Can a child survive a basilar artery injury during endoscopic third ventriculostomy? case-based review.

BACKGROUND: Endoscopic third ventriculostomy (ETV) has become a cornerstone in the management of obstructive hydrocephalus, offering an alternative to shunt placement. Although considered safe, rare vascular injuries-particularly those involving the basilar artery-can lead to catastrophic complications. These lesions may remain clinically silent initially, resulting in delayed pseudoaneurysm diagnosis. METHODS: We report a pediatric case of basilar artery pseudoaneurysm following ETV and tumor biopsy, accompanied by a literature review on the incidence, diagnostic strategies, management, and

Safety of intraoperative transcranial electrical stimulation motor evoked potential monitoring.

This article reviews intraoperative transcranial electrical stimulation (TES) motor evoked potential (MEP) monitoring safety based on comparison with other clinical and experimental brain stimulation methods and clinical experience in more than 15000 cases. Comparative analysis indicates that brain damage and kindling are highly unlikely. There have been remarkably few adverse events. Pulse train TES-induced or coincidental seizures (n = 5) are rare, probably because of very brief (<0.03 second) stimuli, anesthesia, and the general absence of predisposing cerebral conditions. Post-tonsillectomy hemorrhage: an assessment of risk factors.

Hemorrhage is the most frequent complication of tonsillectomy and is responsible for the majority of post-tonsillectomy fatalities. The incidence of this hemorrhage has been reported to be as high as 20% [6]. Despite continued efforts to reduce this problem, it remains a persistent risk. The charts of 1138 patients who underwent tonsillectomy with or without adenoidectomy from 7-1-89 to 6-30-93 were reviewed. Post-tonsillectomy hemorrhage occurred in 36 patients (3%). Preoperative, intraoperative and postoperative risk factors were assessed. Postoperative bleeding occurred more often in older

Caecal bascule: a rare complication following emergency caesarean section.

Caecal bascule is an unusual type of volvulus that presents a challenging diagnosis for clinicians. We present a case of a forty-two year old female who developed a perforated caecal bascule five days post emergency caesarean section. The diagnosis of caecal bascule was made intraoperatively during a hemicolectomy. Greater awareness of this phenomenon and its clinical and radiological findings is important to avert the development of bowel perforation or gangrene.

Persistent left superior vena cava: Review of embryologic anatomy and considerations for cardiopulmonary bypass.

The purpose of this report is to alert the perfusionist to the anatomy of persistent left superior vena cava (PLSVC) and the intraoperative complications that this anomaly may present. The perfusionist should be able to devise a venous circuit that will accommodate three cannulae. He should also be aware that, during cardiopulmonary bypass, a copious volume of deoxygenated blood from the left heart sump, with simultaneous cerebral congestion, may signal the presence of undocumented PLSVC.

Clipping of a cerebral aneurysm in a patient with Loeys-Dietz syndrome: case report.

BACKGROUND AND IMPORTANCE: his is the first case report of clipping a cerebral aneurysm in a patient with Loeys-Dietz syndrome (LDS). LDS is a newly described autosomal dominant connective tissue disease with systemic vascular involvement. Unique to this syndrome is the development of aneurysms at a young age with the propensity of dissection or rupture at a stage that is earlier than when surgical intervention is typically indicated. We describe the nuances in intraoperative and postoperative management. CLINICAL PRESENTATION: A 31-year-old woman who recently received a diagnosis of with LDS

Parallel Configuration of the Cystic and Common Hepatic Ducts: a Surgical Odyssey.

INTRODUCTION: Laparoscopic cholecystectomy is among the most frequently performed surgical procedures worldwide; however, it carries a persistent risk of iatrogenic bile duct injury, particularly in the presence of anatomical variations. Misinterpretation of biliary anatomy remains a leading cause of serious complications, underscoring the importance of recognizing rare variants and adopting safe surgical strategies. CASE REPORT: We report the case of a 78-year-old patient undergoing elective laparoscopic cholecystectomy for recurrent cholecystitis and cholangitis. Given the history of repeat

Closed loop obstruction and adhesive intestinal obstruction in perineal hernia.

Perineal hernia with bowel gangrene is uncommon but known complication of laparoscopic extralevator abdominoperineal excision (ELAPE). We present a rare case of closed loop small bowel obstruction with bowel gangrene secondary to an incarcerated perineal hernia that developed 7 years after an ELAPE. Intraoperatively, we found a definitive transition point due to adhesions in pelvis and a closed loop obstruction of the distal small bowel at different site with gangrenous intestine. She was managed successfully surgically with adhesiolysis and fixation of defect with biological mesh. Prevalence

Intraoperative surgical specimen evaluation: frozen section analysis, cytologic examination, or both? A comparative study of 206 cases.

Having recently become aware of the merits of cytologic preparations, histopathologists are focusing their attention on cytologic examination as a means of intraoperatively evaluating surgical specimens. This study compares the diagnostic accuracy and quality of frozen-section (FS) and cytologic preparations from 206 surgical specimens. The quality of cytologic preparations was significantly superior to that of FSs (P = 0.0001). With the use of a three-level accuracy scale suited to the practical demands of intraoperative evaluation, there was no significant difference between the accuracy of

A national survey of anaesthetists (NAP5 baseline) to estimate an annual incidence of accidental awareness during general anaesthesia in the UK.

