RATIONALE AND OBJECTIVES: Compartmental anatomy of the forearm is complex and controversial. The purpose of this investigation was to demonstrate via magnetic resonance the anatomy of forearm compartments and their preferential ways of communication in a human cadaveric model.
METHODS: Twelve forearm spaces (11 muscles and one intermuscular) were injected with a solution containing gadopentetate dimeglumine. The extent and distribution of leakage were evaluated on axial fat-suppressed T1-weighted spin-echo images. The findings were related to known descriptions of compartmental anatomy of the

This case-based review explores the biomechanical foundations of the wrist by integrating advanced imaging findings with core anatomical and functional principles. We analyze six representative scenarios: distal radioulnar joint instability, ulnocarpal impaction, unstable triangular fibrocartilage complex tear, scapholunate ligament injury, posttraumatic ulnar translocation of the carpus, and scaphotrapeziotrapezoid osteoarthritis. The cases reviewed here illustrate how static and dynamic stabilizers preserve carpal congruence and load transmission. We highlight the roles of the triangular fib

An unstable distal radioulnar joint can cause ulnar-sided wrist pain. Stability of the distal radioulnar joint, which varies among individuals, is dependent on the bony structure of the sigmoid notch and the surrounding soft tissues, such as the interosseous membrane, the extensor carpi ulnaris tendon, the pronator quadratus, and the deep radioulnar ligaments. In particular, avulsions of the deep radioulnar ligaments lead to instability and disturbed wrist proprioception. In patients with ulnar-sided pain, a distiction must always be made between clinical instability and generalized laxity. Tr

Non-union in forearm fractures is an uncommon but complex problem. This is especially given the unique anatomical structure and function of the forearm, making treatment distinctly different to that of other long bone fractures. Anatomical restoration of length, alignment, rotation and the radial bow maintains the optimal stabilising effects of the forearm muscles and interosseous membrane, as well as maximising the range of movement, particularly pronation and supination. Contemporary plate osteosynthesis using variations of the dynamic compression plate (DCP) developed by the AO group combin

BACKGROUND: Persistent or recurrent instability of the radial head (RH) remains a challenge in treating anterior Monteggia fractures despite anatomic restoration of the ulna. RH instability may be caused by the pull of the biceps muscle with rupture of the annular ligament and other soft tissue stabilizers of the proximal radius. Currently, the optimal method to stabilize the proximal radius is unknown. The purpose of this study was to compare annular ligament repair with three different ligament reconstructions in restoring anterior RH stability.
METHODS: Eight cadaveric upper extremities we

Osteogenesis imperfecta (OI) is a rare inherited connective tissue disorder. It is characterized by short stature, fragility and decreased bone mass, which leads to multiple and recurrent fractures after low-energy trauma, which generates susceptibility to long bone deformity and vertebral compression. There are several types of OI, with types I to IV, in which the COL1A1 and COL1A2 genes are affected, being the most frequent. In recent years, the discovery of new forms of OI has led to research into the pathways critical aspects of bone metabolism, with new genes involved being identified. Th

PURPOSE: To report a very rare presentation of a stress fracture in a golfer: a stress fracture of the ulnar diaphysis.
CASE SUMMARY: A 44-year-old woman had a 4-week history of a sore left wrist. She had been playing golf daily and had had no sudden trauma. Extensor tendinitis was suspected. Symptomatic treatment was given. Radiographs showed a spiculated periosteal reaction of the distal ulnar diaphysis with no signs of fracture or bone destruction. The suggestion of a malignant lesion was raised. Magnetic resonance imaging showed a healing ulnar fracture and edema of the interosseous membr

Radial head replacement in complex elbow fractures (Mason III) with either bony or ligamentous injuries or interosseous membrane rupture is unquestionable. Actual modular and metallic prosthesis ease the mounting technique and ensure durability. Nevertheless, these types of prostheses are not always available in a short time in our daily practice. We present the use of a transient polymethacrylate spacer as an alternative in nonreconstructable complex radial head fractures with a unstable elbow. We assessed 38 patients between 2006 and 2007, with a median follow-up of 53.8 months. We included

Thoracic ossification of ligamentum flavum (OLF) caused by skeletal fluorosis is rare. Only six patients had been reported in the English literature. This study reports findings from the first clinical series of this disease. This was a retrospective study of patients with thoracic OLF due to skeletal fluorosis who underwent surgical management at the authors' hospital between 1993 and 2003. Diagnosis of skeletal fluorosis was made based on the epidemic history, clinical symptoms, radiographic findings, and urinalysis. En bloc laminectomy decompression of the involved thoracic levels was perfo

Common fibular nerve (CFN) injury due to ankle fracture is an underreported complication. The authors have proposed that torsional injury to the ankle can be translated along the interosseous membrane (IOM), producing tension on the CFN at the fibular neck. A 23-year-old woman presented to our clinic for left foot drop. Three months prior, the patient sustained a fall with left ankle inversion injury while running. She was diagnosed with a minor ankle fracture and placed in an orthopaedic boot. Unfortunately, her swelling worsened and one week later the patient was diagnosed with foot drop, wh

