Kidney volume and function in autosomal dominant polycystic kidney disease.

BACKGROUND: The significance of total kidney volume (TKV) as a biomarker of kidney function in autosomal dominant polycystic kidney disease (ADPKD) is controversial and has been reappraised. METHODS: Between 2007 and 2012, 64 patients were followed with a mean 39.7-month observation period. TKV measurements by magnetic resonance imaging and estimation of renal function with estimated glomerular filtration rate (eGFR) using the Modification of Diet in Renal Disease equation and 24-h urine creatinine clearance were repeated annually. RESULTS: TKV and its adjusted parameters (height-adjusted, b

[Managing anaemia in kidney transplant patients with chronic kidney disease].

DEFINITION: The definition of anemia is established by the World Health Organization and was subsequently adopted by the American Society of Transplantation, which defines anemia as hemoglobin concentration <12 g/dl in women and <13 g/dl in men. PREVALENCE OF ANEMIA POSTTRANSPLANTATION: Varies throughout the posttransplantation period and is associated with the degree of renal graft function. The relationship between hemoglobin levels and glomerular filtration does not behave the same way as in the population with chronic kidney disease. The results of various studies show a high prevalence in

Improvement of kidney failure with fetal kidney precursor cell transplantation.

BACKGROUND: Current therapies for end-stage renal disease have severe limitations. Dialysis is only a temporary treatment and does not restore kidney function. Transplantation is limited by donor organ shortage and immune-related problems. Here, we show that the transplantation of fetal kidney precursor cells reconstitutes kidney tissues, reduces uremic symptoms, and provides life-saving metabolic support in kidney failure animal models. METHODS: Kidney failure was surgically induced by resecting kidneys, leaving approximately 1/6 of the total kidney mass (5/6 nephrectomy). Fetal kidney precu

Kidney and kidney/pancreas transplantation at Stanford University Medical Center.

The disparity between the supply of cadaveric donors and the demand for renal allografts continues to grow. We have taken a multifaceted approach to increase the allograft pool: 1. Spiral computed tomography to evaluate potential living kidney donors is safer, less invasive, less expensive and more time efficient and thus should encourage living organ donation. 2. Use of selected expanded criteria cadaveric donor kidneys (aged 60 or over, hypertensive) in size- and age-matched recipients have short-term function at 3 and 6 months comparable to standard cadaveric renal allografts. 3. Kidneys fr

Independent association between acute kidney injury and kidney function trajectory.

INTRODUCTION: It is now universally accepted that acute kidney injury (AKI) is not a limited and entirely reversible condition as many previously thought. However, the independent association between mild to moderate AKI and subsequent kidney function trajectory remains poorly characterized because prior studies have not adequately controlled for pre-existing differences between patients who did or did not develop AKI. METHODS: Here, we employed linear mixed-effects regression models to examine the association between an episode of AKI and subsequent trajectory of estimated glomerular filtrat

Diagnosing Mendelian Kidney Disease: Hidden Niches in the (Kidney) Genome.

A certain proportion of patients with Mendelian diseases are overlooked, although substantial technical advances in molecular genetics have been achieved. Massively parallel sequencing (MPS) increasingly identifies genetic variants of unknown significance, which may remain clinically unhelpful. Furthermore, difficult niches in the genome exist, which cannot be solved by standard MPS. In the reported family, autosomal dominant kidney disease leading to renal failure in middle adulthood runs through the maternal and paternal family. Comprehensive genetic analyses and customized functional evalua

Evidence-Based Cardiovascular-Kidney-Metabolic Therapies in Kidney Transplant Recipients.

Pivotal clinical trials have heralded several new classes of pharmacological agents focused on improving combined heart-kidney outcomes among patients with cardiovascular-kidney-metabolic syndrome. This review compiles the current evidence pertaining to the use of these novel, exciting classes of agents among kidney transplant recipients. Treatment with renin-angiotensin-aldosterone system inhibitors continues to remain the backbone of pharmacological management among kidney transplant recipients for several indications. Data from systematic reviews and meta-analyses support the use of sodium-

Predicting Simultaneous Heart Kidney Allocation and Posttransplant Adverse Kidney Outcomes.

