An algorithm for QT interval monitoring in neonatal intensive care units.

QT surveillance of neonatal patients, and especially premature infants, may be important because of the potential for concomitant exposure to QT-prolonging medications and because of the possibility that they may have hereditary QT prolongation (long-QT syndrome), which is implicated in the pathogenesis of approximately 10% of sudden infant death syndrome. In-hospital automated continuous QT interval monitoring for neonatal and pediatric patients may be beneficial but is difficult because of high heart rates; inverted, biphasic, or low-amplitude T waves; noisy signal; and a limited number of e

Stress ulcer prophylaxis in hospitalized patients not in intensive care units.

PURPOSE: A review is presented of the evidence behind the current use of therapies for the prevention of stress-related mucosal disease and bleeding in the nonintensive care unit (ICU), general medicine population. SUMMARY: The use of proton pump inhibitors and histamine H2-receptor antagonists for the prevention of stress ulcers has been well-defined in critical care patients. In 1999, the American Society of Health-System Pharmacists (ASHP) published guidelines on the use of stress ulcer prophylaxis in medical, surgical, respiratory, and pediatric ICU patients. In recent years, the practice

Candiduria in intensive care units: association with heavy colonization and candidaemia.

Candiduria is increasingly detected in intensive care unit (ICU) patients and often coexists with candidal colonization at other anatomical sites. Studies involving surgical and medical ICU patients have consistently reported a relationship between candiduria and heavy colonization. This suggests that candiduria could be considered as a marker for heavy colonization. Risk factors that predispose to heavy colonization are generally similar to those predisposing to candidaemia. Candiduria in ICU patients is characterized by a high mortality, largely through a significant relationship with candid

Risk factors for mortality of nosocomial bacteraemia in intensive care units.

OBJECTIVE: The aim of this study was to follow critically ill patients prospectively in intensive care units (ICUs) to determine risk factors for mortality and outcome associated with nosocomial bacteraemia (NB). SUBJECTS AND METHODS: A case-control study of 176 patients was conducted to identify the risk factors for mortality of NB in ICU patients. The study was performed in emergency, surgical and general surgical ICUs with 23 beds during a 15-month period. A total of 1,450 patients were admitted to the ICUs during the study period. The USA Center for Disease Control and Prevention definiti

Evaluating mortality in intensive care units: contribution of competing risks analyses.

INTRODUCTION: Kaplan-Meier curves and logistic models are widely used to describe and explain the variability of survival in intensive care unit (ICU) patients. The Kaplan-Meier approach considers that patients discharged alive from hospital are 'non-informatively' censored (for instance, representative of all other individuals who have survived to that time but are still in hospital); this is probably wrong. Logistic models are adapted to this so-called 'competing risks' setting but fail to take into account censoring and differences in exposure time. To address these issues, we exemplified t

Outbreaks of influenza A virus infection in neonatal intensive care units.

BACKGROUND: Outbreaks of nosocomial influenza virus infections have been described rarely during childhood and even less so in the neonatal period. METHODS: We report 30 neonates admitted to 2 neonatal intensive care units with nosocomial influenza A virus infection, which occurred in 2 outbreaks during 1999. Risk factors for infection were evaluated, and control measures were adopted. Virus was detected by indirect immunofluorescence antibody screen. Any infant with nasopharyngeal aspirate positive for influenza A virus was considered infected. RESULTS: Of 95 infants screened 30 were positi

Epidemic bacteremia due to Acinetobacter baumannii in five intensive care units.

From March 5, 1986 to September 4, 1987, Acinetobacter baumannii (AB) was isolated from blood or vascular catheter-tip cultures of 75 patients in five intensive care units at a hospital in New Jersey. To identify risk factors for AB bacteremia in the intensive care units, a case-control study was conducted. Characteristics of 72 case-patients were compared with those of 37 controls. Case-patients were more likely than controls to have had peripheral arterial catheters (odds ratio (OR) = 7.0, p less than 0.001), mechanical ventilation (OR = 5.8, p less than 0.001), hyperalimentation (OR = 5.7,

Intensive care units as a source of methicillin-resistant Staphylococcus aureus.

