Surviving Separation: Laryngotracheal Injury in Blunt Neck Trauma-A Case Report and Review.

Laryngotracheal trauma is a rare but fatal cause of injury in patients involved in polytrauma. Most patients succumb on site due to airway compromise. Thus, a delay in instituting interventions in such patients often leads to poor outcomes. In the handful that do make it to the hospital, there is a paucity of literature with regard to the optimal management of such conditions. In this case report, we present our management approach and highlight the dangers of dealing with such a case. We present a case of a young Asian male with complete laryngotracheal transection following blunt trauma in a

Pain Management and Substance Use Disorders: A Position Statement.

The American Society for Pain Management Nursing and the International Nurses Society on Addictions hold the position that persons with co-occurring pain and substance use disorder have the right to be treated with dignity and respect and receive evidence-based, high-quality assessment and management for both conditions using an integrated, holistic, multidimensional approach. Nonopioid and nonpharmacological approaches to pain management are recommended. Opioids should not be withheld from anyone if necessary to treat pain, and a team-based approach, including pain and addiction specialists,

Implementation of evidence-based practice guidelines to prevent urinary retention in hospitals: A process evaluation in orthopaedic care.

BACKGROUND AND OBJECTIVES: National clinical practice guidelines promote evidence-based practice, though are hardly implemented without local facilitation. While further research is needed as for what facilitation strategies work, in what context, and with what outcomes, the Onset PrevenTIon of urinary retention in Orthopaedic Nursing and rehabilitation (OPTION) trialled a tailored implementation intervention for evidence-based clinical practice guidelines in orthopaedic care. Informed by the principles of realism, this paper describes the evaluation of the context, mechanisms, and outcomes tr

The infrastructure of electrophysiology centers impacts the management of cardiac tamponade-Results from a national survey.

BACKGROUND: Although electrophysiological (EP) centers have institutional standards, evidence on management of cardiac tamponade is lacking. AIM AND METHODS: A physician-based survey was conducted by sending out questionnaires to all hospitals in Germany performing EP procedures. To evaluate the infrastructure of EP centers and the impact of center volume and onsite cardiac surgery on the management of cardiac tamponade, the results of the survey were analyzed for low-volume (0-250 procedures per year), mid-volume (250-500 procedures), and high-volume (>500 procedures) centers, as well as for

Simulation as a Tool to Facilitate Practice Changes in Teams Taking Care of Patients Under Investigation for Ebola Virus Disease in Spain.

INTRODUCTION: We analyzed the impact of simulation-based training on clinical practice and work processes on teams caring for patients with possible Ebola virus disease (EVD) in Cantabria, Spain. METHODS: The Government of Spain set up a special committee for the management of EVD, and the Spanish Ministry of Health and foreign health services created an action protocol. Each region is responsible for selecting a reference hospital and an in-house care team to care for patients under investigation. Laboratory-confirmed cases of EVD have to be transferred to the Carlos III Health Institute in

Impact of context on nurse-led implementation of evidence-based pain management practices in the Sumamos Excelencia project: a qualitative study.

BACKGROUND: Context analysis is a key element in the implementation of evidence-based practice. It contributes to understanding why certain interventions succeed in some settings and fail in others, while also supporting the tailoring of implementation strategies to different populations and settings. This task becomes especially challenging when implementing complex interventions that involve multidisciplinary teams operating across multiple organisations. Current literature highlights the need for innovative methodological approaches capable of capturing complexity across settings and examin

Early Detection of Chronic Kidney Disease in Mexico, a Case for Policy Transfer.

INTRODUCTION: Chronic kidney disease (CKD) treatment is often delayed in many countries, creating challenges for the health system. Therefore, it is necessary to strengthen its detection and management from the early stages. Local experiences that have been successful in managing CKD could transfer their policies and thus systematically expand the scope of their organizational learning. PURPOSE: To characterize the elements of a success story of early detection and care of CKD in an adult population with social security, in order to transfer it to a similar organization. PATIENTS AND METHODS

Response to Postoperative Acute Kidney Injury: Does Knowledge Equate to Action?

INTRODUCTION: Acute kidney injury (AKI) is a frequent postoperative complication and is associated with substantial short- and long-term morbidity. Although the adoption of standardized diagnostic criteria has improved recognition of AKI, less is known about how surgical teams manage, document, and ensure appropriate follow-up after AKI. METHODS: We conducted a retrospective cohort study of adults who developed postoperative AKI at a tertiary medical center over a 2-y period. Primary outcomes included the frequency of AKI-related management interventions, completeness of AKI documentation, an

Management of Distal Femoral Fractures in the Elderly.

