Video-assisted thoracic surgery (VATS) is currently used for the repair of pectus excavatum. Analgesia after thoracic surgery can be provided with nerve blocks, intravenous drugs, or neuraxial techniques. Serratus posterior superior intercostal plane block (SPSIPB) is a novel interfascial plane block and it is performed between the serratus posterior superior muscle and the intercostal muscles at the level of the second and third ribs. In this case, we present our successful analgesic experience with SPSIPB in a patient who underwent minimally invasive pectus excavatum repair with a VATS techn

Background: Respiratory muscle electromyography (EMG) can identify whether a muscle is activated, its activation amplitude, and timing. Most studies have focused on the activation amplitude, while differences in timing and duration of activity have been less investigated. Detection of the timing of respiratory muscle activity is typically based on the visual inspection of the EMG signal. This method is time-consuming and prone to subjective interpretation. Aims: Our main objective was to develop and validate a method to assess the respective timing of different respiratory muscle activity in a

Anatomical study of the costoclavicular space, with approach via inferolateral axillary route, allowing resection of the first rib. With the patient in the semisupine position, lifting the arm to the zenith allows the costoclavicular space to be opened widely and separation of the plane of the first rib from the neurovascular structures of the lateral wall of the axillary pyramid. In the first part of our work, we study successively the bony frame and its anatomical variations, the muscular frame and its anomalies, the cervical pleura and its associated fibromembranous complex. The second part

It is shown that the electrophysiological diagnosis of brachial plexus injury is becoming more precise than ever. The intraoperative use of electrodiagnosis especially aids the obtaining of better results for brachial plexus injuries. When nerve transfer is undertaken, biceps function is more usefully restored by the accessory nerve than by the intercostal nerves. Protective sensation was achieved in almost all cases by intercostal nerve transfer. When nerve grafting was undertaken, functional motor restoration was regained in the upper arm muscles in one case, but not in the forearm.

Forces involved in breathing-which effectively pull in air-are the diaphragmatic, intercostal, spine, and neck muscles. Equally important is the bulbar musculature maintaining the architecture of a patent airway conduit and abdominal wall and internal intercostal muscles providing cough. Acute injury along a neural trajectory from brainstem to muscle will impair the coordinated interaction between these muscle groups. Acutely failing respiratory mechanics can be caused by central and peripheral lesions. In central lesions, the key lesion is in the nucleus ambiguus innervating the dilator muscl

Infants afflicted with meningomyelocele are paralyzed as a result of the physiological disconnection of the neural plate from the spinal cord. Intact neural elements within the placode often maintain segmental reflex innervation of the paralyzed lower extremity muscles. It was hypothesized that bridging this central-peripheral gap with vaible intercostal nerves would restore limited voluntary movement to the paralyzed muscles. This procedure has been performed on four infants and seems promising.

AIM: The present study aimed to investigate the role of Interleukin 15 (IL-15) in protein degradation in skeletal muscle mediated by the ubiquitin-proteasome pathway (UPP) in a rat model of chronic obstructive pulmonary disease (COPD).
METHODS: The COPD model was established in 30 Sprague Dawley (SD) rats through the combination of passive smoking and intratracheal injection of lipopolysaccharide (LPS). The pathological changes in lungs and bronchi of COPD rats (n=30) were compared with that of control rats (n=15). The levels of IL-15 and tumor necrosis factor-α (TNF- α) in the serum, diaphra

BACKGROUND: Intercostal neuralgia due to surgical injury of the intercostal nerve is difficult to treat. No treatment modality has given effective pain relief. Experience with other painful neuromas has demonstrated that neuroma resection and muscle implantation has been effective in the upper and lower extremities. This approach was applied to patients with intercostal neuralgia.
METHODS: A retrospective review was done of 5 consecutive patients who have had neurectomy of one or more intercostal nerves. Preoperative and postoperative pain levels, patient demographics, length of follow-up, an

Eighteen adult mongrel dogs underwent unilateral surgical disruption of the brachial plexus. Twelve animals (Group I) had as a second-stage procedure transfer of T4 and T5 intercostal nerves and their accompanying vascular bundles to the distal musculocutaneous nerve stump. Six animals (Group II) had restoration of musculocutaneous nerve continuity with conventional interpositional sural nerve cable grafts. Group I animals demonstrated significantly better electromyographic evidence of reinnervation and, comparing appearance and weight of operated and unoperated biceps muscles at the time of a

