Barriers to access and unmet needs to neuromyelitis optica spectrum disorders care in an Argentinean cohort.

INTRODUCTION: Neuromyelitis optica spectrum disorder (NMOSD) is a rare but severe neuroimmunological condition associated with a significant financial burden. NMOSD is also associated with increased health care utilization, including neurology outpatient visits, magnetic resonance imaging (MRI) use, long-term medication, among others. We aimed to evaluate real-world patient experiences in access to care and NMOSD burden in an Argentinean cohort. METHODS: This cross-sectional study used a self-administered survey and was conducted in Argentina (2022). Patients with NMOSD were divided into thre

Structural Drivers of Race and Sex Disparities in Adolescent Idiopathic Scoliosis Care: A Narrative Review.

PURPOSE: Adolescent idiopathic scoliosis (AIS) is the most common spinal deformity of adolescence and disproportionately affects females, with outcomes strongly dependent on curve magnitude and timing of detection. Increasing evidence demonstrates that structural and systemic factors influence multiple stages of the AIS care continuum and may be the primary drivers of disparities in care amongst this population. This narrative review synthesizes contemporary evidence on sex- and race-based inequities in AIS, emphasizing structural drivers rather than biologic explanations for these disparities

An Examination of Follow-up Services Received by Vulnerable Burn Populations: A Burn Model System National Database Study.

While disparities in healthcare outcomes and services for vulnerable populations have been documented, the extent to which vulnerable burn populations demonstrate disparities in long-term care is relatively underexplored. This study's goal was to assess for differences in long-term occupational or physical therapy (OT/PT) and psychological service use after burn injury in vulnerable populations. Data from the Burn Model System National Database (2006-2015) were analyzed. The vulnerable group included participants in one or more of these categories: 65 years of age or older, nonwhite, no insura

[Is family history dispensable?].

As an element of risk assessment in applications for life insurance, family medical history has a particular significance since given impairments can occur more frequently within families. Family history is not only genetic in nature. Depending on the impairment, it is also explained by external factors. There has been little literature on this topic so far, although the spectrum of family history-related impairments is very large, and their effect is highly dependent on the type of product. This paper presents a new method for assessing the effects of information contained in family history o

Integrating HIV/AIDS services into financial protection systems to increase sustainability of the HIV/AIDS response.

Begun in 2014, the Sustainable Financing Initiative (SFI) was dedicated to mobilizing domestic resources for the HIV response. Among its three programmatic pillars was a focus on ensuring financial protection for people living with HIV (PLHIV). SFI's activities were founded on a strong understanding of and alignment with partner government priorities, as well as costing and actuarial analyses, which allowed for the development of context-specific approaches for improving financial protection for PLHIV. SFI implemented financial protection activities in a total of nine countries; the five count

Insurance Status Predicts Hand Therapy Adherence following Flexor Tendon Repair: A Retrospective Cohort Study.

BACKGROUND: Many patients are not adherent to hand therapy rehabilitation following surgical repair of traumatic flexor tendon injuries, which can negatively affect surgical outcomes and long-term hand function. The authors aimed to identify the factors that predict patient nonadherence to hand therapy following flexor tendon repair surgery. METHODS: This retrospective cohort study included 154 patients who underwent surgical repair of flexor tendon injuries at a level I trauma center between January of 2015 and January of 2020. A manual chart review was performed to collect demographic data,

Ayushman card (PM-JAY) as a tool for financial risk protection in orthopaedic trauma care: Evidence from a Level-1 trauma centre.

BACKGROUND: Orthopaedic trauma care is highly resource-intensive and often requires urgent surgical intervention, costly implants, and prolonged hospitalisation. In low- and middle-income settings, lack of financial protection can delay or prevent access to definitive care. Publicly funded insurance schemes such as Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (PM-JAY) aim to improve access and reduce out-of-pocket expenditure. However, real-world evidence on utilization of these schemes in trauma surgery remains limited. METHODS: A retrospective observational study was conducted at a tert

A Machine Learning Approach for Predicting Long-term Care Needs and Identifying Risk Factors Among Older Adults in Japan.

