Early potential safety signals for gliptins and gliflozins using real-world pharmacy data compared to spontaneous reporting.

BACKGROUND: Dipeptidyl peptidase 4 inhibitors (DPP4i, gliptins) and sodium-glucose co-transporter 2 inhibitors (SGLT2i, gliflozins) are two oral antidiabetic drug classes that have been widely used in recent decades. Their pharmacological actions have led to the identification of new adverse drug reactions and contraindications. In this context, ambulatory real-world data (RWD) analysis has the potential to detect early signals of suspected adverse drug reactions (ADR) that complement or anticipate risk events. METHODS: We conducted an exploratory pharmacovigilance study based on a multicentr

Optimization of Albuminuria-Lowering Treatment in Diabetes by Crossover Rotation to Four Different Drug Classes: A Randomized Crossover Trial.

OBJECTIVE: Renin-angiotensin system (RAS) inhibitors decrease the urinary albumin to creatinine ratio (UACR) but are ineffective in up to 40% of patients. We hypothesized that rotation through different drug classes overcomes RAS inhibitor resistance and tested this in a randomized crossover trial. RESEARCH DESIGN AND METHODS: We assigned 26 adults with type 1 diabetes and 37 with type 2 diabetes and UACR between 30 and 500 mg/g and estimated glomerular filtration rate >45 mL/min/1.73 m2 to 4-week treatment periods with telmisartan 80 mg, empagliflozin 10 mg, linagliptin 5 mg, and baricitinib

Distinct proteomic signatures of urinary extracellular vesicles link to albuminuria and treatment responses in diabetic kidney disease.

INTRODUCTION: Urinary extracellular vesicles (uEVs) offer a noninvasive approach to gain pathophysiological insight into kidney disease and treatment effects. Our study aimed to investigate the impact of albuminuria and antialbuminuric therapies on the uEV proteome in diabetic kidney disease. METHODS: uEVs were isolated from urine samples of 20 patients with diabetes and albuminuria (age 64 years, estimated glomerular filtration rate 85 ml/min per 1.73 m2, urinary albumin creatinine ratio 130 mg/g) who participated in a crossover trial. Participants received telmisartan, empagliflozin, linagl

Treating hyperglycaemia in a patient with maternally inherited diabetes and deafness with an inhibitor of dipeptidyl peptidase-4: a case report and two-year follow-up.

Maternally inherited diabetes and deafness (MIDD) is a mitochondrial disorder with diverse characteristics, which make early diagnosis difficult. We report a case of 32-year-old woman with diabetes who was admitted due to weight loss and poor glycemic control. She had a history of gestational diabetes at age 26. Pancreatic function was evaluated by oral glucose tolerance. An ophthalmologic examination detected conjunctivitis and refractive errors and hearing tests were normal. The patient had a family of diabetes. Then we tested the patient and her first-degree relatives with a confirmed genet

Peroxisome proliferator-activated receptor-alpha activation and dipeptidyl peptidase-4 inhibition target dysbiosis to treat fatty liver in obese mice.

BACKGROUND: Obesity and comorbidities onset encompass gut dysbiosis, altered intestinal permeability, and endotoxemia. Treatments that target gut dysbiosis can cope with obesity and nonalcoholic fatty liver disease (NAFLD) management. Peroxisome proliferator-activated receptor (PPAR)-alpha activation and dipeptidyl-peptidase-4 (DPP-4) inhibition alleviate NAFLD, but the mechanism may involve gut microbiota modulation and merits further investigation. AIM: To address the effects of PPAR-alpha activation and DPP-4 inhibition (isolated or combined) upon the gut-liver axis, emphasizing inflammato

Evaluation of Risk of Bullous Pemphigoid With Initiation of Dipeptidyl Peptidase-4 Inhibitor vs Second-generation Sulfonylurea.

