Current issues in the respiratory care of patients with amyotrophic lateral sclerosis.

Amyotrophic lateral sclerosis is a progressive neuromuscular disease, resulting in respiratory muscle weakness, reduced pulmonary volumes, ineffective cough, secretion retention, and respiratory failure. Measures as vital capacity, maximal inspiratory and expiratory pressures, sniff nasal inspiratory pressure, cough peak flow and pulse oximetry are recommended to monitor the respiratory function. The patients should be followed up by a multidisciplinary team, focused in improving the quality of life and deal with the respiratory symptoms. The respiratory care approach includes airway clearance

Resting Hyperinflation Predicts Incremental Shuttle Walk Distance in Chronic Obstructive Pulmonary Disease.

Rationale: The incremental shuttle walk test (ISWT) correlates closely with peak oxygen uptake in COPD and relates to important outcomes such as mortality, response to treatment and hospital readmission. Despite this, there is limited data on the physiological determinants of ISWT distance (ISWD) in COPD. Methods: In this exploratory, prospective observational study, spirometry, lung volumes, diffusion capacity (DLCO) and oscillometry were performed in patients with confirmed COPD. Patients then completed two ISWT with the results of the best test, measured by distanced walked taken. The deter

Physiological underpinnings of exertional dyspnoea in mild fibrosing interstitial lung disease.

The functional disturbances driving "out-of-proportion" dyspnoea in patients with fibrosing interstitial lung disease (f-ILD) showing only mild restrictive abnormalities remain poorly understood. Eighteen patients (10 with idiopathic pulmonary fibrosis) showing preserved spirometry and mildly reduced total lung capacity (≥70% predicted) and 18 controls underwent an incremental cardiopulmonary exercise test with measurements of operating lung volumes and Borg dyspnoea scores. Patients' lower exercise tolerance was associated with higher ventilation (V̇E)/carbon dioxide (V̇CO2) compared with con

Peak inspiratory flow in Austrian patients with asthma and/or COPD using dry powder inhalers- a cross-sectional, observational real-world study.

BACKGROUND: Dry powder inhalers (DPIs) prescribed for respiratory diseases require patients to generate sufficient peak inspiratory flow (PIF) for optimal drug delivery to the airways. However, information is scarce regarding PIF of asthma and COPD patients in real-world practice and whether prescribed devices can be properly used. METHODS: The observational, non-interventional real-world study PIF NIS included Austrian asthma and COPD patients regularly using a DPI and assessed PIF with the In-Check DIAL G16 inhalation airflow training meter. RESULTS: The study examined 134 asthma patients

Airflow Shape Is Associated With the Pharyngeal Structure Causing OSA.

BACKGROUND: OSA results from the collapse of different pharyngeal structures (soft palate, tongue, lateral walls, and epiglottis). The structure involved in collapse has been shown to impact non-CPAP OSA treatment. Different inspiratory airflow shapes are also observed among patients with OSA. We hypothesized that inspiratory flow shape reflects the underlying pharyngeal structure involved in airway collapse. METHODS: Subjects with OSA were studied with a pediatric endoscope and simultaneous nasal flow and pharyngeal pressure recordings during natural sleep. The mechanism causing collapse was

The mechanism of respiratory arrest in inspiratory loading and hypoxemia.

Respiratory arrests occur in the clinical setting of respiratory failure, but the mechanism is unclear. We used a dog model with increased inspiratory resistance and hypoxemia to explore the cause. We hypothesized that respiratory muscle fatigue (RMF) played a role in these respiratory arrests, and that the combination of hypoxia and resistive loading would produce respiratory arrest by the mechanism of RMF. Our preparation had transdiaphragmatic pressures that were 40% of maximum (Pdimax = 46.3 +/- 10.0 cm H2O) and progressive hypoxia resulting in a final arterial PO2 of 38 +/- 9 mm Hg and a

Peak Inspiratory Flow in COPD Patients Treated with Pressurized Metered-Dose Inhalers and Spacers: Implications for Inhaler Selection. A Multicenter Cross-Sectional Study.

BACKGROUND: Spacer devices are frequently prescribed with pressurized metered-dose inhalers (pMDIs) in patients with COPD for a variety of clinical and practical reasons, including concerns about inspiratory capacity, inhaler technique, and hand-breath coordination. However, inspiratory flow is rarely measured objectively. PURPOSE: To determine whether COPD patients treated with pMDIs and spacers can generate adequate peak inspiratory flow (PIF) for dry powder inhaler (DPI) use, to evaluate the impact of incomplete exhalation, and to examine patient perceptions of spacer use. Peak nasal inspiratory flow measurement and visual analogue scale in a large adult population.

OBJECTIVES: Nasal obstruction is the most common symptom in nasal diseases. It can be evaluated objectively, that is by means of peak nasal inspiratory flow (PNIF) measures and/or subjectively by means of validated questionnaires. However, it has been reported that there is a lack of reliable correlation between subjective and objective measurements of nasal obstruction. The aim of the present study was to evaluate the correlation between PNIF measurements and the subjective sensation of nasal obstruction measured by means of a visual analogue scale (VAS) in a large population of consecutive r

Evaluation of Airway Volume in Cleft Lip and Palate Following Nasoalveolar Molding.

Patients with cleft lip and palate (CLP) have commonly reduced nasal airways and are more prone to snoring, mouth breathing and hypopnea during sleep. Therefore, the morphometric evaluation of pharyngeal airway in patients with CLP is crucial. The purpose of this study is to evaluate the pharyngeal airway volumes of patients with CLP who underwent nasoalveolar molding (NAM) and to compare them with a well-matched control group without NAM. The study consisted of 40 patients with CLP divided into 2 main groups (26 with NAM; 14 without NAM) and 4 subgroups (15 unilateral CLP [UCLP] with NAM, mea

Consecutive transcranial magnetic stimulation twitches reduce flow limitation during sleep in apnoeic patients.

What is the central question of this study? A transcranial magnetic stimulation (TMS)-induced twitch applied on isolated single breaths over the motor cortex somatotopic representation of the tongue briefly recruits submental muscles and improves airflow dynamics of flow-limited respiratory cycles without arousing sleep apnoea patients. However, the mechanical impact of the TMS-induced twitch applied during consecutive breathing cycles on airflow dynamics remains unknown. What is the main finding and what is its importance? Our results show that application of TMS with the stimulator output se