The actions of pyridostigmine (Pyr), a quaternary carbamate compound, and physostigmine ( Phy ), a tertiary carbamate, both known for their reversible inhibition of acetylcholinesterase (AChE), were studied on the electrically excitable membrane and acetylcholine (ACh) receptor of the frog cutaneous pectoris, sartorius, and interosseal muscles, as well as the chronically denervated soleus muscle of the rat and myoballs from neonatal rats. Both Pyr and Phy first potentiated, then depressed and finally blocked the indirectly evoked muscle twitch. Pyr and Phy had negligible effects upon either me

Essex-Lopresti injury is characterized by a radial head fracture accompanied by dislocation of the distal radioulnar joint and rupture of the interosseous membrane (IOM). This type of injury typically results from high-energy axial forces transmitted through the wrist to the elbow, leading to tears in the IOM, fractures of the radial head, injuries to the distal radioulnar joint, and disruption of the triangular fibrocartilage complex, ultimately causing axial instability of the radioulnar joint. Due to its complexity, this injury is often overlooked in clinical settings, with a reported misdi

UNLABELLED: An absolutely convincing technique of anterior transfer of the tibialis posterior (TP) tendon for treating drop foot has not been developed. Thirty-seven consecutive adult patients with drop foot owing to deep peroneal nerve injury were treated with bone-to-bone TP tendon transfer. The TP tendon with a small bony attachment was procured from the undersurface of the navicula and then transferred through a tunnel of the interosseous membrane. The navicular attachment was implanted in the tunnel of the navicula or intermediate cuneiform. Cancellous bone graft procured from the distal

The forearm is composed of the radial and ulnar shafts, which are linked by the interosseous membrane and intercalated between the elbow and wrist. The radius and ulna are connected by three joints, the proximal, middle, and distal radioulnar joints. The forearm ensures pronation/supination and longitudinal load transfer. The biomechanical and clinical relevance of the proximal and distal radioulnar joints is well established. In contrast, the middle radioulnar joint was considered relatively unimportant until studies published in the last decade showed that it fulfils crucial biomechanical fu

Surgical correction was performed on nine patients who had equinovarus deformity caused by severe crush injury of the leg sustained in an earthquake. The operative procedure used involved the transfer of the posterior tibial tendon to the dorsum of the foot by passing it through the interosseous membrane using a modified procedure as published in 1978. This procedure was combined with percutaneous Achilles tendon lengthening and tenotomy of toe flexors when needed. The average follow-up time after the operation was 21 months. The treatment improved the heel-toe steppage gait in all patients an

A comparative morphological analysis of human and non-human hominoids was conducted in an attempt to determine the mode of locomotion of the protohominid. Although the generalized hominoid anatomy permits variation of locomotion: brachiation, knuckle-walking, etc., minor variations in structure determine which behavior is favored. Arboreal arm swinging requires a flexible forelimb while terrestrial fist or knuckle-walking demands more rigidity of the hand and wrist. It is demonstrated that the large human thumb accompanied by the strong adduction of the thenar, hypothenar, and palmar interosse

The Essex-Lopresti injury is an extremely rare condition combining radial head or neck fracture, distal radio-ulnar joint dislocation and rupture of the interosseous membrane. However, there are rare or unusual varieties or associated injuries. We report a case of a 46-year-old woman with a posterior dislocation of the radio-humeral joint, a radial shaft fracture, and a distal radio-ulnar joint dislocation. She underwent open reduction and internal fixation of the radial shaft fracture followed by an immobilization by a long elbow splint in supination for 6 weeks. At 18 months of follow-up, sh

Distal radioulnar joint (DRUJ) instability is a rare but disabling problem. Surgical treatment remains a challenge. The advent of arthroscopic techniques has helped to rebuild the triangular fibrocartilaginous complex (TFCC), especially its deep part. However, isolated TFCC damage is not responsible for instability. Its repair is sometimes not sufficient to restore DRUJ stability, or the chronicity of the injury prevents its direct repair. Open surgical procedures still have a role in these cases. They require a detailed knowledge of the various stabilizers of the DRUJ. Passive and active stab

The kinematics of the normal distal radioulnar joint (DRUJ) and the stabilizing function of various structures about the DRUJ were investigated in a study involving six fresh frozen cadavers. Sequential division of the supporting structures was correlated with abnormalities detected by computerized tomography (CT). The infratendinous portion of the extensor carpi ulnaris is a major restraint against dorsal and palmar subluxation. Division of the radioulnar ligaments and triangular fibrocartilage alone produced only minor changes. Lateral displacement was controlled by the interosseous membrane

BACKGROUND: Ankle syndesmosis injury is difficult to diagnose accurately at the initial visit. Missed diagnosis or improper treatment can lead to chronic complications. Complete syndesmosis injury with a concomitant rupture of the interosseous membrane (IOM) is more unstable and severe. The relationship between this type of injury and Maisonneuve injury, in which the syndesmosis is also injured, has not been discussed in the literature previously.
CASE SUMMARY: A 16-year-old patient sustained left medial malleolar fracture, and the associated inferior tibiofibular syndesmotic instability was
