Dislocation of the major joints of the lower extremities--hip, knee, and ankle--can occur due to motor-vehicle crashes, falls, and sports injuries. Hip dislocations are the most common, and they require emergent management to prevent avascular necrosis of the femoral head. Knee dislocations are uncommon but potentially dangerous injuries that can result in amputation due to the potential for missed secondary injury, especially if they are reduced spontaneously. Isolated ankle dislocations are relatively rare, as most ankle dislocations involve an associated fracture. This review presents an al

PURPOSE: Knee dislocations can cause significant damage to intra-articular knee structures, but currently there are limited data reporting articular cartilage and meniscal injuries in this setting. The purpose of this study is to (1) report the rate of concomitant intra-articular injuries at the time of multiligament reconstruction for knee dislocation, (2) determine whether the pattern of ligament injury is associated with the presence of chondral and meniscal injuries, and (3) assess the relationship between timing of surgery and incidence of chondral and meniscal injuries.
METHODS: The rec

BACKGROUND: Knee dislocations are uncommon but devastating orthopaedic injuries. Little is known about their frequency and the types of patients who are affected.
QUESTIONS/PURPOSES: Using a large national insurance database, we determined (1) the incidence of knee dislocation in patients with orthopaedic injuries and examined the incidence as a function of (2) year of diagnosis, (3) dislocation type (open versus closed, direction), and (4) patient demographic factors (sex, age).
METHODS: We searched the PearlDiver database, a national database of private insurance records consisting of 11 m

In addition to ultrasound and conventional x-ray, magnetic resonance imaging (MRI) plays an important role in the visualization of lesions of the knee. It allows the orthopaedic surgeon to safely detect ruptures of the cruciate ligaments, the meniscus, osteochondral lesions and other ligamentary structures such as the collateral ligaments and the medial patellofemoral ligament (MPFL). In patellar dislocations, risk factors for recurrent dislocations can be determined and the therapy can be adjusted accordingly. This articles aims to give the reader a comprehensive overview on current concepts a

The purpose of the present study was to investigate our 6-10 year results for knee dislocations with posterolateral corner (PLC) involvement, where a primary repair was performed laterally, the anterior cruciate ligament (ACL) reconstructed, but the posterior cruciate ligament (PCL) was left without surgery. Four consecutive patients with knee dislocation with complete rupture of the ACL, the PCL and the PLC were operated on by the same surgeon with similar technique. There were no other major injuries. We used strict inclusion criteria to get as homogenous population as possible. The ACL inju

Congenital dislocation of the knee is a rare disorder. Late presentation of congenital dislocation of the knee at an older age is a therapeutic challenge. A 12-year-old girl presented to us with congenital dislocation of the knee and with complaints of limp, short limb, and pain on weight bearing. Two-stage surgery was performed with quadricepsplasty followed by gradual distraction in the first stage and repeat quadricepsplasty, anterior capsular release, and open reduction in the second stage. The result was fair to good with a stable and painless knee on walking.

BACKGROUND: Congenital dislocation of the knee is a rare condition for which the treatment is difficult and remains controversial. For severe cases associated with neuromuscular disorders treatment has usually consisted of extensive surgical reconstruction. The purpose of this study is to assess the short-term results of a new method of treatment for this patient population that involves casting and less extensive surgery.
METHODS: We retrospectively reviewed the cases of 8 consecutive patients, 4 girls and 4 boys, with 16 congenitally dislocated knees that had been diagnosed and treated by a

While posterolateral knee dislocation is a rare entity, its identification has important implications. The invagination of skin seen in posterolateral knee dislocations is referred to as the "dimple sign" on physical exam. We report a case where the "dimple sign" was also demonstrated on magnetic resonance imaging. Failure to recognize this finding on imaging will render the knee irreducible, as a result of interposed tissues, until discovered by the orthopedic surgeon intraoperatively. The incarcerated medial capsule and medial collateral ligament could also be misinterpreted as torn meniscus

OBJECTIVE: To analyze the traumatic pathological characteristics of posterolateral dislocation of knee joints and its treatment.
METHODS: Nine cases of posterolateral dislocation of knee joint, 5 cases of fresh injuries (the fresh injury group) and 4 cases of old injuries ( the old injury group) were reviewed and analyzed. In the fresh injury group 4 cases failed in close reduction due to "buttonholing" through the medial joint, among them 3 cases underwent repair of the damaged ligaments. In the old injury group 2 cases underwent ACL and MCL repair only in acute stage, but re-dislocated. Of

