Outcome of the Lima periurethral constrictor in children and adolescents: a European perspective.

PURPOSE: The Lima constrictor was described in 1996 as a less complex and less expensive alternative to the artificial urinary sphincter for use in cases of pediatric neuropathic sphincter incontinence. The device provides a fixed periurethral resistance which creates continence, yet allows urethral catheterization without the need to deflate the cuff. We report our multicenter experience, and continence, revision and erosion rates. MATERIALS AND METHODS: We performed a retrospective review of 14 consecutive patients who underwent insertion of the periurethral constrictor (Silimed, Rio de Jan

[Diagnosis and treatment of neurogenic bladder].

Bladder function has two phases, urine storage and urine evacuation which are based on the complex neurological controls including central as well as peripheral nervous system. Therefore, various neurological lesions can cause bladder dysfunctions such as disturbed storage or disturbed urine evacuation. Micturitional symptoms can be divided into storage symptoms and voiding symptoms. Storage symptoms include urgency, frequency of micturition and urinary incontinence, on the other hand voiding symptoms include difficulty in starting micturition, prolonged or intermittent micturition and urinary

Ileovesicostomy for adults with neurogenic bladders: complications and potential risk factors for adverse outcomes.

AIMS: Risk factors for complications following ileovesicostomy have not been well defined. This study's purpose was to examine outcomes following ileovesicostomy in adults and identify possible risk factors that may contribute to post-operative complications. METHODS: Retrospective database review identified ileovesicostomy procedures from August 1999 to September 2003. Demographic, pre-operative, and post-operative data were extracted. Statistical analysis determined whether risk factors influenced outcomes of urethral continence, re-operation, and post-operative complications. Factors inclu

Mitrofanoff for valve bladder syndrome: effect on urinary tract and renal function.

PURPOSE: Despite early diagnosis and valve ablation, progressive renal impairment develops in a significant proportion of boys born with posterior urethral valves. Bladder dysfunction is thought to have an important role in the etiology of this renal deterioration. We report the outcome of treating bladder dysfunction with clean intermittent catheterization and overnight drainage via a Mitrofanoff appendicovesicostomy with respect to upper tract imaging, urodynamic findings and renal function. MATERIALS AND METHODS: All patients were established on a clean intermittent catheterization program

Early evolution of bladder emptying after myelomeningocele closure.

OBJECTIVES: To analyze bladder emptying in newborns with spina bifida after closure of the myelomeningocele defect. Manipulation of the spinal cord and nerve roots results in temporary changes in bladder function in many settings. METHODS: We reviewed 62 consecutive newborn patients who underwent closure of a myelomeningocele defect at our institution from January 1990 to December 1997. We examined the catheterized urine volumes obtained before and after closure, radiographic and urodynamic studies, and the subsequent need for intermittent catheterization. Poor bladder emptying was defined as

Urological complications following aorto-iliac reconstructive surgery.

The incidence of urological complications after aorto-iliac graft surgery was prospectively studied in 49 patients and retrospectively in 35 other patients operated on more than five years before. Follow-up of all 49 patients was one year. In one patient a mild asymptomatic hydronephrosis was discovered after four months. Two patients developed urinary tract infections and two other patients a urethral stricture. Late hydronephrosis was encountered in one out of 35 patients. This patient had mild unilateral hydronephrosis. Based on the results of this study, routine examinations to detect hydr

Clean intermittent catheterization reverses hydronephrosis in a child with congenital nephrogenic diabetes insipidus: a case report.

BACKGROUND: Congenital nephrogenic diabetes insipidus (CNDI) is most frequently caused by mutations in the AVPR2 gene. Patients exhibit persistent polyuria due to renal insensitivity to antidiuretic hormone. Chronic high urine output predisposes to bladder dysfunction and upper urinary-tract dilatation, notably hydronephrosis. Although pharmacotherapy can partially reduce urine volume, its capacity to reverse established hydronephrosis is limited. Clean intermittent catheterization (CIC), a mainstay in managing neurogenic bladder, warrants investigation regarding its utility in CNDI-associated

Bladder management in children with transverse myelitis.

BACKGROUND: Patients with transverse myelitis (TM) often present with urinary retention. While many recover their bladder function, some have persistent voiding dysfunction, and both intermediate and long-term outcomes are variable. OBJECTIVE: In patients who develop urinary retention requiring clean intermittent catheterization (CIC) at onset of TM, we sought to assess factors associated with improved voiding function and the risk of requiring persistent CIC over time. STUDY DESIGN: We reviewed children evaluated at our institution for TM from April 1998 to October 2018. Patients were inclu

Continent urinary diversion in a patient with prune belly syndrome and urethral atresia.

A case of prune belly syndrome which was managed by left ureterostomy and right nephrectomy during the neonatal period is reported. The patient also had anorectal agenesis with rectovesical fistula, urethral atresia and patent urachus. When the patient was 14 years of age, the urinary diversion was converted to a continent diversion by an antireflux ureteroneocystostomy of both the left and the right ureter together with a right cutaneous ureterostomy. The patient manages very well by clean intermittent catheterization of the right ureterostomy and is fully continent.

Bilateral rectus femoris pedicle flaps for detrusor augmentation in the prune belly syndrome.

A 20-year-old patient with the prune belly syndrome presented with urinary retention and renal failure. In preparation for renal transplantation bladder rehabilitation, consisting of urethral dilation, reduction cystoplasty and detrusor augmentation with paired rectus femoris pedicle flaps, was undertaken. This procedure enabled the patient to empty the bladder totally by spontaneous voiding. Successful renal transplantation was performed subsequently and he has not required intermittent or continuous catheterization. The surgical approach and technique are described, and their application to

Quality of Life and Anxiety Levels in Patients Using Clean Intermittent Catheterization.

PURPOSE: Clean intermittent catheterization (CIC) is a reliable and effective method of maintaining bladder function. However, if not properly managed, especially during long-term use, it can lead to complications such as pain, infection, bleeding, urethritis, and urethral stricture, negatively affecting the individual's quality of life. This study was conducted to evaluate the quality of life and anxiety levels of patients undergoing CIC, and to determine the factors affecting them in relation to descriptive characteristics and CIC usage. MATERIALS AND METHODS: A descriptive cross-sectional

Comparative 30-day life cycle assessment of bladder drainage strategies.

OBJECTIVE: To identify the most environmentally sustainable urinary drainage strategies commonly used to manage bladder outlet obstruction and also to highlight actionable levers for reducing the footprint of urinary drainage in clinical practice. MATERIALS AND METHODS: We conducted a comparative life cycle assessment of three urinary drainage strategies (i.e., indwelling urinary catheter, clean intermittent self-catheterisation [CISC], and temporary urethral stent) over a 30-day period, in accordance with International Organization for Standardization (ISO) 14040 and ISO 14044 standards.

The rationale for bladder washouts in children with neurogenic bladder.

Children with a neurogenic bladder are at risk of developing recurrent urinary tract infections and long-term kidney failure. Due to an altered lower urinary tract, children may be overtreated for simple bacteriuria or undertreated for a potentially severe urinary tract infection. This group of patients represent high users of healthcare, and are at risk of colonization and development of antibiotic resistance. Bladder washouts with non-antibiotic electrochemically activated solutions are a potential new prophylactic option for patients with bladder dysfunction when clean intermittent catheter
