Effect of controlled hyperventilation on the pressor response to laryngoscopy and tracheal intubation.

Pressor response to laryngoscopy and tracheal intubation includes rises in blood pressure and heart rate. This response may be harmful in the presence of cerebral or myocardial diseases. Although different preventive measures have been developed the choice of the agent or method has not been defined clearly. Hypocapnia is commonly used in anesthesia practice for different indications. It depresses the cardiovascular system and lowers the cardiac output. This study investigated the effect of controlled hyperventilation on the pressor response to laryngoscopy and tracheal intubation in three gro

Second dose thiopentone attenuates the haemodynamic response to laryngoscopy and intubation.

The study was undertaken to evaluate the effectiveness of large (divided) thiopentone dosage on the peripheral haemodynamic response to laryngoscopy and intubation. Seventy-six (76) patient aged 18 to 67 years were sequentially assigned to either the second dose thiopentone group (n = 36) or the control group (n = 40). The first group had 4 mg.kg-1 thiopentone for induction of anaesthesia, then 1.5 mg.kg-1 suxamethonium chloride for muscle relaxation and a second dose of thiropentone (4 mg.kg-1) just before laryngoscopy and intubation. The control group had thiopentone 4 mg.kg-1 for induction

[Unilateral paralysis of the vocal fold: correlation between laryngoscopy and electromyography].

From a clinical point of view, unilateral vocal fold paresis means that one of the vocal folds shows reduced mobility. Etiology may be mechanical or supranuclear, but in most cases it is a partial peripheral neurogenic lesion in the muscles innervated by one of the recurrent laryngeal nerves. On the one hand, a retrospective analysis was made of 71 cases with laryngoscopically reduced mobility of one vocal fold: 58 patients had electromyographic signs of partial denervation; 13 had normal electromyographic activity. Possible explanations are mechanical factors, paradoxical reinnervation, or ce

A novel inhaled anesthesia technique for concurrent direct laryngoscopy in rats.

The current anesthetic standard for laryngoscopy in rats utilizes injectable intraperitoneal anesthesia. Injectable anesthesia is suboptimal for short procedures due to variability in anesthesia duration and anesthetic side effects. Conversely, inhalational gas anesthesia offers precise titration with a rapid onset and offset. However, its use during laryngoscopy has not been documented due to existing administration techniques obstructing direct visualization of the larynx. The technique described here allows real-time visualization of the rat larynx with concurrent administration of inhaled

Investigation of exertional dyspnoea by cardiopulmonary exercise testing with continuous laryngoscopy.

OBJECTIVES: Abnormal breathlessness at maximal exercise may be caused by a range of conditions, including exercise-induced bronchospasm, breathing pattern disorder, or exercise-induced laryngeal obstruction. These three disorders may not be detected on standard cardiopulmonary exercise testing. The aim of this study was to describe diagnostic outcomes of an expanded protocol during cardiopulmonary exercise testing. DESIGN: Retrospective cohort study. METHODS: Patients presenting with abnormal breathlessness on maximal exercise underwent continuous laryngoscopy with cardiopulmonary exercise t

A Predictor of Difficult Airway: The Tasli Classification in Transnasal Flexible Laryngoscopy.

INTRODUCTION: Difficult airway is defined as difficulty or failure in one or more steps in upper airway management. Evaluation of the upper airway with physical examination methods and endoscopic devices is crucial in predicting difficult airway. The aim of this study was to evaluate bedside tests, Cormack Lehane (CL) and Tasli Classification (TC) scores of the patients and it was aimed to reveal the role of TC which will be performed preoperatively as a predictor of difficult tracheal intubation (DTI). METHODS: The study included a total of 98 patients who underwent surgical treatment under

A new simple score for prediction of difficult laryngoscopy: the EL.GA+ score.

BACKGROUND: Preoperative airway assessment plays a key role in the context of difficult airway management. Several scores have been proposed to predict difficult intubation including the el-Ganzouri index (EL.GA). Anatomical parameters such as the opening of the mouth or the circumference of the neck (which currently is not usually evaluated) are used to predict difficult intubation. The nutritional status of super-morbid obesity (body mass index [BMI] > 50 kg m-2) is a recognised risk factor for difficult intubation. METHODS: This is a single-centre, retrospective, observational study whose

Radiologic indicators for prediction of difficult laryngoscopy in patients with cervical spondylosis.

