Among 212 institutionalized mentally retarded, there were 16 patients with keratoconus (7.5%). Eight of the keratoconus cases were unilateral. The possible role of eye rubbing as well as other etiological factors are discussed.

It is observed that few patients over the age of 60 regularly attend the keratoconus clinic at Moorfields Eye Hospital. The hypothesis that patients with keratoconus have a shorter life expectancy owing to underlying connective tissue related disease was tested. During examination of 131 penetrating keratoplasty specimens from patients with keratoconus obtained in an 11-year period, we observed two histopathologic variants based on the appearance of Bowman's layer and the corneal epithelium. Sixteen corneal buttons were obtained from 16 patients with keratoconus at the time of penetrating keratoplasty (mean age, 34 +/- 3.2 yr) and prepared for phosphorus-31 nuclear magnetic resonance spectroscopy following perchloric acid extraction. Two XP patients are presented. A corneal perforation in the left eye of the first patient necessitated an at-random transplantation à chaud. The graft was remarkably well tolerated, which is possibly explained by UV-light-induced suppression of the cellular immune response in the patient. The right eye of this patient and both eyes of the second patient had a keratoconus. In case of keratoconus the epithelial cells are also participating in the pathological processes. Their activity of producing basal membrane is increased. In the stroma four cell types are found; two of them are normal elements. The origin of the two pathological forms is unknown. They may represent as well vagrant cells. Or they originate perhaps from the endothelial cells, being more or less modified. The original endothelial cells are mostly destroyed. In all sorts of the corneal cells one sees abundantly enlarged mitochondria.

In a survey of 162 keratoconus patients, we set out to investigate individual conditions, such as asthma, hay fever, connective tissue syndromes, and eye rubbing, and to determine their relationship of keratoconus. It was found that the prevalence of asthma rises from 0.4% to 1% in the control group to 17.9% in the keratoconus group, or an eighteenfold increase. In addition, the incidence of hay fever in keratoconus patients was found to be 35.7%. The examination of reducible collagen crosslinks in keratoconus cornea revealed the presence of lysinonorleucine in amounts far greater than in normal age-matched corneas. There was no indication of decreased hydroxylysine levels in keratoconus, and there were no clinical indications of a generalized connective tissue disorder. The abnormal levels of dehydrolysinonorleucine in the tissue may represent a change in hydroxylation of selected lysyl residues of normal collagen or the synthesis of abnormal collagen, perhaps an unusual type.

PURPOSE: This clinical case report demonstrates blepharoptosis-induced corneal steepening and its subsequent resolution after blepharoptosis surgery.
METHODS: A 62-year-old man complaining of blurred vision without apparent cause on clinical examination underwent keratometry and computerized corneal topography, which revealed superior corneal steepening in both eyes. Bilateral upper eyelid blepharoptosis surgery was performed.
RESULTS: Three months after blepharoptosis surgery, repeat computerized corneal topography revealed normal corneal contours with improved vision in both eyes.

BACKGROUND: Posterior keratoconus is a rare corneal disorder characterized by a diffuse or localized protrusion of the posterior corneal surface into a thinning stroma. It is believed to be a congenital, nonprogressive condition.
CASE REPORT: A case of bilateral circumscribed posterior keratoconus is presented. Diagnostic ocular signs include localized protrusion of the posterior corneal surface, normal anterior corneal surface, stromal thinning, and endothelial changes.