IMPORTANCE: Despite several recent reports on the elevated risk of bullous pemphigoid in patients with type 2 diabetes treated with dipeptidyl peptidase-4 (DPP-4) inhibitors, evidence on the absolute risk and comparative safety against other antidiabetics is limited. OBJECTIVE: To characterize the incidence rate of bullous pemphigoid associated with DPP-4 inhibitor use compared with second-generation sulfonylureas. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used data from 2 large commercial insurance claims databases (Optum Clinformatics Data Mart from October 17, 2006, to December

Eczema and Petechiae in an Infant.

BACKGROUNDDespite growing preclinical evidence that glucagon-like peptide1 receptor agonists (GLP-1RAs) could be repurposed to treat alcohol use disorder (AUD), clinical evidence is scarce. Additionally, the potential impact of dipeptidyl peptidase-4 inhibitors (DPP-4Is) on alcohol intake is largely unknown.METHODSWe conducted a large cohort study using 2008-2023 electronic health records data from the U.S. Department of Veterans Affairs. Changes in Alcohol Use Disorders Identification Test-Consumption (AUDIT-C) scores were compared between propensity-score-matched GLP-1RA recipients, DPP-4I r

Dipeptidyl peptidase-4 inhibitors and bullous pemphigoid: A systematic review and adjusted meta-analysis.

BACKGROUND/OBJECTIVE: There have been a number of case reports and small clinical series reporting the potential association between dipeptidyl peptidase-4 inhibitors (DPPIs) for diabetes and the onset of bullous pemphigoid (BP). The aim of this study was to assess the association between DPPI use and BP, and whether this varied according to DPPI type. METHODS: We performed a systematic review and meta-analysis according to PRISMA guidelines. We identified five studies with cases and controls. We performed unadjusted and adjusted meta-analyses to assess the potential association. RESULTS: Ad

Case report: Epidermolysis bullosa acquisita following dipeptidyl peptidase-4 inhibitor therapy and complicated by immune thrombocytopenic purpura.

Epidermolysis bullosa acquisita (EBA) is a rare autoimmune blistering disease caused by autoantibodies to type VII collagen. While dipeptidyl peptidase-4 (DPP-4) inhibitors are established triggers for bullous pemphigoid (BP), their association with EBA has not been reported. A 68-year-old Japanese man with type 2 diabetes and chronic renal dysfunction, treated with linagliptin, developed widespread tense blisters with mucosal involvement. Histopathology and immunological studies confirmed EBA. Despite corticosteroids, cyclosporine, pulse therapy, and intravenous immunoglobulin, the disease re

Evaluation of LG's anticancer acti̇vi̇ty on human gastric cancer cell line HGC-27 via molecular docking and gene expression analysis.

Gastric cancer (GC) is one of the most prevalent gastrointestinal malignancies and continues to pose a significant global health burden. Repurposing clinically approved drugs is a cost-effective strategy for identifying new anticancer agents. In this study, the anticancer effects of linagliptin (LG), a dipeptidyl peptidase-4 (DPP-4) inhibitor, were evaluated in the human gastric cancer cell line HGC-27. LG significantly reduced cell viability in a dose-dependent manner, with a half-maximal inhibitory concentration (IC50) value of 50.29 μM, and increased apoptotic cell death as confirmed by flo

PACAP in Parkinson's disease: A multifunctional neuropeptide at the crossroad of autophagy, inflammation, and metabolic repurposing.

Parkinson's disease (PD) is a progressive neurodegenerative disorder characterized by the accumulation of α-synuclein, mitochondrial dysfunction, and chronic neuroinflammation. In this complex pathology, pituitary adenylate cyclase-activating polypeptide (PACAP) has become a crucial neuroprotective regulator; however, its signaling pathways are markedly impaired in Parkinson's disease (PD). This review consolidates the increasing evidence that PACAP counteracts the pathogenesis of PD through multiple mechanisms: restoring autophagic flux, diminishing oxidative stress, altering the balance from
