Gene Transfer in Mycobacterium tuberculosis: Shuttle Phasmids to Enlightenment.

Infectious diseases have plagued humankind throughout history and have posed serious public health problems. Yet vaccines have eradicated smallpox and antibiotics have drastically decreased the mortality rate of many infectious agents. These remarkable successes in the control of infections came from knowing the causative agents of the diseases, followed by serendipitous discoveries of attenuated viruses and antibiotics. The discovery of DNA as genetic material and the understanding of how this information translates into specific phenotypes have changed the paradigm for developing new vaccine

Systematics and epidemiology of trichinella.

In this review, we describe the current knowledge on the systematics, ecology and epidemiology of Trichinella and trichinellosis, and the impact of recent research discoveries on the understanding of this zoonosis. The epidemiology of this zoonosis has experienced important changes over the past two decades, especially with regard to the importance of the sylvatic cycle and the sylvatic species. Outbreaks of trichinellosis due to Trichinella spiralis from domestic swine, while still frequent, increasingly are caused by other Trichinella spp. infecting hosts such as horses, dogs, wild boars, be

Vaccine Development against Zoonotic Hepatitis E Virus: Open Questions and Remaining Challenges.

Hepatitis E virus (HEV) is a fecal-orally transmitted foodborne viral pathogen that causes acute hepatitis in humans and is responsible for hepatitis E outbreaks worldwide. Since the discovery of HEV as a zoonotic agent, this virus has been isolated from a variety of hosts with an ever-expanding host range. Recently, a subunit HEV vaccine developed for the prevention of human disease was approved in China, but is not yet available to the rest of the world. Meanwhile, notable progress and knowledge has been made and revealed in recent years to better understand HEV biology and infection, includ

Clinical genetics of craniosynostosis.

PURPOSE OF REVIEW: When providing accurate clinical diagnosis and genetic counseling in craniosynostosis, the challenge is heightened by knowledge that etiology in any individual case may be entirely genetic, entirely environmental, or anything in between. This review will scope out how recent genetic discoveries from next-generation sequencing have impacted on the clinical genetic evaluation of craniosynostosis. RECENT FINDINGS: Survey of a 13-year birth cohort of patients treated at a single craniofacial unit demonstrates that a genetic cause of craniosynostosis can be identified in one qua

A RaPID Macrocyclic Peptide Inhibitor of ROR1/2 Strongly Reduces Proliferation of Diffuse Intrinsic Pontine Glioma Cells.

We report the de novo discovery of a cyclic peptide inhibitor of ROR1/2 from the RaPID (random nonstandard peptides integrated discovery) system. Multiple independent affinity selection campaigns converged on a highly conserved family of sequences exhibiting single-digit nanomolar binding affinities to ROR1. One peptide from this family containing three nonproteinogenic residues, including a key cyclic β-amino acid, displayed pronounced activity in cellular assays. Strikingly, this peptide induced potent growth inhibition across multiple Diffuse Intrinsic Pontine Glioma (DIPG) cell lines, wher

[Pendrin: its role in kidney function and hypertension].

Loss-of-function mutations of the pendrin gene are responsible for a pathology characterized by dysfunctions of thyroid and inner ear. However, it rapidly appeared after its discovery that the anion exchanger encoded by this gene plays a central role in kidneys. This brief review pictures the evolution of our knowledge regarding renal functions of pendrin, with a special emphasis to its recently identified roles in the maintenance of sodium homeostasis and blood pressure.

Multiple cardiac anomalies in an elderly man with Klinefelter's syndrome.

We report a case of an elderly man with Klinefelter's syndrome and multiple cardiac defects, including partial anomalous pulmonary venous connection, atrial septal defect and pulmonary arterial hypertension. To the best of our knowledge, partial anomalous pulmonary venous connection in association with Klinefelter's syndrome has never been described in the literature. This anomalous pulmonary connection was a serendipitous discovery following the malpositioning of a peripherally inserted central catheter.

Outlier analysis for accelerating clinical discovery: An augmented intelligence framework and a systematic review.

Clinical discoveries largely depend on dedicated clinicians and scientists to identify and pursue unique and unusual clinical encounters with patients and communicate these through case reports and case series. This process has remained essentially unchanged throughout the history of modern medicine. However, these traditional methods are inefficient, especially considering the modern-day availability of health-related data and the sophistication of computer processing. Outlier analysis has been used in various fields to uncover unique observations, including fraud detection in finance and qua

DReAmocracy: A Method to Capitalise on Prior Drug Discovery Efforts to Highlight Candidate Drugs for Repurposing.

In the area of drug research, several computational drug repurposing studies have highlighted candidate repurposed drugs, as well as clinical trial studies that have tested/are testing drugs in different phases. To the best of our knowledge, the aggregation of the proposed lists of drugs by previous studies has not been extensively exploited towards generating a dynamic reference matrix with enhanced resolution. To fill this knowledge gap, we performed weight-modulated majority voting of the modes of action, initial indications and targeted pathways of the drugs in a well-known repository, nam

Historical concepts and contemporary perspectives of dental caries-a tribute to Henrik Dam (1895-1976).

