Endogenous nitric oxide attenuates neutrally mediated cutaneous vasoconstriction.

Cutaneous vasoconstrictor responsiveness may be impaired by substance(s) directly or indirectly responsible for cutaneous active vasodilatation. In this study, we tested the hypothesis that endogenous nitric oxide (NO) attenuates the reduction in cutaneous vascular conductance (CVC) during an orthostatic challenge combined with whole-body heating, as well as during whole-body cooling. In protocol 1, healthy subjects were pretreated with an intradermal injection of botulinum toxin A (BTX) to block the release of neurotransmitters from nerves responsible for cutaneous active vasodilatation. Hypohydration Increases Heart Rate Despite Preserved Hemodynamic Compensation During Blood Flow Restricted Exercise.

During blood flow restriction (BFR) exercise, blood becomes sequestered in the active limbs due to impeded venous return, contributing to central hypovolemia. Dehydration also reduces blood volume and can exacerbate the ability of the cardiovascular (CV) system to defend against CV stressors, such as exercise and orthostatic stress. As BFR becomes more widely used, its application in individuals who may be suboptimally hydrated underscores the need to understand how hydration status influences cardiovascular strain. We tested whether hypohydration affects the CV response to BFR exercise and it

Prolonged head down bed rest-induced inactivity impairs tonic autonomic regulation while sparing oscillatory cardiovascular rhythms in healthy humans.

BACKGROUND: Physical inactivity represents a major risk for cardiovascular disorders, such as hypertension, myocardial infarction or sudden death; however, underlying mechanisms are not clearly elucidated. Clinical and epidemiological investigations suggest, beyond molecular changes, the possibility of an induced impairment in autonomic cardiovascular regulation. However, this hypothesis has not been tested directly. METHODS: Accordingly, we planned a study with noninvasive, minimally intrusive, techniques on healthy volunteers. Participants were maintained for 90 days strictly in bed, 24 h a

Investigation of the Optimal Pressure for Skin Grafting Using Negative Pressure Wound Therapy.

BACKGROUND: Recent studies suggest that negative pressure wound therapy (NPWT) is more effective than tie-over bolsters for skin graft fixation, improving graft take rates and reducing operation times. However, the relationship between negative pressure and compression pressure remains unclear. This study aims to explore that relationship. METHODS: Study 1 investigated compression pressure at the thigh, anterior tibia, and posterior lower leg under varying negative pressure settings. Study 2 measured compression pressure at the anterior ankle joint and knee joint during flexion. Study 3 compa

Capacity to Compensate for Central Hypovolemia and Effects of Menstrual Cycle Phases.

BACKGROUND: Tolerance to central hypovolemia is dictated by exhaustion of the physiological capacity to compensate called the compensatory reserve. Such physiological compromise can have detrimental impact on performance in aerospace environments as well as survival from hemorrhage on the battlefield. We induced central hypovolemia using progressively stepwise lower body negative pressure (LBNP) in women during various phases of the menstrual cycle to test the hypothesis that similar tolerance across all menstrual cycle phases would be reflected by similar changes in compensatory reserve.METHO

Evaluation of choroidal perfusion changes in obese patients: ocular effects of insulin resistance.

PURPOSE: We aimed to evaluate choroidal per-fusion changes in obese patients using optical coherence tomography and dynamic contour tonometry, to determine whether these changes are associated with body mass index, and to assess the ocular effects of insulin resistance. METHODS: We retrospectively evaluated the data of 32 obese patients with body mass index >30 kg/m2 and 45 healthy control individuals. Intraocular pressure and ocular pulse amplitude values of the patients were measured using dynamic contour tonometry, and the mean choroidal thickness was measured using enhanced depth imaging

Orthostatic hypotension in aging humans.

We tested the hypothesis that hypotension occurred in older adults at the onset of orthostatic challenge as a result of vagal dysfunction. Responses of heart rate (HR) and mean arterial pressure (MAP) were compared between 10 healthy older and younger adults during onset and sustained lower body negative pressure (LBNP). A younger group was also assessed after blockade of the parasympathetic nervous system with the use of atropine or glycopyrrolate and after blockade of the beta(1)-adrenoceptor by use of metoprolol. Baseline HR (older vs. younger: 59 +/- 4 vs. 54 +/- 1 beats/min) and MAP (83 +

Noninvasive indicators of intracranial pressure before, during, and after long-duration spaceflight.