A machine learning model using Extreme Gradient Boosting (XGBoost) was developed to predict long-term nursing care needs among older adults, based on comprehensive claims and health checkup data from Japan's public insurance system. The model demonstrated strong predictive performance (AUC: 0.878; sensitivity: 0.784; specificity: 0.820) on the test dataset, supporting its use for early identification of high-risk individuals. Key risk factors identified through permutation importance and marginal effect analyses included advanced age, prior care needs, neurological and gastrointestinal disease

Cost, coverage, and comparative effectiveness research: the critical issues for oncology.

A new national initiative in comparative effectiveness research (CER) is part of a broad and long-term evolution toward greater reliance on scientific evidence in clinical practice and medical policy. But CER has been controversial because of its high profile in the health care reform effort, its instantiation in a prominent new national research institute, and lingering concerns that the ultimate goal of CER is to empower the government and private insurers to reduce health care costs by restricting access to expensive new medical tests and treatments. This article presents an analysis of the

Implementation of Point of Care Sexually Transmitted Infections Testing in a Community Clinic Setting.

The rates of sexually transmitted infections (STIs) in the United States, including chlamydia and gonorrhea, are rising. Point-of-care (POC) testing could increase access to testing and treatment. This evaluation found POC STI testing to be concordant with the results of traditional laboratory testing for 100% of patients who were tested. Ninety-five percent of the patients reported being satisfied with the experience, and 66% preferred it to traditional laboratory testing. The most commonly reported reason for preferring the test was the short amount of time it took to receive results. Howeve

Impact of Health Insurance Expansions on Nonelderly Adults With Hypertension.

INTRODUCTION: Hypertension is a risk factor for cardiovascular disease (CVD), the leading cause of death in the United States. The treatment and control of hypertension is inadequate, especially among patients without health insurance coverage. The Affordable Care Act offered an opportunity to improve hypertension management by increasing the number of people covered by insurance. This study predicts the long-term effects of improved hypertension treatment rates due to insurance expansions on the prevalence and mortality rates of CVD of nonelderly Americans with hypertension. METHODS: We deve

Deep learning model for the prediction of all-cause mortality among long term care people in China: a prospective cohort study.

This study aimed to develop a deep learning model to predict the risk stratification of all-cause death for older people with disability, providing guidance for long-term care plans. Based on the government-led long-term care insurance program in a pilot city of China from 2017 and followed up to 2021, the study included 42,353 disabled adults aged over 65, with 25,071 assigned to the training set and 17,282 to the validation set. The administrative data (including baseline characteristics, underlying medical conditions, and all-cause mortality) were collected to develop a deep learning model

Fragility fracture site is associated with frailty in older adults: a population-based cohort study in Japan.

BACKGROUND: Fragility fractures cause disability in older adults. Although frailty determines adverse outcomes, the relationship between anatomical fracture site and functional status remains unclear. Clarifying these associations may improve geriatric assessment and secondary prevention strategies. METHODS: We conducted a population-based retrospective cohort study using the Shizuoka Kokuho Database, which integrates medical and long-term care insurance claims in Japan. Individuals aged ≥65 years with a first-time, single-site fragility fracture (proximal humerus, distal radius, hip, distal

Gender disparities in German home-care arrangements.

An ageing population correlates with rising needs for long-term care (LTC). Support programmes should consider the specific needs of the various subgroups of care dependents and family caregivers. The objective of this study was to analyse the gender-specific disparities in home-care arrangements in Germany, and for this purpose, survey and insurance claims data were used. A survey of 2545 insured care recipients with high-level care needs was conducted in 2012 with the Barmer GEK, a major German statutory healthcare insurance. Insurance claims data were provided for a follow-up, focussing on