Transdiaphragmatic intercostal hernias represent rare injuries resulting most commonly from blunt trauma. When extending into the intercostal and diaphragmatic regions, these hernias pose significant diagnostic and reconstructive challenges. We report the case of a patient with a right thoracoabdominal mass that was identified on a CT scan 4 months after blunt thoracic trauma. Imaging revealed a large right posterior transdiaphragmatic hernia with intercostal extension between the 8th and 9th ribs, avulsion of the internal oblique and transversus abdominis muscles, and costal cartilage disinse

Restoration of finger flexion and extension as well as elbow flexion and extension with a double free-muscle and multiple nerve transfers following complete avulsion of the brachial plexus (nerve roots C5 to T1) is reported. The procedure combines (1) free-muscle transfer with reinnervation by the spinal accessory nerve to achieve elbow flexion and finger extension, (2) free-muscle transfer with reinnervation by the fifth and sixth intercostal nerves to restore finger flexion, (3) third and fourth intercostal motor nerve transfer to the triceps brachi to extend and stabilize the elbow, (4) ner

Electrical stimulation of any muscle group may produce tetany. If the intercostal muscles are involved this may lead to respiratory embarrasment. A case is presented in which transcutaneous electrical nerve stimulation (TENS) treatment for angina pectoris resulted in respiratory arrest.

General botulinic intoxication in cats, accompanied by severe respiratory insufficiency, was characterized by distinct impairments in histamine and serotonin metabolism in external respiratory system. Pronounced increase in content of histamine was observed in impaired muscles and their corresponding regional centers of medulla spinalis, when botulinic intoxication had the local type with unilateral paralysis of diaphragm and intercostal muscles. Content of serotonin was increased in the tissues studied only at the later steps of local botulinic intoxication.

Late-onset Pompe's disease, a generalized lysosomal glycogen storage disease caused by acid maltase deficiency, usually presents as a slowly progressive muscular weakness of proximal muscles in lower limbs, followed by involvement of respiratory muscles. In the case presented here, however, respiratory failure was the first and selective symptom which caused the uncommon appearance of a patient entering our outpatient clinic on foot carrying his own artificial respirator. Intercostal muscle biopsy eventually led to the diagnosis.

Postoperative spinal wound dehiscence is a significant complication following the posterior midline approach. It is postulated that this approach disrupts the vasculature supplying the paraspinal muscles and overlying skin. Although the spinal vasculature has been investigated previously, the smaller arterioles have not been described in the context of the posterior midline approach. Eight cadaveric neck and posterior torso specimens were dissected after injection with a radio-opaque lead oxide mixture and subsequent radiographs taken were analyzed. The deep cervical, vertebral, superficial ce

Skeletal muscles are composed of fibres of different types, each type being identified by the isoform of myosin heavy chain which is expressed as slow 1, fast 2A, fast 2X, and fast 2B. Slow fibres are resistant to fatigue due to their highly oxidative metabolism whereas 2X and 2B fibres are easily fatiguable and fast 2A fibres exhibit intermediate fatigue resistance. Slow fibres and fast fibres are present in equal proportions in the adult human diaphragm while intercostal muscles contain a higher proportion of fast fibres. A small fibre size, abundance of capillaries, and a high aerobic oxida

It has been found that in cases of obstetric brachial plexopathy, injured phrenic nerve or C3/4/5 roots may sprout into the adjacent injured upper and middle trunks of the brachial plexus. This aberrant regeneration produces co-contraction of the diaphragm and proximal upper limb muscles. This phenomenon, referred to as respiratory synkinesis or "the breathing arm", may not be limited to the upper cervical roots. We present two cases, identified through electromyographic investigations, of respiratory synkinesis selectively affecting intrinsic hand muscles, and propose that upper thoracic root

Due to its long history, the study of human gross anatomy has not adequately incorporated modern embryological findings; consequently, the current understanding has often been incompatible with recent discoveries from molecular studies. Notably, the traditional epaxial and hypaxial muscle distinction, and their corresponding innervation by the dorsal and ventral rami of the spinal nerve, do not correspond to the primaxial and abaxial muscle distinction, defined by the mesodermal lineages of target tissues. To resolve the disagreement between adult anatomy and embryology, we here propose a nove
