Intra-atrial block after COVID-19 vaccination-induced fulminant myocarditis: a case report.

BACKGROUND: Myocarditis is associated with an increased risk of conduction disturbances during the acute phase, which recovers in most cases but rarely recurs during the chronic phase. CASE SUMMARY: A 50-year-old man who developed fulminant myocarditis after COVID-19 mRNA vaccination was discharged 24 days after admission. He was readmitted for heart failure associated with two P waves: one P wave (P1) had a normal amplitude and was dissociated from the QRS, and the other (P2) had a very low amplitude and was associated with the QRS. The patient was referred for pacemaker implantation. Before

Electrophysiologic evidence of epicardial connections between low right atrium and remote right atrial region or coronary sinus musculature: Relevance for catheter ablation of typical atrial flutter.

BACKGROUND: The coronary sinus (CS) is surrounded by a myocardial coat with extensive connections to the left and right atria that contributes to the interatrial electrical connection. Whereas epicardial connections between CS musculature and the left atrium have largely been demonstrated, clinically relevant epicardial connections from the CS musculature toward the low right atrium (LRA) and epicardial connections between two regions of the right atrium remain questionable. METHODS: Five patients underwent electrophysiology (EP) study for typical atrial flutter (AFl) using either conventiona

The Influence of Interatrial Conduction Disorders on Atrial Mechanical Function - Atrial Strain and Pulmonary Veins Reversal Flow in Patients with COVID-19.

BACKGROUND: The physiological activation of the left atrium (LA) happens through the Bachmann bundle, which is crucial for the heart's proper functioning. Bayes de Luna first described interatrial blocks (IABs) in 1979, noting their disruption of atrioventricular (AV) synchrony. This study aims to evaluate LA mechanics by analyzing LA strain in cases of normal and impaired interatrial conduction, focusing on retrograde flow in the pulmonary veins (PV). METHODS: The study included 51 patients who tested positive for SARS-CoV-2 and exhibited related symptoms. Six patients with persistent atrial

Intracardiac echocardiography guided anatomical approach to cardioneuroablation: feasibility and outcomes.

AIMS: Anatomical studies have documented a close topographical relationship between the ganglionated plexi (GP) containing parasympathetic inputs to the sinus node (SN) and atrioventricular node (AVN) and the epicardial fat pads (FPs) within the Waterston's interatrial groove. We aimed to investigate the feasibility and outcomes of a novel anatomical approach to cardioneuroablation (CNA) that targets the atrial areas adjacent to the interatrial FPs identified with intracardiac echocardiography (ICE). METHODS AND RESULTS: About 17 patients [37.3 ± 10.2 years, 47% female] undergoing CNA for rec

Cystic tumor of the atrioventricular node: an unexpected finding in an explanted heart.

SUMMARY: We report herein a unique case of cystic tumor of atrioventricular (AV) node (CTAVN), which, to our knowledge, is the first of its kind diagnosed in an explanted heart specimen and only the fourth diagnosed antemortem. Often, this rare tumor can only be diagnosed by careful gross examination and adequate sampling of AV node region. It is an important differential diagnosis in young patients with syncopal attacks and varying degrees of heart blocks. CONTEXT: CTAVN is a rare, benign tumor. Most cases have been reported in young females (mean age, 38 years). Patients typically present w

Contrasting anatomical challenges in the implantation of tine-fixed and helix-fixed leadless pacemakers in a single patient: A case report.

UNLABELLED: Patient-specific anatomical variations can significantly influence the implantation technique of different types of leadless pacemakers (LPMs). A 74-year-old man with complete atrioventricular block initially underwent successful implantation of a tine-fixed LPM. Three years later, an elevated pacing threshold caused premature battery depletion, and a helix-fixed LPM was selected for reimplantation. Unlike the tine-fixed LPM implantation procedure, this implantation proved technically challenging. The catheter developed a sigmoid deformation while crossing the tricuspid valve, whic

P-Wave Beat-to-Beat Analysis to Predict Atrial Fibrillation Recurrence after Catheter Ablation.

The identification of patients prone to atrial fibrillation (AF) relapse after catheter ablation is essential for better patient selection and risk stratification. The current prospective cohort study aims to validate a novel P-wave index based on beat-to-beat (B2B) P-wave morphological and wavelet analysis designed to detect patients with low burden AF as a predictor of AF recurrence within a year after successful catheter ablation. From a total of 138 consecutive patients scheduled for AF ablation, 12-lead ECG and 10 min vectorcardiogram (VCG) recordings were obtained. Univariate analysis re

Thrombosed sinus of Valsalva aneurysm masquer ading as a cardiac tumour: a case report.

BACKGROUND: An aortic sinus of Valsalva aneurysm (SVA) often remains undiagnosed until it ruptures. An SVA filled with thrombus can be challenging to diagnose accurately. CASE SUMMARY: A 70-year-old man was admitted with a clinical diagnosis of well-tolerated complete atrioventricular block (AVB). Transthoracic echocardiography revealed a spherical mass (43 × 49 mm) at the interatrial septum. Enhanced computed tomography (CT) showed a well-defined, hollow, and non-enhanced mass suggesting a cardiac tumour. However, 18F-fluorodeoxyglucose positron emission tomography/CT (18F-FDG PET/CT) showed

Extended trans-septal versus left atrial approach in mitral valve surgery: 1017 patients' experience.

