Attention re-alignment in multimodal large language models via intermediate-layer guidance.

Multimodal large language models (MLLMs) have achieved impressive performance in understanding and describing visual content, setting new state-of-the-art results on a variety of visual question answering (VQA) benchmarks. However, during decoding, these models often fail to attend to fine-grained visual details in the input image. Our analysis of intermediate attention layers reveals that MLLMs are not inherently incapable of perceiving target objects; rather, attention to visual details becomes diluted in deeper layers due to the dominance of language priors. To address this limitation, we p

Real-world performance of open-source large language models in diabetes diagnosis.

BACKGROUND: This study aimed to evaluate the performance of diverse open-source large language models (LLMs) in diagnosing diabetes subtypes and comorbidities from unstructured clinical text, assessing the impact of model characteristics, prompting, and language. METHODS: We conducted a retrospective analysis of 11,329 adult diabetes patients from a large Chinese tertiary center (2010-2020). Various open-source LLMs were tested using four prompting strategies in English and Chinese. Primary outcomes were F1-scores for multi-class diabetes subtyping and binary classification of diabetic kidney

Exploring teacher-student interaction through multimodal large language models: an empirical investigation.

Teacher–student interaction is central to classroom learning, yet traditional observation and machine-learning approaches often remain inefficient and subjective. This study explores the use of multimodal large language models (MLLMs) for systematic analysis of classroom dynamics. We fine-tuned VisualGLM-6B on 2,380 annotated images from 30 classroom videos, covering five interaction types: guided, collaborative, questioning, independent, and exhibitive. LoRA-based fine-tuning combined with prompt engineering was employed to enhance interpretability and domain-specific accuracy. Model performa

Artificial intelligence in laboratory medicine: From machine learning to large language models.

The integration of artificial intelligence (AI) into laboratory medicine has undergone a remarkable evolution over the past decade, shifting from traditional machine learning (ML) applied to structured laboratory data to the recent emergence of large language models (LLMs) capable of processing unstructured clinical text, interpreting complex laboratory findings, and supporting clinical decision-making. This narrative review traces that progression and critically compares four main AI paradigms accessible to laboratory professionals: conventional ML, deep learning, general-purpose LLMs, and la

Identifying and timing patient outcomes in clinician notes using large language models.

BACKGROUND: Key challenges in leveraging unstructured clinician notes for predictive models include identifying and timing patient outcomes. To address these challenges we applied large language models (LLMs) to identify and temporally localize patient outcomes in clinician notes, and evaluated whether this contextual data enhances predictive modeling for conditions like sepsis. METHODOLOGY: We applied the Medical Concept Annotation Tool (MedCAT) and two LLMs, Meta-Llama-3.1-8B and BioMistral 7B, to clinician notes in the Medical Information Mart for Intensive Care version III to identify and

Comparing supervised machine learning and large language models in title-abstract screening.

BACKGROUND: Systematic reviews require reviewers to decide on the eligibility of large numbers of articles derived from database searches. To accelerate review conduct while continuously more literature gets published, past studies proposed automating the title/abstract-screening step by either supervised machine learning or large language models. Because prior studies mainly compared results within the same model family, we directly compared common TF-IDF-based supervised baselines and a zero-shot, criteria-prompted, and open-weight large language model on the same data to discuss whether, an

Current Artificial Intelligence Large Language Models Exhibit Sycophantic Behavior in Orthopaedic Contexts.

BACKGROUND: The use of large language models (LLMs) is increasingly common. However, LLMs may exhibit sycophancy, echoing users' beliefs while avoiding contradiction. In the present study, we describe sycophancy in general-purpose LLMs when applied to orthopaedic contexts. METHODS: We investigated sycophancy in 2 general-purpose LLMs. We evaluated performance on 3 tasks: (1) accuracy on benchmark answering: LLMs were tested on validated benchmark orthopaedic questions, with correct and incorrect cues, and the change in accuracy and sycophancy error rate were determined; (2) user belief agreem

Development of a benchmarking dataset for symptom detection using large language models.

OBJECTIVES: To develop a pipeline for evaluating large language models (LLMs) on the task of capturing symptoms from clinical encounters. MATERIALS AND METHODS: We created a gold standard dataset of symptom annotations from simulated doctor-patient encounter excerpts (264 encounters; 16 symptoms; double-coded and adjudicated). Nine different LLMs from 4 vendors (OpenAI, Meta, DeepSeek, Moonshot AI) were used as examples to test our evaluation pipeline; outputs were assessed for correct structure and symptom information. RESULTS: Of 3085 excerpts, 2087 (68%) contained symptoms. Pain, cough, a

Fragile preferences: A deep dive into order effects in large language models.

Large language models (LLMs) are increasingly deployed in decision-support systems for high-stakes domains such as hiring and university admissions, where choices often involve selecting among competing alternatives. While prior work has noted position biases in LLM-driven comparisons, these biases have not been systematically analyzed or linked to underlying preference structures. We present the first comprehensive study of position biases across multiple LLMs and two distinct domains: resume comparisons, representing a realistic high-stakes context, and color selection, which isolates positi

Evaluating the diagnostic performance of multimodal large language models in thyroid cytology.

Thyroid fine-needle aspiration (FNA) cytology is a widely used, cost-effective method for evaluating thyroid nodules. However, interpretation of thyroid FNA smears remains challenging, particularly in settings with limited access to experienced cytopathologists. Multimodal large language models (LLMs) have shown potential in static image interpretation in histopathology and may serve as supportive tools when expert review is unavailable. Nevertheless, data regarding their application in cytopathology, including thyroid cytology, remain limited. In this study, we evaluated the diagnostic accura