BACKGROUND: We identified the most useful variables for prediction of difficult laryngoscopy in patients with cervical spondylosis according to physical indicators and preoperative skeletal X-ray and soft tissue MRI measurements. We hypothesized that there was a closer association between difficult laryngoscopy and radiologic indicators. METHODS: We randomly enroled 315 patients undergoing elective cervical spine surgery and analysed the radiological and physical data in predicting difficult laryngoscopy. RESULTS: We identified five variables that were most useful in predicting difficult lar

Laryngoscopy evaluation protocol for the differentiation of essential and dystonic voice tremor.

INTRODUCTION: Although syndromes that cause voice tremor have singular characteristics, the differential diagnosis of these diseases is a challenge because of the overlap of the existing signs and symptoms. OBJECTIVE: To develop a task-specific protocol to assess voice tremor by means of nasofibrolaryngoscopy and to identify those tasks that can distinguish between essential and dystonic tremor syndromes. METHODS: Cross-sectional study. The transnasal fiberoptic laryngoscopy protocol, which consisted of the assessment of palate, pharynx and larynx tremor during the performance of several voc

Airway evaluation by indirect laryngoscopy in patients with lingual tonsillar hypertrophy.

INTRODUCTION AND OBJECTIVES: Prevalence of the lingual tonsillar hypertrophy is unknown but it is believed that its presence is associated with the difficult airway. To investigate this, indirect laryngoscopy was performed on patients in the preoperative evaluation and this pathology was diagnosed. The relationship with difficulty of viewing the larynx, intubation and ventilation, under general anaesthesia and using direct laryngoscopy, was then studied. METHODS: We performed the demographic variable checks and tests for predicting difficult intubation (mouth opening, thyromental distance, ce

Prediction of difficult laryngoscopy in obstetric patients scheduled for Caesarean delivery.

BACKGROUND AND OBJECTIVE: Failed intubation is an important cause of anaesthetic-related maternal mortality. The purpose of this study was to determine the ability to predict difficult visualization of the larynx from the following preoperative airway predictive indices, in isolation and combination: modified Mallampati test, the ratio of height to thyromental distance and the Upper-Lip-Bite test. METHODS: We collected data on 400 consecutive parturients scheduled for elective Caesarean delivery under general anaesthesia requiring endotracheal intubation and then assessed all three factors be

Indirect fluorescence laryngoscopy in the diagnosis of precancerous and cancerous laryngeal lesions.

Indirect fluorescence endoscopy of the larynx has proven to facilitate the detection and delineation of precancerous and cancerous lesion. The different methods are easy to handle and can be performed on an outpatient basis. Early diagnosis of laryngeal cancer and its precursor lesions is simplified. The aim of the present study is to compare indirect autofluorescence laryngoscopy to 5-ALA-induced PPIX fluorescence laryngoscopy. In a prospective study, 56 patients with suspected precancerous or cancerous lesions were primarily investigated by indirect autofluorescence laryngoscopy. In a second

A comparison of direct laryngoscopy and jaw thrust to aid fibreoptic intubation.

We compared two manoeuvres, jaw thrust and laryngoscopy, to open the airway during fibreoptic intubation in 50 patients after induction of anaesthesia in a crossover study. Patients were randomly allocated to receive either jaw thrust or conventional Macintosh laryngoscopy first. Airway clearance was assessed at both the soft palate and the epiglottis. Direct laryngoscopy provided significantly better airway clearance at the level of the soft palate than jaw thrust (44 (88%) vs 31 (62%), respectively; p = 0.002). At the level of the larynx, airway clearance was equally good in both groups (45

Laryngoscopy and pharyngeal pH are complementary in the diagnosis of gastroesophageal-laryngeal reflux.

Pharyngeal pH monitoring and laryngoscopy are routinely used to diagnose gastroesophageal-laryngeal reflux as a cause of respiratory symptoms. Although their use seems intuitive, their ultimate diagnostic value is yet to be defined. We studied 10 asymptomatic (control) subjects and 76 patients with respiratory symptoms. Both patients and control subjects were given a symptom questionnaire. Each underwent direct laryngoscopy using the reflux finding score (RFS) to grade laryngeal injury, esophageal manometry, and 24-hour esophagopharyngeal pH monitoring. The patients were then classified as RFS
