Association of Family Medicine Milestone Ratings With Early-Career Patient Retention: A US National Insurance Claims Study.

BACKGROUND: US family medicine (FM) residency training emphasizes the development of patient-physician relationships, which are associated with patient satisfaction, improved health outcomes, cost containment, and retention. It is important to examine whether this emphasis translates into observable performance during physicians' early years of independent practice. OBJECTIVE: To assess the association between residency Milestone ratings related to patient-physician relationships and patient retention in early clinical practice. METHODS: We analyzed data from 2375 FM residents across 435 pro

Illustrating Poststratification Methods in Medical Claims Data: A Korean Example.

BACKGROUND: Observational studies often rely on nonprobability samples that may not represent the target population, limiting the generalizability of findings. Health examination data from the Korean National Health Insurance Service (NHIS) faces this challenge, as voluntary participation introduces sampling bias. Poststratification offers a potential solution by reweighting samples to match population distributions. METHODS: Using the NHIS-National Sample Cohort, we demonstrated three poststratification approaches-simple poststratification, raking, and multilevel regression with poststratifi

Neonatal Outcome Ascertainment in Mother-Infant Paired Claims.

PURPOSE: Claims data are a valuable source to study neonatal outcomes across a wide range of clinical questions. Infants' delayed enrollment in infant insurance poses challenges in capture of neonatal outcomes, which may be charged to the maternal health plan, posing misclassification risks. We evaluated outcome ascertainment across three infant enrollment scenarios. METHODS: We used Merative MarketScan databases (2012-2018) in the United States to construct a mother-infant linked cohort and assess the outcome ascertaiment precision with varying infant enrollment requirement. RESULT: We foun

Burden of glucocorticoid use in commercially insured adults with generalized myasthenia gravis in the United States: A retrospective claims-based analysis.

Generalized myasthenia gravis (gMG) is a chronic autoimmune condition characterized by muscle weakness. If frontline treatments inadequately manage symptoms, patients may require use of glucocorticoids (GC); however, long-term use of GCs is associated with toxicities. Few studies have examined the incidence of GC-related toxicities and economic burden among patients with gMG. This study aims to describe GC toxicities, healthcare resource utilization, and costs among US adults with gMG, stratified by level of GC use. Adults with gMG were identified using IQVIA PharMetrics® Plus data and classif

Noninvasive tests and diagnostic pathways to MASH diagnosis in the United States: a retrospective observational study.

AIM: Although liver biopsy is considered the most reliable diagnostic tool for metabolic dysfunction-associated steatohepatitis (MASH), it is invasive and can be costly. Clinicians are increasingly relying on routine biomarkers and other noninvasive tests (NITs) for diagnosis. We examined real-world diagnostic pathways for patients newly diagnosed with MASH with a primary focus on NITs. MATERIALS AND METHODS: This retrospective, observational study analyzed healthcare claims data (Merative MarketScan Commercial and Medicare Databases) from patients in the United States newly diagnosed with MA

Impact Of Medicare Readmissions Penalties On Targeted Surgical Conditions.

The Hospital Readmissions Reduction Program, announced in 2010 to penalize excess readmissions for patients with selected medical diagnoses, was expanded in 2013 to include targeted surgical diagnoses, beginning with hip and knee replacements. Whether these surgical penalties reduced procedure-specific readmissions is not well understood. Using Medicare claims, we evaluated the penalty announcements' effects on risk-adjusted readmission rates, episode payments, lengths-of-stay, and observation status use. Risk-adjusted readmission rates declined for both procedures from 7.6 percent in 2008 to

Screening for Latent Tuberculosis Infection Among HIV-Infected Medicaid Enrollees.

OBJECTIVES: In the United States, universal screening for latent tuberculosis (TB) infection among people with HIV is recommended, but the percentage receiving screening is unknown. This study assessed screening for latent TB infection among people with HIV enrolled in Medicaid during 2006-2010. METHODS: We used nationwide fee-for-service Medicaid records to identify people with HIV, measure screening for latent TB infection, and examine associated demographic, social, and clinical factors. We used logistic regression analysis to calculate odds ratios (ORs) and 95% confidence intervals (CIs).

External validation of a claims-based algorithm for classifying kidney-cancer surgeries.

BACKGROUND: Unlike other malignancies, there is no literature supporting the accuracy of medical claims data for identifying surgical treatments among patients with kidney cancer. We sought to validate externally a previously published Medicare-claims-based algorithm for classifying surgical treatments among patients with early-stage kidney cancer. To achieve this aim, we compared procedure assignments based on Medicare claims with the type of surgery specified in SEER registry data and clinical operative reports. METHODS: Using linked SEER-Medicare data, we calculated the agreement between M

Claim Denials: Low-Income Patients From Disadvantaged Racial And Ethnic Groups Experienced The Largest Burdens.

Insurance claim denials are a common source of administrative burden, especially for patients with private health insurance. Contesting denied claims requires considerable investment from physicians and patients or caregivers, including both institutional knowledge of health policies and billing practices and the means to engage in reconciliation. We used a novel national data set comprising remittance data and patient demographics to describe disparities in the rates of seeking and receiving claim denial corrections across demographic and socioeconomic dimensions. We found that patients from