Distal femoral fractures (DFF) cause significant morbidity and mortality comparable with neck of femur fractures. In view of this, NICE, National Institute of Clinical Excellence, has introduced a Best Practice Tariff (BPT) for management of distal femoral fractures in the elderly to improve morbidity and reduce regional variation of standards. Operative management includes application of external fixation, open reduction and internal fixation, or arthroplasty. While these are associated with significant morbidity, non-operative management may also be considered with immobilisation or traction

Better & systematic trauma care (BEST) in Norway: from a local quality improvement project to a national collaborative.

PURPOSE: Healthcare professionals in Norwegian local hospitals have focused on the need for improved trauma care since the late 1990s. Because of lack of national guidelines the hospital management of patients with serious trauma varied. The authors and colleague health care providers felt a need for improvement. Therefore, a project called Better & Systematic Trauma care (BEST) was established. The goal was to improve trauma care through training and preparation for infrequently occurring situations, with a focus on the team. METHODS: The project began as a bottom-up initiative in four local

Outcomes and Management of Peripheral Intravenous Infiltration Injuries.

Background: Although intravenous (IV) infiltration is relatively common, data regarding complications and outcomes of this problem remain limited. In addition, there is wide variation in institutional protocols for the management of IV infiltrations. Through retrospective review, we aim to delineate complications and outcomes, and propose an algorithm for the management of these injuries. Methods: We performed a retrospective review of all patients who had an IV infiltration at a tertiary care center's inpatient and outpatient facilities between January 1, 2016, and December 31, 2018. Results:

The effectiveness of a multidisciplinary, team-based approach to cesarean hysterectomy in modern obstetric practice.

OBJECTIVE: To examine the effectiveness of a multidisciplinary, team-based approach to management of cesarean hysterectomy. METHODS: In a retrospective chart review, data were analyzed from a quality assurance database of hysterectomies performed after cesarean delivery at one institution in the USA. Patients were identified through billing codes for cesarean delivery, cross-referenced to codes for hysterectomy. Demographic, reproductive, and outcome data were compared before (2000-2005) and after (2011-2013) implementation of a multidisciplinary team-based protocol. RESULTS: Across the two

Supportive Management of Patients with Advanced Pheochromocytomas and Paragangliomas Receiving PRRT.

Peptide receptor radionuclide therapy (PRRT) is used to treat patients with advanced malignant pheochromocytomas (PCCs) and paragangliomas (PGLs). Patients are at risk of a PRRT-induced catecholamine crisis, and standard guidelines regarding the prevention and management of infusion reactions are lacking. In this case series, the institutional experience of five sequential patients with metastatic PCCs and PGLs receiving PRRT on an outpatient basis is described, of which four had symptomatic tumors and three had a high burden of disease. All patients with symptomatic tumors were treated with p

Anesthetic Approach to a Case of Hepatoblastoma With Right Atrial Spread for Simultaneous Resection of Both.

Hepatoblastoma is the most common primary liver tumor in childhood. However, cases involving age >5 years are extremely rare. Invasion of the inferior vena cava and right atrium in hepatoblastoma places the patient in a high-risk group, and due to the rarity of such presentation, the preferred surgical approach is not clear. We present the perioperative anesthetic management of hepatoblastoma in an eight-year-old child with right atrial invasion at diagnosis, with no regression in size of the tumor after chemotherapy, treated subsequently with combined cardiac and liver surgery. Due to the pos

The management of juvenile nasopharyngeal angiofibroma (JNA) has changed during the last decades but it still continues to be a challenge for the multidisciplinary head and neck surgical team. The aim of this study was to review the used treatment approach and outcome of JNA in a single institution series of 27 patients diagnosed and treated during the years 1970-2009. All patients were male, with the median age of 17 years (range 11-33 years). Surgery was used as the primary treatment in every case. Surgical approaches varied, transpalatal approach (N = 14) being the most common approach used

The treatment of obesity in type 2 diabetes mellitus.

Obesity has a critical role in the pathophysiology of type 2 diabetes mellitus, and prevention of weight gain and treatment after onset of obesity is crucial to the management of the disease. A recent National Institutes of Health (NIH) report on the evaluation and treatment of overweight and obesity serves as a model for managing obese individuals with type 2 diabetes. Lifestyle intervention is the fundamental approach and should be implemented by a multidisciplinary team of health professionals. Adjunctive therapies, such as anti-obesity pharmacotherapy and surgery, should only be considered