INTRODUCTION: For individuals with both end-stage heart failure and end-stage kidney disease or persistent acute kidney injury (AKI), simultaneous heart-kidney transplantation (SHKT) emerges as a viable treatment option, potentially yielding superior survival rates compared with heart transplantation (HT) alone. Nevertheless, accurately forecasting kidney recovery following HT in patients with moderate kidney failure poses challenges, thereby complicating the decision-making process for SHKT. METHODS: This study employed a random forest (RF) machine learning algorithm, using 15 variables with

Dietary Patterns, Kidney Function, and Sarcopenia in Chronic Kidney Disease.

Sarcopenia is a condition characterized by the loss of muscle mass and function. It is a risk factor for adverse clinical outcomes, including falls, disability, and mortality in patients with chronic kidney disease (CKD). The progression of CKD leads to metabolic disturbances and pathophysiological changes. These alterations, such as metabolic acidosis, dysregulated muscle proteostasis, and excessive inflammation, contribute to accelerated muscle wasting, resulting in sarcopenia. Proper nutritional interventions are essential in the management of sarcopenia in patients with CKD. Appropriate di

Monogenic Kidney Diseases in Adults With Chronic Kidney Disease (CKD).

BACKGROUND: According to current evidence, every 10th to 11th adult with chronic kidney disease (CKD) has a monogenic disease of the kidney. METHODS: This review is based on reported studies in which molecular genetic diagnostic techniques were used to investigate monogenic kidney diseases in adults with CKD. The studies were identified by a selective literature search using predefined criteria. RESULTS: In 12 selected studies, diagnostic variants of 179 different genes were identified in 1467 out of 6607 study participants with CKD (22.2%). More than 60% of these variants affected 8 genes (

Kidney Stone Events after Kidney Transplant in the United States.

BACKGROUND: Kidney stone disease is common and can lead to complications such as AKI, urinary tract obstruction, and urosepsis. In kidney transplant recipients, complications from kidney stone events can also lead to rejection and allograft failure. There is limited information on the incidence of kidney stone events in transplant recipients. METHODS: We identified 83,535 patients from the United States Renal Data System who received their first kidney transplant between January 1, 2007, and December 31, 2018. We examined the incidence of kidney stone events and identified risk factors associ

Beyond the kidney biopsy: genomic approach to undetermined kidney diseases.

BACKGROUND: According to data from large national registries, almost 20%-25% of patients with end-stage kidney disease have an undetermined kidney disease (UKD). Recent data have shown that monogenic disease-causing variants are under-diagnosed. We performed exome sequencing (ES) on UKD patients in our center to improve the diagnosis rate. METHODS: ES was proposed in routine practice for patients with UKD including kidney biopsy from January 2019 to December 2021. Mutations were detected using a targeted bioinformatic customized kidney gene panel (675 genes). The pathogenicity was assessed us

Chronic Kidney Disease and Cognitive Impairment: The Kidney-Brain Axis.

BACKGROUND: Cognitive impairment, increasingly recognized as a major social burden, is commonly found in chronic kidney disease (CKD) patients. SUMMARY: Vascular damage, uremic toxicity, oxidative stress, and peripheral/central inflammation induced by CKD might be involved in brain lesions and ultimately result in cognitive decline. Uncovering the pathophysiology of CKD-associated cognitive impairment is important for early diagnosis and prevention, which undoubtedly prompts innovative pharmacological treatments. KEY MESSAGES: Here, we sequentially review the current understanding and advanc

Ferrostatin-1 modulates dysregulated kidney lipids in acute kidney injury.

Ferroptosis, a form of regulated necrosis characterized by peroxidation of lipids such as arachidonic acid-containing phosphatidylethanolamine (PE), contributes to the pathogenesis of acute kidney injury (AKI). We have characterized the kidney lipidome in an experimental nephrotoxic AKI induced in mice using folic acid and assessed the impact of the ferroptosis inhibitor Ferrostatin-1. Matrix-assisted laser desorption/ionization (MALDI) imaging mass spectrometry (IMS) was used to assess kidney lipidomics and it discriminated between glomeruli, medulla, and cortex in control kidneys, AKI kidney

Chronic Kidney Disease after Snake Envenomation Induced Acute Kidney Injury.

Snake bite is an important cause for acute kidney injury (AKI) in the tropics and the victims are often otherwise healthy young adults without conventional risk factors for chronic kidney disease (CKD). Available literature on long-term outcomes of AKI -snake envenomation is limited, with only two small retrospective studies with follow-up periods of two years or less. In a hospital-based prospective and retrospective follow-up study, all consecutive adult patients who developed AKI and discharged alive were recruited in the prospective group and for retrospective analysis, we retrieved medica
