PURPOSE: The purpose of this study was to report clinical results of open excision of extra-articular ganglion cysts around the knee joint combined with arthroscopic management of intra-articular pathologies if present.
MATERIALS AND METHODS: Of the total 107 cases of cystic lesions around the knee, 23 cases of extra-articular ganglion cysts were reviewed between January 2006 and July 2011. To perform surgery in the inaccessible area of the posterior compartment of the knee joint with the conventional anterior-posterior triangulation technique, we have used a direct posteromedial-posterolateral triangulation technique. This technique has been easy and safe in performing arthroscopic surgery in the posterior compartment, including synovectomy of 350 knees and removal of loose bodies in 12 knees. Using this method in addition to conventional anterior-posterior triangulation, the whole area of the posterior compartment could be covered for performing arthroscopic surgery.

Synthetic tubes are the second choice for below-knee bypass materials. The ring-reinforced thin-walled Goretex-PTFE graft is an approved material which was tested in a multicenter study. The primary 3 years patency for below-knee femoral-popliteal bypass was 64%, for crural bypass 39%. A total of 195 grafts have been implanted. Long-term patency depends very much on the run-off conditions (81% in good and 43% in bad cases). The over-all limb salvage rate was 80% after 3 years.

Computed tomography (CT) was used to examine the knee joint in 175 patients with various pathological conditions. In 118 of these patients a unique technique of study was employed. In 1,377 patients who had severe injuries about the knee joint, vascular complications were present in 28 (2%). Injuries to the popliteal artery or its branches present serious problems. Amputation was necessary in 13 patients, and 12 patients had a Volkmann's syndrome. In fractures of the lower end of the femur or upper end of the tibia, and in knee dislocations, the importance of early diagnosis of the acute ischemic syndrome cannot be overemphasized. Tibial plateau fractures produced the highest percentage of vascular complications and indicate immediate application of therapeutic measures.

In a 10 years period (1971 to 1980) 648 patients underwent 811 operations because of ligamentous lesions of the knee joint. 63.3% of them occurred due to sports activities. 41.2% of these patients were 15 to 30 years of age.
