Models for Standardising Clinical Notes and Information Extraction in Addiction Psychiatry-An Empirical Study.

INTRODUCTION: Electronic health records contain both structured and unstructured data, with unstructured clinical notes widely used in addiction psychiatry. Clinical notes have numerous errors and require proofreading to ensure accuracy and readability. This study evaluates natural language processing methods and adapts a Large Language Model (LLM) for proofreading clinical notes and extracting substance-related information. METHODS: We analysed clinical notes from a 5-year addiction medicine electronic health record dataset (2018-2023), selecting 6500 notes. The proofreading task involved co

A scalable approach for critical care data extraction and analysis in an academic medical center.

BACKGROUND: Electronic health records are a valuable asset for research, but their use is challenging due to inconsistencies of records, heterogeneous formats and the distribution over multiple, non-integrated information systems. Hence, specialized health data engineering and data science expertise are required to enable research. To facilitate secondary use of clinical routine data collected in our intensive care wards, we developed a scalable approach, consisting of cohort generation, variable filtering and data extraction steps. OBJECTIVE: With this report we share our workflow of data re

Reliably Filter Drug-Induced Liver Injury Literature With Natural Language Processing and Conformal Prediction.

Drug-induced liver injury describes the adverse effects of drugs that damage the liver. Life-threatening results were also reported in severe cases. Therefore, liver toxicity is an important assessment for new drug candidates. These reports are documented in research papers that contain preliminary in vitro and in vivo experiments. Conventionally, data extraction from publications relies on resource-demanding manual labeling, which restricts the efficiency of the information extraction. The development of natural language processing techniques enables the automatic processing of biomedical tex

Establishing risk factors and outcomes for congenital hypothyroidism with gland in situ using population-based data linkage methods: study protocol.

INTRODUCTION: There has been an increase in the birth prevalence of congenital hypothyroidism (CH) since the introduction of newborn screening, both globally and in the UK. This increase can be accounted for by an increase in CH with gland in situ (CH-GIS). It is not known why CH-GIS is becoming more common, nor how it affects the health, development and learning of children over the long term. Our study will use linked administrative health, education and clinical data to determine risk factors for CH-GIS and describe long-term health and education outcomes for affected children. METHODS AND

Predicting dengue incidence leveraging internet-based data sources. A case study in 20 cities in Brazil.

The dengue virus affects millions of people every year worldwide, causing large epidemic outbreaks that disrupt people's lives and severely strain healthcare systems. In the absence of a reliable vaccine against dengue or an effective treatment to manage the illness in humans, most efforts to combat dengue infections have focused on preventing its vectors, mainly the Aedes aegypti mosquito, from flourishing across the world. These mosquito-control strategies need reliable disease activity surveillance systems to be deployed. Despite significant efforts to estimate dengue incidence using a vari

Real-world data in retinal diseases treated with anti-vascular endothelial growth factor (anti-VEGF) therapy - a systematic approach to identify and characterize data sources.

BACKGROUND: Real-world data (RWD) has been a valuable addition to the scientific literature regarding treatment pathways, clinical outcomes and characteristics of patients with retinal diseases in recent years. Registries, observational studies and patient databases are often used for real-world research. However, there is limited information for each data source on the design, consistency, data captured, limitations and usability for assessing research questions. Using a systematic approach, we identified RWD sources for patients with retinal diseases and assessed them for completeness of dat