BACKGROUND: As part of the 5th National Audit Project of the Royal College of Anaesthetists and the Association of Anaesthetists of Great Britain and Ireland concerning accidental awareness during general anaesthesia, we issued a questionnaire to every consultant and staff and associate specialist anaesthetist in the UK. METHODS: The survey was designed to ascertain the number of new cases of accidental awareness that became known to them, for patients under their direct or supervised care, for a calendar year, and also to estimate how many cases they had experienced during their careers. Bispectral index during epidural puncture predicts anterograde amnesia in patients given midazolam premedication.

PURPOSE: We hypothesized anterograde amnesia could be predicted by the bispectral index (BIS) during epidural puncture in patients premedicated with intramuscular midazolam. METHODS: We investigated 64 consecutive patients undergoing gynecological laparotomy under general anesthesia combined with epidural anesthesia. Midazolam (5 mg) was administered intramuscularly at 15 min before arrival at the operating room. The anesthesiologist informed the patient of the operating room number after evaluating her using the Observer's Assessment of Alertness and Sedation (OAA/S) scale. A BIS probe was t

Technological Advances and Adjuncts to Optimize Outcomes in Craniomaxillofacial Trauma.

Complex maxillofacial trauma is a challenging problem for even the most experienced reconstructive surgeon. However, advances in presurgical planning, utilization of intraoperative technology, occlusal splints, three dimensional (3D) printing, and creation of custom plates can aid in simplifying challenging cases, decreasing operative time, and optimizing patient outcomes. However, there are limitations to the use of these innovations that surgeons must be aware of. This article will discuss technological advances in maxillofacial trauma.

Circumaortic left renal vein: Surgical challenges and management in a case report and literature review.

INTRODUCTION AND IMPORTANCE: The circumaortic left renal vein is an uncommon anatomical variant, accounting about 10 % of cases. Undiagnosed, this vascular anomaly may increase the risk of intraoperative vascular injury, particularly during renal surgeries or transplantation procedures. Recognizing this variation preoperatively is essential to minimize intraoperative challenges and improve patient outcomes. CASE PRESENTATION: We report the case of a 54-year-old patient who referred to our institute for the management of serous endometrial adenocarcinoma diagnosed following postmenopausal metr

Massive Localized Lymphedema: Analysis of Intraoperative Care.

Massive localized lymphedema (MLL) is rare, but as the prevalence of obesity in adults rises, it can be foreseen that cases will increase in prevalence. These patients are unable to ambulate and have uncontrollable rash and skin breakdown. Operative management for MLL is offered when nonsurgical measures are ineffective. The surgery can be difficult with ambiguous tissue planes saturated with lymphatic fluid, ill-defined margins, large arteries and dilated veins, and extensive intraoperative fluid and blood loss. We performed a retrospective chart review and analysis with a focus on the intrao

Accuracy of Fluoroscopic Navigation for Predicting Leg Length and Offset in Primary Hip Arthroplasty.

BACKGROUND: Fluoroscopic-assisted computer navigation is widely used to guide intraoperative decisions on leg length (LL) and offset in primary total hip arthroplasty (THA), but its accuracy remains under-evaluated. This study assessed the precision of a fluoroscopy-based navigation system by comparing its intraoperative predictions to validated postoperative measurements. METHODS: We retrospectively reviewed 490 consecutive primary THA cases performed by three surgeons at a tertiary academic center. Intraoperative changes in LL and offset using a fluoroscopic navigation system were recorded.

Neurotoxicity of subarachnoid Gd-based contrast agent accumulation: a potential complication of intraoperative MRI?

OBJECTIVE: Intraoperative MRI with Gd-based contrast agent (GBCA) improves the extent of resection of contrast-enhancing brain tumors. Signal changes of CSF due to perioperative GBCA leakage in the subarachnoid space have been reported. However, although GBCA potentially exhibits neurotoxic effects, so far no associated complications have been described. In this case series, the authors report a single-center cohort of patients with subarachnoid GBCA extravasation after intraoperative MRI and discuss potential neurotoxic complications and potential ways of avoiding them. METHODS: All patients

Risk of Bone Wax Migration During Retrosigmoid Craniotomy for Microvascular Decompression: Case-Control Study.

BACKGROUND AND OBJECTIVES: Bone wax is a flexible hemostatic agent commonly used for surgery in the posterior cranial fossa to control bleeding from the mastoid emissary vein. A large amount of bone wax can migrate into the sigmoid sinus through the mastoid emissary canal (MEC). We aimed to identify clinical factors related to intraoperative bone wax migration through the MEC during microvascular decompression (MVD) surgery, which may result in sigmoid sinus thrombosis. METHODS: We retrospectively collected the clinical data of patients with trigeminal neuralgia, hemifacial spasm, or trigemin
