Orbital fat herniation is primarily in a subconjunctival location. However, herniation through the inferior orbital fissure (IOF) has been scantly reported. Here, we report a cadaveric case of herniation of orbital fat through the inferior orbital fissure and into the infratemporal fossa. The cadaver's orbital anatomy and orbital fat herniation were found during routine dissection. The details of this case are reported. We also discuss anatomical variations of the inferior orbital fissure as well as the related vascular and nervous system structures related to orbital fat herniation. This is a

The authors describe a 34-year-old man who presented with an unapparent swelling on the right preauricular zygomatic region, with progressive limitation of mouth opening and slight temporomandibular joint pain. Based on fine-needle aspiration cytology and radiological investigations, the patient was diagnosed preoperatively as having a possible tenosynovial giant cell tumour (TGCT). Surgical treatment was effective and no local recurrence was observed. Histological and immunohistochemical examination diagnosed an extra-articular diffuse TGCT. The clinical features, histopathological characteri

OBJECTIVE: In the case of deep invasion of an infratemporal fossa (ITF) tumor, surgeons find it difficult to gain sufficient visualization and working space by conventional surgical approaches. To overcome these limitations, we have developed a novel surgical technique, maxillo-orbito-zygomatic (MOZ) approach, by combining partial lateral maxillectomy with the conventional orbito-zygomatic approach.
METHODS: A 63-year-old male presented with the fifth recurrent adenoid-cystic carcinoma in the right deep ITF. Using a Weber-Ferguson-type incision and partial dismasking, we elevated the skin and

The parapharyngeal space (PPS) is a challenging anatomical region, rich in vascular and nervous vital structures. Surgery is considered the treatment of choice for the majority of PPS lesions. Herein, we present a retrospective evaluation on ten patients with various types of lesions of the parapharyngeal and infratemporal fossa (ITF) regions operated on via an endoscopic-assisted transoral-transpharyngeal approach (EATTA), focusing on feasibility and safety. A retrospective evaluation of patients treated by means of EATTA to PPS and/or ITF lesions was carried out. The clinical records of pati

Squamous cell carcinoma (SCC) of the maxilla is an uncommon malignancy that often presents at an advanced stage because of its nonspecific clinical features and proximity to adjacent anatomical structures. In rare instances, it may closely mimic chronic osteomyelitis, particularly when presenting as a non-healing extraction socket with exposed necrotic bone and extensive osteolytic destruction, leading to delayed diagnosis and treatment. We report a diagnostically challenging case of advanced maxillary SCC that initially masqueraded as extensive osteomyelitis. A 55-year-old female with a 20-ye

The infratemporal fossa is a deep anatomical space rarely involved in lymphoproliferative disorders. Small lymphocytic lymphoma (SLL), a low-grade subtype of non-Hodgkin lymphoma (NHL), is exceptionally rare in this region, with only a few cases reported in the literature. We report the first known case of a 48-year-old patient with a primary, bilateral, butterfly-shaped SLL of the infratemporal fossae, discovered incidentally during imaging after a road traffic accident. Clinical signs of infratemporal fossa involvement are often nonspecific or absent, particularly in indolent forms like SLL.

Objective: To evaluate the outcomes of a modified infratemporal fossa approach (ITFA) that preserves the posterior external auditory canal (EAC) in patients with tumors in the infratemporal fossa and skull base, focusing on postoperative hearing and facial nerve function. Methods: This retrospective study included nine patients who underwent ITFA with posterior EAC preservation for tumor removal while minimizing facial nerve rerouting. All surgeries were performed by a single surgeon. Preoperative and postoperative hearing levels, facial nerve function, tumor characteristics, and surgical outc

A 76-year-old patient presented with a recurrent mass of a malignant chondrosarcoma in the right infratemporal fossa and in the left maxillary sinus with orbital invasion. The patient was treated with a palliative intention with MR-guided laser-induced thermotherapy using a modified applicator technique. Following treatment clinical symptoms improved and MRI revealed complete laser-induced tumor necrosis.