Dental caries is a multifactorial disease, which is the result of a complex interplay between the diet, the host, the saliva, and dental biofilms. Although the prevalence of dental caries has decreased dramatically since 1950 in many countries, it continues to be one of the most common health conditions globally. The aim of the present review is to summarize the investigations on dental caries performed by the late Noble prize winner Henrik Dam and his colleagues in the middle of the 20th century, and to relate the knowledge and state of the art at the time to current concepts on dental caries

Gilbert's syndrome revisited.

Gilbert's syndrome, also known as benign hyperbilirubinaemia, was described more than 100 years ago. It has usually been considered a physiological abnormality characterised by a mild elevation of the systemic level of unconjugated bilirubin, in the absence of any underlying liver or overt haemolytic disease. However, since the re-discovery of the potent antioxidant effects of bilirubin in the late 1980s, as well as multiple intracellular signalling pathways affected by bilirubin, an ever-increasing body of evidence suggests that individuals with Gilbert's syndrome may benefit from the mild hy

3D-QSAR approaches in drug design: perspectives to generate reliable CoMFA models.

Drug discovery is mostly guided by innovative and knowledge by the application of experimental and computational approaches. Quantitative structure-activity relationships (QSAR) have a critical task in the discovery and optimization of lead compounds, thereby contributing to the development of new chemical entities. 3D-QSAR methods use the information of the tridimensional molecular structure of ligands and can be applied to elucidate the relationships between 3D molecular interactions and their measured biological property, therefore, providing a rational approach for the development of new p

The biology and clinical impact of genetic lesions in myeloid malignancies.

A longstanding endeavor to define the genetic lesions that drive myeloid malignances has stimulated a period of remarkable discovery. Enabled by technological advances that have sharply decreased the cost of DNA sequencing, the full compendium of common, recurrent somatic mutations in the coding genome of myeloid malignancies is nearly complete. As the focus of genetic discovery shifts to the noncoding genome, renewed attention is being applied to the clinical and biological implications of recent genomic advances. Although the potential for this newfound knowledge to influence the care of pat

Ex Vivo Plasmodium malariae Culture Method for Antimalarial Drugs Screen in the Field.

In vitro and ex vivo cultivation of Plasmodium (P) falciparum has facilitated active research into the malaria parasite toward the quest for basic knowledge and the discovery of effective drug treatments. Such a drug discovery program is currently difficult for P. malariae simply because of the absence of in vitro and ex vivo cultivation system for its asexual blood stages supporting antimalarial evaluation. Despite availability of artemisinin combination therapies effective on P. falciparum, P. malariae is being increasingly detected in malaria endemic countries. P. malariae is responsible fo

Determination of antiprotozoal drug mechanisms by metabolomics approaches.

The discovery, development and optimal utilization of pharmaceuticals can be greatly enhanced by knowledge of their modes of action. However, many drugs currently on the market act by unknown mechanisms. Untargeted metabolomics offers the potential to discover modes of action for drugs that perturb cellular metabolism. Development of high resolution LC-MS methods and improved data analysis software now allows rapid detection of drug-induced changes to cellular metabolism in an untargeted manner. Several studies have demonstrated the ability of untargeted metabolomics to provide unbiased target

Nicotinamide adenine dinucleotide metabolism as an attractive target for drug discovery.

Nicotinamide adenine dinucleotide (NAD(+)) has crucial roles in many cellular processes, both as a coenzyme for redox reactions and as a substrate to donate ADP-ribose units. Enzymes involved in NAD(+) metabolism are attractive targets for drug discovery against a variety of human diseases, including cancer, multiple sclerosis, neurodegeneration and Huntington's disease. A small-molecule inhibitor of nicotinamide phosphoribosyltransferase, an enzyme in the salvage pathway of NAD(+) biosynthesis, is presently in clinical trials against cancer. An analog of a kynurenine pathway intermediate is e

Molecular and cellular biology of the inner ear. The next frontier.

Molecular and cellular biology is the study of cellular ultrastructure and function. Current research in immune regulation, delineation of receptor control function (signal translation at the cellular level), and nucleic acid manipulation (genetic engineering) illustrates fundamental relationships among biochemistry, immunology, and molecular and cellular biology. Our knowledge already includes significant discoveries in membrane permeability, ionic gradients, and mediators of homeostasis. To these are added newer discoveries in bone metabolism, vascular anatomy and physiology, trace element d

A re-appraisal of the conventional history of antibiosis and Penicillin.

The popular perception of the history of antibiosis and penicillin is that Alexander Fleming was the sole researcher on penicillin. The literature, however, has documentation of preceding persons who reported definitively on these topics, from the late 19th century. Divergent reports on "firsts" in the discovery of antimicrobial activity of Penicillium and on the use of penicillin as a therapeutic agent, are present. This review adds knowledge from diverse sources, and restores historical priorities to the conventional story of Penicillin.

Fresh fields and pathways new: recent genetic insights into cardiac malformation.

Cardiovascular malformations are the most common type of birth defect. Currently, only a fraction of cases have associated causative factors and little is known about the aetiology of the rest. Despite this, our understanding of normal and abnormal heart development continues to grow, a number of recent discoveries even challenging long-held concepts. In this review, we highlight some of this new knowledge, emphasising aspects that may be of interest to the clinician.