Weightlessness induces a cephalad shift of blood and cerebrospinal fluid that may increase intracranial pressure (ICP) during spaceflight, whereas lower body negative pressure (LBNP) may provide an opportunity to caudally redistribute fluids and lower ICP. To investigate the effects of spaceflight and LBNP on noninvasive indicators of ICP (nICP), we studied 13 crewmembers before and after spaceflight in seated, supine, and 15° head-down tilt postures, and at ∼45 and ∼150 days of spaceflight with and without 25 mmHg LBNP. We used four techniques to quantify nICP: cerebral and cochlear fluid pre

Characteristics of sustained attention impairment in individuals under simulated positive acceleration stress: separation effect of early and late stage cognitive processing.

OBJECTIVE: This study aimed to investigate the mechanisms underlying impaired sustained attention under positive acceleration stress. METHODS: We used a lower body negative pressure model to simulate positive acceleration stress and employed electroencephalography (EEG) to observe changes in brain function. RESULTS: EEG results indicated that sustained attention exhibited a dissociation effect between early and late stages of processing following positive acceleration stress. The latency of the P2 component induced by non-target stimuli at the Cz electrode was significantly reduced under low

Autonomic heart rate control at rest and during unloading of the right ventricle in repaired tetralogy of Fallot in adolescents.

Arrhythmias in patients with repaired tetralogy of Fallot (ToF) might be due in part to altered autonomic heart rate control caused by altered right ventricle hemodynamics. This study investigated autonomic heart rate control in adolescents with ToF at rest and during unloading of the right ventricle. A total of 17 patients with ToF and 56 healthy controls aged 12 to 18 years underwent orthostatic stress with lower body negative pressure of -20 mm Hg. Heart rate, blood pressure, and stroke volume were recorded noninvasively. Indices of heart rate variability were computed in time and frequency

Effects of angiotensin II receptor antagonism on the renal hemodynamic response to cardiovascular stress.

BACKGROUND: To elucidate the effect of the angiotensin type 1 (AT1) receptor antagonist (AT1RA) eprosartan (E) on renal hemodynamics in normotensive and borderline hypertensive subjects, we investigated the hormonal and renal hemodynamic responses during cardiopulmonary stress testing. METHODS: In a prospective, double-blind, randomized, placebo-controlled crossover study, the effects of E on renal plasma flow (RPF), renal blood flow (RBF), glomerular filtration rate (GFR), and the concentration of angiotensin II (Ang II) levels were measured with the subjects at rest and during perturbation

Vasopressin release during orthostatic hypotension after cardiac transplantation.

At the time of cardiac transplantation all nerves from the donor ventricles are cut. These nerves may regrow, but there is no method of measuring any regrowth. Arginine vasopressin (AVP) release was studied during hypotension induced by head-up tilt and lower body negative pressure (LBNP) in transplant recipients and in normal controls. Subjects were tilted to 60 degrees for up to 60 min or until symptomatic. Lower body negative pressure (40 mmHg) was applied for 10 min after 30 min rest. Seven of 17 transplant recipients and 11 of 12 controls became symptomatic during tilt testing, and 9 of 1

Abdominal compartment syndrome: a rare complication of plication of the diaphragm.

Abdominal compartment syndrome is an increasingly recognized phenomenon. We report the case of an otherwise fit and healthy 42-year-old man who underwent plication of the right hemidiaphragm for idiopathic phrenic paresis. His postoperative recovery was complicated by abdominal compartment syndrome, which was managed conservatively. We believe this is the only report of this complication after diaphragmatic plication and one of very few reported thoracic causes of abdominal compartment syndrome.

Blood flow restriction pressure for narrow cuffs (5 cm) cannot be estimated with precision.

Blood flow restriction pressures are set relative to the lowest pressure needed to occlude blood flow with that specific cuff. Due to pressure limitations of some devices, it is often not possible to occlude blood flow in all subjects and apply a known relative pressure in the lower body with a 5 cm wide cuff.Objective. To use a device capable of generating high pressures (up to 907 mmHg) to create and validate an estimation equation for the 5 cm cuff in the lower body using a 12 cm cuff.Approach. 170 participants had their arterial occlusion pressure (AOP) with a 5 cm and 12 cm cuff and their

[Local and systemic hemodynamic reactions as components of action of local lowered pressure].

It is shown that low atmospheric pressure produces a positive effect on systemic and regional hemodynamics in patients with diseases of the small pelvis and lower limbs manifesting by increased speed of regional blood flow due to lowering of peripheral resistance in small caliber arteries and arterioles, reduction of venous congestion and improvement of microcirculation. This makes hypobarotherapy perspective in treating diseases running with arterial ischemia and venous congestion.