BACKGROUND: Approaching tumors involving the infratemporal fossa (ITF) is technically challenging due to the complex relationships between several neurovascular structures and the deep-seated location of these lesions.
METHODS: The authors describe in detail a minimally invasive approach to the ITF, consisting on a single-nostril endoscopic endonasal transmaxillary approach aided by an anterior transmaxillary port (EEA-ATMa). Anatomical landmarks and surgical tenets of this approach are highlighted and reviewed with the goal to guarantee successful tumor resection and avoid operative complica

Herein, we present the case of a previously healthy 54year-old female who developed several weeks of unilateral tinnitus and aural fullness. She subsequently underwent unilateral pressure equalization tube placement at an outside institution after exam demonstrated a middle ear effusion, conductive hearing loss and normal nasopharyngoscopy. Ultimately, an MRI revealed an occult mass in the infratemporal fossa (ITF), which was successfully removed via an endoscopic transnasal ITF approach. Following resection of a histopathologically confirmed benign neurofibroma, she reported complete resoluti

OBJECTIVE: Intracranial meningiomas extending into the infratemporal fossa (ITF) are uncommon. This series describes the radiographic characteristics, histological pattern of invasion, and implications for surgical treatment of intracranial meningiomas.
METHODS: Nine patients (median age, 52 yr) underwent resection of a transcranial meningioma extending into the ITF. Five patients (56%) had undergone a previous resection; however, none had involvement of the ITF. Four patients (44%) had received prior radiation therapy to the area.
RESULTS: Preoperative neuroradiography uniformly showed eros

In order to study the change in the morphometric parameters of projection triangles of the facial skull during tooth loss in adult patient, 185 categorized skulls were studied from the craniological collection of the Museum of the Department of Normal Anatomy of the Military Medical Academy. The skulls belonged to adults aged from 22 to 75 years and were separated into two groups - with complete set of teeth and complete absence of teeth on the upper jaw (100 and 85 skulls respectively). Based on standard points, the following projection triangles were developed: lateral front triangle, anteri

The paper analyzes application of orbitozygomatic approaches at the Department of Skull Base and Craniofacial Surgery of the Burdenko Neurosurgical Institute for a 14-year period. During this time, 723 patients were operated on using the orbitozygomatic approach, which has become the workhorse of surgery for skull base tumors spreading into the orbit, paranasal sinuses, and pterygopalatine and infratemporal fossae. The authors describe seven major modifications of the orbitozygomatic approach that they have used in their practice.

OBJECTIVES: Endoscopic endonasal approaches to the pterygopalatine and infratemporal fossae are technically challenging due to the complex anatomy of these areas. This project attempts to develop an anatomic and surgical model to enhance the understanding of these spaces from the endonasal endoscopic perspective.
METHODS: Eight pterygopalatine and infratemporal fossae were dissected in four adult human specimens in accordance with institutional protocols. All specimens were prepared with vascular injections using colored latex. Both the pterygopalatine and infratemporal fossae were accessed u

OBJECTIVE: Ossification of pterygoalar and pterygospinous ligaments traversing the superior aspect of the infratemporal fossa results in formation of osseous bars that can obstruct percutaneous needle access to the trigeminal ganglion through the foramen ovale (FO), interfere with lateral mandibular nerve block, and impede transzygomatic surgical approaches. Presence of these ligaments has been studied on dry skulls, but description of their radiological anatomy is scarce, in particular on cross-sectional imaging. The aim of this study was to describe visualization of pterygoalar and pterygosp

We report about ectopic glial tissue of the skull base and the parapharyngeal space presenting as a soft tissue mass in the middle ear. An 11-year-old boy presented with bilateral conductive hearing loss since early childhood. The history included previously removed lesions consistent with ectopic neuroglial tissue of the tongue and the parapharyngeal space soon after birth, as well as surgery for cleft palate. High resolution computed tomography of the petrous bone and magnetic resonance imaging were useful in identifying the skull base defect and in characterizing the lesion's relation to th
