Levonorgestrel intrauterine system embedded within tubal ectopic pregnancy: a case report.

BACKGROUND: The presence of the levonorgestrel-releasing intrauterine system embedded within an ectopic pregnancy is a rare occurrence. Tubal migration of an intrauterine device is not well understood and has not been extensively studied in literature. CASE PRESENTATION: A 34-year-old African woman, para 1, gravida 2, presented with symptoms of ruptured ectopic pregnancy. She underwent a laparoscopy where a ruptured left ectopic pregnancy was found with a levonorgestrel-releasing intrauterine system inserted 2 years prior embedded within the tube. A left salpingectomy was performed with remov

Laparoscopic removal of an intrauterine device following colon perforation.

BACKGROUND: Intrauterine device (IUD) migration and colon perforation is a rare but serious complication occurring sometimes years after insertion. Laparoscopic removal of a colon-perforating device is a seldom-used approach. CASE: A 35-year-old presented with right upper quadrant (RUQ) abdominal pain one year following intrauterine device (IUD) insertion. Upon vaginal examination, no IUD string was detected. An abdominal X-ray depicted the IUD in a position below the hepatic flexure. Abdominal computerized tomography (CT) scan suggested an IUD-like foreign object posterior and medial to the

The perforated intrauterine device: endoscopic retrieval.

BACKGROUND: Uterine perforation is the most serious complication associated with an intrauterine contraceptive device (IUD). Minimally invasive techniques, such as hysteroscopy and advanced laparoscopy, are ideally suited to the diagnosis and surgical management of the perforated IUD. CASE REPORTS: Three cases of uterine perforation caused by an IUD and treated with endoscopic surgery are presented. In all 3 cases, the IUD was located by using x-rays, ultrasonography, or pelvic magnetic resonance imaging. Diagnostic laparoscopy was performed to identify the specific location of the IUD and to

Surgical removal of an intrauterine device perforating the sigmoid colon: a case report.

BACKGROUND: Intrauterine devices (IUDs) are among the most common contraceptive methods in the world, but have been associated with extrauterine dislocation and bowel perforation. We report a case of an IUD perforation of the sigmoid colon. CASE REPORT: A 35-year-old woman with complaints of abdominal pain, fever, and diarrhea underwent exploratory laparotomy. During the procedure, a left tubo-ovarian abscess was found as well as an IUD extruding through the uterine cavity with the two wings of the device entirely in the lumen of the sigmoid colon. The patient had total abdominal hysterectomy

Minimally invasive transurethral holmium laser surgery for the management of an IUD with bladder migration.

CASE: Intrauterine device (IUD) is used worldwide as an effective contraceptive method, but the migration of IUD is a serious complication. We report the case of IUD migration leading to bladder calculus formation and a minimally invasive transurethral surgical approach was performed for treatment. Holmium laser was used to break up the bladder calculus and cut through the bladder mucosa where the IUD was attached, finally the IUD was removed through the urethra. This minimally invasive procedure is a safe and effective treatment for IUD migration, and similar cases have not been reported in t

Gynaefix frameless IUD: Cause of bowel resection.

A case of a lost GyneFix intrauterine contraceptive device (IUD) is described, in which laparoscopy failed to identify the device and laparotomy had to be carried out to remove the IUD, which was embedded in the small bowel necessitating bowel resection. Awareness of this complication is necessary, and advanced training is required in order to minimize risks. A description of the GyneFix device, the possible adverse effects and incidence of complications, the importance of post-insertion follow-up, and the need for awareness of the possibility of migration through the bowel are discussed.

[Intra-vesical translocation of an intrauterine device, report of a case].

The intra-uterine device TCu 380 (IUD) has a great acceptance among the group of women that take the fertility control in not definitive form and it is being accepted by more women. In spite of the simple technique to insert the IUD, the people in charge of the planning family program in the first level attention section of the centers of health, have not enough knowledge for its insertion. We reported the case of a 21 year old woman that applied the IUD immediately after her baby's birth and she had sudden expulsion the following week. She went to the first level attention section where they

[Intrauterine contraceptive device in an appendix--a case report].

The application of an intrauterine contraceptive device can perforate the uterus and migrate to adjacent organs such as the bladder or small bowel. The main symptoms are painful insertion of the intrauterine contraceptive device and missing IUD strings. The diagnosis of perforation and transuterine migration of the IUD is made on the basis of an ultrasound examination and an abdominal X-ray. The proper management is such case is immediate removal of the IUD. The aim of this paper was to present a case of a 34-year-old woman with a copper IUD found during a caesarean section.

Colonoscopic retrieval of a lost intrauterine contraceptive device: a case report and review of articles.

A case of translocated intrauterine contraceptive device (IUCD) lying partly in the bowel wall at the rectosigmoid junction and its removal by colonoscope is described. This case highlights the possibility of safe retrieval of an IUCD by colonoscopy when it is partly embedded in the bowel wall. Routine sigmoidoscopy alongside other investigations is recommended for translocated IUCDs. Its use can select those patients for whom rectal recovery of the IUCD is feasible, thus avoiding unnecessary surgical intervention.

The lost intrauterine device: removal by hysterectomy.

A case of migration of an intrauterine device (IUD) deep within the endometrium, presenting as heavy vaginal bleeding and abdominal pain, is discussed. The IUD had been in situ for 31 years. Standard retrieval techniques failed and removal necessitated hysterectomy. The complications of 'lost IUDs' are discussed and the need to enquire about removal in all women on approaching the menopause is emphasised.

Management of perforated essure with migration into small and large bowel mesentery.

The Essure permanent birth control system (Conceptus Inc, San Carlos, CA) is currently the only Food and Drug Administration-approved hysteroscopic sterilization method and has been widely accepted as a safe and effective procedure. We present a rare case of tubal perforation, coil fragmentation, and distal migration into small and large bowel mesentery 8 days after the insertion of the Essure device. We describe the successful management of this complication using laparoscopy and intraoperative fluoroscopy. Providers using Essure must be aware of the possibility of fragmentation of the Essure

The intrauterine contraceptive device: an often-forgotten and maligned method of contraception.

Although 90% of women at risk for unintended pregnancy in the United States use contraception, <1% of these women use the intrauterine contraceptive device. The mechanism of action of intrauterine contraceptive devices has been controversial, but several studies suggest that interference with sperm migration or function and with fertilization may be the most likely mechanisms. More important, there is lack of compelling evidence that the intrauterine contraceptive device acts as an abortifacient. The risk for pelvic inflammatory disease among users now appears to be extremely low, primarily as

A case report of removal and uterine suturing of an incarcerated intrauterine sterilization ring.

The clinical management of a patient with adenomyosis in whom the levonorgestrel intrauterine release system was completely embedded in the myometrium is reported. The intrauterine device was located in the pelvis using qualitative imaging (plain abdominal X-ray, computed tomography, magnetic resonance imaging); localization (intraoperative ultrasound localization of the device within the myometrium of the anterior wall of the lower uterine segment); and definitive assessment (hysteroscopy to guide surgical manipulation and subsequent uterine suturing after removal of the device). The purpose

Bladder perforation related to intrauterine device.

Intrauterine devices (IUDs) are currently one of the most popular reversible contraception methods used world wide. Uterine perforation is a rarely observed complication. The bladder is one of the organs that an IUD can migrate to because of its close proximity to the uterus. There are about 70 cases in the literature of IUDs that have migrated into the bladder. The resulting bladder perforation can be complete or partial. Here, we report 2 cases, 1 of complete migration and the other of partial migration.
