The Intersection of Incarceration and Reproductive Health: Contraceptive Perspectives of Women in Rural Maine Jails.

The U.S. carceral health care system presents a unique opportunity to expand reproductive health services. Evidence suggests that women incarcerated in jails want to initiate contraception, but access varies across facilities and is limited by concerns about cost, reproductive coercion, and a lack of consistent policies. To better understand the reproductive health needs of rural incarcerated women, a cross-sectional study of women incarcerated in four rural Maine jails was conducted using a 59-question survey on contraceptive history, needs, and preferences. Seventy-three women with complex h

Assessing Rural-Urban Differences in Screening for Mental Health Needs Among Individuals in County Jails.

Rural jails are increasingly contributing to the overall jail population and little is known about how they differ from non-rural jails. This study compares demographic, behavioral health and criminal/legal histories of 3,797 individuals who booked into three rural jails and seven non-rural jails. In addition, the study assessed how jails identify mental illness, which was compared to an objective screening instrument (Kessler-6). Individuals in rural jails were more likely to be white, female, have a history of mental health services, misuse substances, and to recidivate. After controlling fo

Post-incarceration outcomes for individuals who continued methadone treatment while in Connecticut jails, 2014-2018.

PURPOSE: To assess post-release outcomes associated with continuation of methadone treatment in correctional centers. METHODS: This case-control study of the post-incarceration impact of pilot methadone programs operating in jails in New Haven and Bridgeport, Connecticut, USA was conducted in 2014-18. The study compared non-fatal overdose, fatal overdose, reincarceration, and resumption of methadone in the community experienced by 1564 eligible men, 660 (42.2 %) of whom continued treatment while incarcerated. RESULTS: Continuation of methadone was associated with a significant decrease in no

Assessing Risk-Based Policies for Pretrial Release and Split Sentencing in Los Angeles County Jails.

Court-mandated downsizing of the CA prison system has led to a redistribution of detainees from prisons to CA county jails, and subsequent jail overcrowding. Using data that is representative of the LA County jail system, we build a mathematical model that tracks the flow of individuals during arraignment, pretrial release or detention, case disposition, jail sentence, and possible recidivism during pretrial release, after a failure to appear in court, during non-felony probation and during felony supervision. We assess 64 joint pretrial release and split-sentencing (where low-level felon sent

An Interpretive Description of Drug Withdrawal Among Pregnant Women in Jail.

OBJECTIVE: To explore the experience of drug withdrawal among pregnant women in jail. DESIGN: A qualitative interpretive descriptive approach. SETTING/PROBLEM: The care of incarcerated pregnant women constitutes a complex and significant public health problem. Many have substance use disorder (SUD) and cycle in and out of jails in their community, resulting in repeated experiences of drug withdrawal. Most jails do not provide medication-assisted therapy for management of withdrawal, a situation that violates standards of care set by leading health organizations. The experience of drug withdr

Young Children's Contact with their Parents in Jail and Child Behavior Problems.

Contact between incarcerated parents and their children can help maintain family relationships. However, links between parent-child contact and child behavior problems are understudied in young children, especially the contexts in which visits and other forms of contact occur. Data were collected from 157 parents incarcerated in one of two jails regarding contact with their young children (age 2-6), as well as children's behavior problems. Findings indicated that jail site moderated the association between visit frequency and child behavior problems. At the jail that offered both on-site video

Preventing Interruptions in Health Care After Release From Jail.

The burden of illness, especially mental health and substance abuse, among the incarcerated population is a major factor for recidivism. County jails throughout the United States provide health care disconnected to care given before and after incarceration. Although the mental health issues of incarcerated individuals are well managed during incarceration, they have inadequate medical coverage and medications upon release. In partnership with the Philadelphia Department of Prisons, we launched a new initiative to enroll women into Medicaid before release and set up their first doctor's appoint

"Just like Jail": Trauma Experiences of Older Homeless Men.

People experiencing homelessness often have a history of trauma, and losing one's home is a traumatic event. The trauma of living in emergency shelters and on the streets uniquely impacts older adults, whose experiences of homelessness increase the likelihood of multiple health conditions and premature aging. This paper includes data from 18 men, aged 50 and older, who had experienced multiple instances of homelessness, as part of a larger study to understand the return path to homelessness from permanent housing. When participants spoke with us about their failure to be in housing, their inab

Unmet Mental Health Needs of Jailed Parents with Young Children.

OBJECTIVE: Mental health symptoms in jailed parents with young children were examined, including relations to gender, race, trauma, and supports. BACKGROUND: Most U.S. incarceration occurs in jails, and it is a significant stressor for families because most incarcerated individuals are parents. Although jails are notorious for housing people with mental health problems, few studies focus on the mental health of jailed parents or supports that might mitigate such problems. METHOD: 165 jailed parents with young children (2-6 years) completed an interview and self-administered questionnaires. R

Epidemiology of Incarceration: Characterizing Jail Incarceration for Public Health Research.

BACKGROUND: Each year, 9 million individuals cycle in and out of jails. The under-characterization of incarceration as an exposure poses substantial challenges to understanding how varying levels of exposure to jail may affect health. Thus, we characterized levels of jail incarceration including recidivism, number of incarcerations, total and average number of days incarcerated, and time to reincarceration. METHODS: We created a cohort of 75,203 individuals incarcerated at the Coconino County Detention Facility in Flagstaff, Arizona, from 2001 to 2018 from jail intake and release records. RE

Posttraumatic Stress Disorder and Job Burnout Among Jail Officers.

OBJECTIVE: The aim of this study was to explore posttraumatic stress disorder (PTSD) symptom prevalence and health characteristics among jail correctional officers, a generally understudied population of public safety workers. METHOD: A Conservation of Resources (COR)-inspired framework explored relationships to PTSD symptoms among jail officers (N = 320) employed in Midwest US jails. RESULTS: More than half (53.4%) of jail officers screened positively for PTSD. Hierarchical regression analysis indicates that burnout was a significant predictor of symptoms of PTSD (B = 0.25, P < 0.001). Self

"It's Not Like Therapy": Patient-Inmate Perspectives on Jail Psychiatric Services.

Jails may serve an important public health function by treating individuals with psychiatric problems. However, scholars debate the service qualities that can best achieve this aim. Some suggest the possibility of comprehensive psychiatric services in jails, while others recommend a narrower focus on basic elements of care (assessments, medication management, and crisis intervention). To date, this debate remains uninformed by service recipients. This qualitative study addresses this gap by illuminating patient-inmate perspectives on jail psychiatric services. Patient-inmate experiences indica

Stress Reduction Through a Brief Writing Intervention With Women in Jail.

OBJECTIVE: To assess the feasibility/acceptability of writing as a stress reduction strategy with incarcerated women. METHODS: The authors implemented a writing intervention in which six women wrote on topics of their choosing for 20 minutes on 5 consecutive days. Feasibility was assessed with investigator field notes; daily post-writing evaluations assessed intervention acceptability. The study also assessed the continuation of writing at 1-month postintervention and acceptability of instruments for evaluation of future interventions. RESULTS: Implementation barriers were minimal; participa

Perceived coercion among jail diversion participants in a multisite study.

OBJECTIVE: Although jail diversion is considered an appropriate and humane response to the disproportionately high volume of people with mental illness who are incarcerated, little is known regarding the perceptions of jail diversion participants, the extent to which they feel coerced into participating, and whether perceived coercion reduces involvement in mental health services. This study addressed perceived coercion among participants in postbooking jail diversion programs in a multisite study and examined characteristics associated with the perception of coercion. METHODS: Data collected