OBJECTIVE: The mitral valve may be accessed directly through the left atrium but visualisation can sometimes be challenging. A trans-septal interatrial approach provides better exposure and easy access for concomitant tricuspid procedures especially in difficult cases. This retrospective study evaluates the safety and effectiveness of the extended vertical trans-septal approach (EVTSA) for routine mitral valve exposure. METHOD: 1017 consecutive patients undergoing an isolated primary mitral valve procedure (repair/replacement) through a median sternotomy were retrospectively studied between 2

Atrial Conduction Disorders.

Atrial conduction disorders result from impaired propagation of cardiac impulses from the sinoatrial node through the atrial conduction pathways. Disorders affecting interatrial conduction alter P-wave characteristics on the surface electrocardiogram. A variety of P-wave indices reflecting derangements in atrial conduction have been described and have been associated with an increased risk of atrial fibrillation (AF) and stroke. Interatrial block (IAB) is the most well-known of the different P-wave indices and is important clinically due to its ability to predict patients who are at risk of th

Multiscale Fine-Grained Heart Rate Variability Analysis for Recognizing the Severity of Hypertension.

Hypertension is a common and chronic disease and causes severe damage to patients' health. Blood pressure of a human being is controlled by the autonomic nervous system. Heart rate variability (HRV) is an impact of the autonomic nervous system and an indicator of the balance of the cardiac sympathetic nerve and vagus nerve. HRV is a good method to recognize the severity of hypertension due to the specificity for prediction. In this paper, we proposed a novel fine-grained HRV analysis method to enhance the precision of recognition. In order to analyze the HRV of the patient, we segment the over

Fetal cardiac interventions: clinical and experimental research.

Fetal cardiac interventions for congenital heart diseases may alleviate heart dysfunction, prevent them evolving into hypoplastic left heart syndrome, achieve biventricular outcome and improve fetal survival. Candidates for clinical fetal cardiac interventions are now restricted to cases of critical aortic valve stenosis with evolving hypoplastic left heart syndrome, pulmonary atresia with an intact ventricular septum and evolving hypoplastic right heart syndrome, and hypoplastic left heart syndrome with an intact or highly restrictive atrial septum as well as fetal heart block. The therapeuti

[Cryosurgical ablation of the atrioventricular conduction system--experimental study for the determination of the appropriate ablation site and the long term follow-up results with clinical application].

In a series of experimental studies in dogs, we determined the exact site for the disruption of the atrioventricular conduction system by cryosurgical ablation which does not disturb the intraventricular conduction. The long-term symptomatic effects were evaluated in 9 patients with supraventricular tachycardia. In a total of 15 adult mongrel dogs, a thoracotomy was performed under pentobarbital anesthesia that was maintained during the procedure with artificial ventilation and cardiopulmonary bypass. The body temperature was maintained at 37 degrees C and throughout each experiment, the ECG w

Correction of total anomalous pulmonary venous connection of the cardiac type.

Surgical treatment of the cardiac type of total anomalous pulmonary venous connection requires special techniques. The treatment and outcome in 17 consecutive patients who had undergone primary repairs of the cardiac type between 1965 and 1996 were reviewed. The median age was 3 months and median weight 4.2 kg. The connection was the coronary sinus in ten patients, and the right atrium in six. Interatrial communication was routinely augmented. The right atrial cavity was partitioned using a patch to direct the anomalous pulmonary veins into the left atrium through the atrial septal defect in t

Misplacement of pacemaker leads: How to avoid and how to approach?

The use of cardiac implantable electronic devices has increased in recent years due to factors such as a growing population, increased life expectancy, and improved access to healthcare services. In parallel with this trend, complications related to pacemakers have also increased. One of the most important and preventable complications among these is lead misplacement. Misplacement of a permanent pacemaker lead in the LV can lead to a variety of complications, including damage to the heart muscle, arrhythmias, blood clotes and cerebrovascular events. It is recommended to use the fluoroscopic 4

[Cardiac and breast diffuse large B-cell lymphoma with pericardial effusion and AV-block].

Primary cardiac lymphoma is extremely rare and is associated with a poor prognosis. In most cases, cardiac involvement occurs as a late symptom and the diagnosis is thus delayed. We herein report a 35-year-old woman with cardiac diffuse large B-cell lymphoma (DLBCL) with breast infiltration. The patient was admitted to our hospital based on an initial presentation with dyspnea on exertion, chest pain, and a hard mass of the left breast. Echocardiography revealed a mass in the right atrium wall and interatrial septum, and massive pericardial effusion. ECG showed atrioventoricular block. We prom

Surgical treatment of cardiac hydatid disease in 13 patients.

Cardiac hydatidosis is an uncommon but potentially fatal disease. It remains endemic in developed and developing countries. Its clinical presentation ranges from an absence of symptoms to congestive heart failure or other life-threatening sequelae. Herein, we report our clinical experience with the disease. From 1991 through 2009, 7 male and 6 female patients (mean age, 36 +/- 18.3 yr; age range, 10-68 yr) underwent surgical treatment at our hospital for cardiac hydatid disease. Cardiac hydatidosis was established as a component of multiorgan echinococcosis in 8 patients, and it was diagnosed

The use of echocardiographic colour kinetic wall motion to differentiate broad complex tachycardia.

Discrimination between supraventricular tachycardia (SVT) with aberrant conduction from ventricular tachycardia (VT) is vital for the safe and effective management of both conditions. Electrocardiographic algorithms for the differentiation of broad complex tachycardia are complex and difficult to implement in the acute setting, with misdiagnosis occurring in up to 40% of acute presentations. This case study shows the potential for echocardiographic colour kinesis (eck) to support electrocardiographic differentiation.