AIM: We demonstrate the management and treatment of dislocation of the knee associated with vascular injury. The goal of the treatment is to avoid complications due to ischemia. The injured vessel can be repaired either by direct suture or by interposition of a saphenous vein graft. Capsule and ligaments should be reconstructed secondarily.
PATIENTS AND METHODS: The charts of ten patients treated in the Division of Traumatology of the University of Zurich between 1979 and 1996 have been retrospectively checked.
RESULTS: In eight of ten patients the injured vessel has been reconstructed with

Arthrograms and operative findings of 19 patients with 26 congenital dislocations of the knee (CDK) were reviewed to clarify the pathologic features, methods of treatment, and the optimum treatment for the patient with multiple joint deformities. The fundamental pathologic features in CDK involved a shortening of the quadriceps femoris tendon, a tight anterior articular capsule, and hypoplasia of the suprapatellar bursa. Valgus deformity of the knee, which was observed in half of the patients, was caused by the fragility of the medial components of the knee. This fragility was attributable to

One hundred fifteen patients with a unilateral knee dislocation underwent arteriography to examine the popliteal artery. The incidence of popliteal artery injury was 23% (27 patients). Clinically, 29 (25%) of the 115 patients had an abnormal ipsilateral pedal pulse and 23 (79%) of these 29 patients had an arteriographically identified popliteal artery injury. Twenty-two arteries were surgically repaired and one was treated without surgery. Eight-six patients had normal pulses; the arteriogram showed no abnormalities in 77, demonstrated spasm in five, and revealed an intimal flap in four. All 8

A retrospective survey of 135 posterior dislocations and fracture-dislocations of the hip was carried out in order to define the pattern of associated knee injuries. Thirty-five patients had sustained a significant injury to the knee, of which twenty-five were clearly attributable to a direct blow on the front of the knee (fractured patella, traumatic chondromalacia, fractures of femoral and tibial condyles) and ten were compatible with valgus, varus or rotational forces (medial, lateral and cruciate ligament tears). The second type of injury has not been widely recognised but it is important

Congenital knee dislocation (CKD) (genu recurvatum congenitum) is a rare neonatal orthopedic deformity characterized by hyperextension of the knee joint and potentially associated with intrauterine mechanical factors, quadriceps contracture, or syndromic disorders. Early recognition is essential, as prompt conservative management can prevent long-term disability and reduce the need for surgical intervention. The objective of this single case report is to describe a rare presentation of bilateral congenital knee dislocation managed successfully through early conservative treatment in a rural an

Acute knee dislocation is a rare orthopedic injury with an incidence of <0.02% annually. The authors discuss a case involving a vascular injury sustained from a mechanical fall at home causing compartment syndrome. The case illustrates known complications associated with knee dislocations: vascular injury and compartment syndrome as well as the high suspicion needed in certain patient populations, such as Down's syndrome. It highlights the importance of rapid diagnosis, a complete physical exam, and the need to involve consultants in a timely fashion. A high index of suspicion for vascular inj

Popliteal artery injury is a potentially limb-threatening complication of traumatic knee dislocation. We describe 2 such cases that had been treated in our unit over the last decade. The first one was a 23-year-old woman who injured her right knee during a long jump competition, and the second was a 27-year-old man who had a motorbike accident. Both suffered traumatic knee dislocation along with significant ligament and neurovascular injuries. In the first patient, the popliteal artery was found thrombosed due to intimal rupture and required thrombectomy and vein patch repair, whereas in the s

INTRODUCTION: At least 25% of knee dislocations are associated with common fibular nerve injury. Diagnosis is usually based on clinical and neurophysiological findings. We assessed the role of nerve ultrasound in common fibular nerve injury.
METHODS: Eight consecutive patients (6 men and 2 women, mean age 34 years) with knee luxation referred to our laboratory underwent clinical, neurophysiological, and ultrasound examination.
RESULTS: In all patients we observed a similar pattern: severe weakness (plegia or severe paresis); neurophysiological involvement of both fibular nerve branches; and

BACKGROUND: Popliteal artery injury can occur in up to one-third of patients with knee dislocation. Delay in the diagnosis of popliteal artery injury is the leading cause of amputation in this limb-threatening injury.
OBJECTIVES: To remind emergency physicians to consider popliteal artery injury in any pulseless lower extremity, even in cases of spontaneous reduction of the knee dislocation before evaluation by medical personnel.
CASE REPORT: We present a case of popliteal artery injury and occult knee dislocation in which the diagnoses were delayed for hours by incorrectly attributing the a