Over a 12 month period, 209 isolates of methicillin resistant Staphylococcus aureus (MRSA) were obtained in 39 patients admitted to an ICU. In 23 patients MRSA was the major pathogen, producing either pneumonia, bacteremia or wound infection. In eight patients death was directly related to the MRSA infection. This study suggests an increasing occurrence of MRSA infections in ICU and the need to adopt control measures.

[Bronchial asthma: clinico-pathological study of fatalities in intensive care units].

Due to the great amount of asthmatic patients presenting complications which could endanger their lives, a revision of the possible threatening factors is carried out. The factors concurring with the base pathologic mechanism are analyzed and it is demonstrated, that some of these factors intervene independently from bronchial asthma. The study is carried out on the basis of age, sex, hospital stay duration, clinical picture, blood gases analysis, complications and pathology findings: 1) Most of the deceased patients presented various pathologies besides bronchial asthma which contributed to t

Key characteristics of palliative care integration in intensive care units (ICUs): A scoping review.

BACKGROUND: Intensive care units (ICUs) are increasingly managing patients with greater clinical complexity and life-limiting conditions. Systematic review evidence highlights that early palliative care, integrated alongside active treatment and not solely at end-of-life, can significantly improve outcomes for both patients and their families. Despite evidence on the effectiveness of palliative care integration models in the ICU setting, the specific characteristics for integration remain poorly articulated and insufficiently understood. OBJECTIVES: To systematically map key characteristics o

Identifying essential medications for critical care: initial findings from US academic intensive care units.

BACKGROUND: Low- and middle-income countries (LMICs) shoulder a disproportionately high amount of death and disability caused by both communicable and non-communicable diseases. As a result, there is a higher occurrence of serious illness in these countries. Intensive Care Units (ICUs) have emerged in the past twenty years in LMICs. However, the World Health Organization (WHO) Model List of Essential Medicines lacks many common medicines used in critical care, and the lack of necessary medications to treat critical illnesses has hindered the function of these ICUs. This study aimed to develop

Breaking down barriers: enabling care-by-parent in neonatal intensive care units in China.

BACKGROUND: Denying parents access to their infant in the Neonatal Intensive Care Unit (NICU) is a standard practice in most hospitals across China. Visitation is not usually permitted or may be strictly limited, and NICU care for most neonates is provided by health-care professionals with little participation of the parents. An exception to this rule is the level 2 "Room-In" ward in Qilu Children's Hospital, Shandong University, where parents have 24-hour access to their infants and participate in providing care. METHODS: This retrospective cohort study compared the outcomes of infants who w

Intensive Care Infection Score (ICIS) is an Early Marker for Infection in Time of Admission to Intensive Care Units.

BACKGROUND: Despite all the effort, infections remain one of the major causes of morbidity and mortality in clinically ill patients, and novel diagnostic markers detecting infections in early stages are searched for. Intensive Care Infection Score (ICIS) was developed as such a marker. METHODS: A total of 102 patients admitted to intensive care units (ICU) in the University Hospital Hradec Kralove, Czech Republic were enrolled in this study. ICIS along with relevant biochemical markers (procalcitonin, C-reactive protein and Interleukin 6) was analyzed on the day of the admission. Individual p

Epidemiology and Outcomes of Patients Readmitted to the Intensive Care Unit After Cardiac Intensive Care Unit Admission.

Readmission to the intensive care unit (ICU) during the index hospitalization is associated with poor outcomes in medical or surgical ICU survivors. Little is known about critically ill patients with acute cardiovascular conditions cared for in a cardiac intensive care unit (CICU). We sought to describe the incidence, risk factors, and outcomes of all ICU readmissions in patients who survived to CICU discharge. We retrospectively reviewed Mayo Clinic patients from 2007 to 2015 who survived the index CICU admission and identified patients with a second ICU stay during their index hospitalizatio
