Liver disease associated with intestinal failure in the small bowel syndrome.

The introduction of the Total Parenteral Nutrition (TPN) has given rise to a new hope in the treatment of intestinal failure (LF) associated with the Short Bowel Syndrome (SBS). However, together with the TPN and the increase of survival of these patients, new problems and questions have emerged, as well as new therapeutical procedures. Taking into consideration this emerging reality, this paper has the purpose to undertake a review of current concepts and available treatments for patients with IF associated-liver disease. Although TPN provides an increase of survival of patients with intestin

Treatment failure in intestinal strongyloidiasis: an indicator of HTLV-I infection.

BACKGROUND: The association of severe strongyloides with HTLV-I is well known; however, the seroprevalence of HTLV-I in other groups with strongyloidiasis is still unknown. We conducted a prospective study in patients with intestinal strongyloidiasis without known immunodepression who failed to respond to standard therapy with ivermectin or thiabendazole (failure was defined as one positive stool examination at the post-therapy follow up). All these patients were tested for HTLV-I by ELISA and Western Blot. RESULTS: Forty seven patients were evaluated: 74.5% (35 out of 47) were HTLV-I positiv

Upper gastro-intestinal mucosal changes in patients with chronic renal failure.

A prospective study was undertaken to determine the radiological, endoscopic and histological changes of upper gastro-intestinal mucosa in patients with end-stage renal failure and the effect of haemodialysis. Of the 30 patients studied, 28 (93%) had presented with upper gastrointestinal symptoms. Abnormal findings on upper gastrointestinal endoscopy were observed in 66.7% patients. These comprised oesophagitis (23.3%) gastritis (30%), duodenitis (33.3%) and peptic ulcer (6.7%). Histological study of the mucosal tissue biopsy in 24 patients showed oesophagitis, gastritis and duodenitis in 12.5

Human and canine ventricular vasoactive intestinal polypeptide: decrease with heart failure.

Vasoactive intestinal polypeptide (VIP) is a systemic and coronary vasodilator that may have positive inotropic properties. Myocardial levels of VIP were assayed before and after the development of heart failure in two canine models. In the first, cobalt cardiomyopathy was induced in eight dogs; VIP (by radioimmunoassay) decreased from 35 +/- 11 pg/mg protein (mean +/- SD) to 5 +/- 4 pg/mg protein (P less than 0.05). In six dogs with doxorubicin-induced heart failure, VIP decreased from 31 +/- 7 to 11 +/- 4 pg/mg protein (P less than 0.05). In addition, VIP content of left ventricular muscle o

Improving Appropriateness and Timeliness of Antibiotic Delivery in Pediatric Intestinal Failure.

BACKGROUND: Children with pediatric intestinal failure (PIF) on parenteral nutrition through central venous catheters are at high risk for central line-associated bloodstream infections (CLABSI), a major cause of morbidity and mortality. Current guidelines recommend prompt empiric antibiotic therapy, yet limited data exist on quality improvement initiatives in this population. OBJECTIVE: To increase appropriate antibiotics ordered in the emergency department (ED) for febrile PIF patients by 50% and increase antibiotics administered within 60 minutes by 50% within 2 years. METHODS: This singl

From childhood to adulthood in chronic intestinal failure: A nationwide study.

BACKGROUND AND AIMS: Pediatric chronic intestinal failure (CIF) requires long-term home parenteral nutrition (HPN) to support growth and health. Transitioning to adult care is a critical period, and long-term outcomes remain underexplored. This nationwide study assessed long-term survival, HPN dependence, psychosocial status and characteristics of CIF management in adult patients with pediatric-onset CIF who received HPN during childhood and transition to adulthood. METHODS: We conducted a multicenter retrospective study of patients who started HPN for CIF in childhood and transitioned to adu

Chronic intestinal failure: Medical treatment, home parenteral nutrition, training and monitoring.

Chronic intestinal failure (CIF) requires parenteral nutrition (PN) as its main treatment to sustain health. Short Bowel Syndrom (SBS) is its main cause. Medical management includes dietary approaches, anti-secretory and anti-motility drugs. Recently, GLP-2 analogs like teduglutide for selected patients have helped treated patient to reduce PN. A multidisciplinary nutritional support team (NSTs) is essential for HPN training, covering catheter care, complication prevention, and lifestyle adaptation to promote patients' autonomy. Monitoring protocols recommend 3 months-interval visits for stabl

FEEDING CHALLENGES IN PEDIATRIC INTESTINAL FAILURE: INSIGHTS FROM THREE DIFFERENT INSTRUMENTS.

BACKGROUND & AIMS: Feeding difficulties are frequently reported in children with intestinal failure (IF) and may represent an important barrier to transition from parenteral nutrition (PN) to enteral autonomy. Appropriate assessment tools are essential for accurate recognition and management of feeding challenges. The objective was to evaluate feeding difficulties using three complementary assessment instruments in children with IF receiving prolonged PN. METHODS: This cross-sectional study included children aged 6 months to 18 years with IF receiving PN for more than two months. Children wit

Measuring quality standards in chronic intestinal failure: A pilot international survey.

BACKGROUND & AIMS: It is recognised that the approach to chronic intestinal failure (CIF) care delivery can differ both within and between countries. The aim of this pilot study was to explore the utility and fulfilment of the European Society for Clinical Nutrition and Metabolism-endorsed quality-of-care standards across European CIF centres. METHODS: To evaluate differential approaches to quality-of-care delivery, lead clinicians of small (<30 current patients), medium (30-100 patients) and large (>100 patients) CIF centres from five countries were asked to complete a 139-item questionnaire

Chronic Mucosal Inflammation in Pediatric Intestinal Failure Patients-A Unique Phenomenon.

INTRODUCTION/OBJECTIVES: As intestinal failure (IF) management improves and long-term survival rate increases, its physiological complications have become more apparent. The development of chronic intestinal inflammation resembling inflammatory bowel disease (IBD) in this population has been reported, but the literature describing it in detail is sparse. The present study was designed to characterize children with IF who developed chronic intestinal inflammation and identify the potential predisposing clinical factors. METHODS: This retrospective study was based on the electronic medical reco

Vertigo-Associated Vomiting: Acute Presentation of Thiamine Deficiency in Intestinal Failure.

Thiamine deficiency can manifest as Wernicke encephalopathy, with the classic clinical triad of altered mental status, nystagmus, and ataxia. Although a rare diagnosis in pediatric patients within developed countries, gastrointestinal disorders that impair nutritional intake and absorption can place patients at higher risk. Rapid diagnosis and early empiric treatment of Wernicke encephalopathy is paramount due to high risk of mortality and long-term morbidity. We present a patient with intestinal failure who developed thiamine deficiency following weaning off parenteral nutrition with acute on

[13C]Methionine breath test to assess intestinal failure-associated liver disease.

Oxidation of L[1-C]methionine ([C]-Met) in liver mitochondria can be quantified by measuring exhaled CO2. We hypothesized that CO2 recovery after i.v. administered [C]-Met would provide a noninvasive measure of liver function in pediatric intestinal failure-associated liver disease (IFALD). After Institutional Review Board (IRB) approval, 27 patients underwent L[1-C]-Met breath tests ([C]-MBTs), five of whom underwent repeat testing after clinical changes in liver function. Sterile, pyrogen-free [C]-Met was given i.v. Six breath samples collected during 120 min were analyzed for CO2 enrichment
