Sagittal wrist motion of carpal bones following intraarticular fractures of the distal radius.

Forty patients (mean age, 37 years) with intraarticular C2 and C3 Colles fractures were treated by open reduction, internal fixation and bone grafting. At a mean follow-up of 8 years radiocarpal and midcarpal motion was evaluated, the depth of the articular surface of the distal radius in the sagittal plane was measured and the presence of arthritis was noted. The fractures healed with a mean palmar tilt of 6 degrees , a mean ulnar tilt of 18 degrees and ulna variance within 1 mm of the contralateral side. The depth of the articular surface of the distal radius was 1.3 mm greater than the unin

Combined Vascularised Bone Graft Techniques for Displaced Coronal Fracture in Kienböck Disease: A Case Report.

Surgical treatment of Kienböck disease remains controversial and depends on patient age, activity and disease stage. We report an 18-year-old female badminton player who presented with wrist pain, limited motion and decreased grip strength. Imaging revealed a displaced coronal lunate fracture, with the palmar fragment locked against the volar rim of the radius, consistent with Lichtman Stage IIIC and Bain grade 2b. MRI showed low signal intensity, suggesting compromised vascularity. Arthroscopy confirmed intact cartilage except at the fracture site. Lunate reconstruction using the Kuhlmann tec

Pronator quadratus pedicled bone graft in the treatment of Kienböck disease: follow-up 2 to 12 years.

We inserted a pronator quadratus pedicled bone graft into the avascular lunate with preservation of its corticocartilaginous portion for the treatment of Kienböck disease. Twenty-seven patients (14 men, 13 women; mean age, 42 years (range 17-66 years)) were treated between September 2005 and July 2014 and followed up from 2 to 12 years. Three patients had Lichtman stage II disease, 13 had stage IIIA, and 11 had stage IIIB. Among them, 18 patients showed radiological improvement in lunate morphology. The Lichtman stage was unchanged in 23 patients and aggravated in four patients. The Stahl inde

Scapholunate Ligament Reconstruction With a Novel Technique for Passing Split Extensor Carpi Radialis Brevis Tendon Graft Through Bone Tunnels.

We report 2 patients with acute scapholunate dissociation with static instability and wide scapholunate gapping. They underwent scapholunate reconstruction using a split extensor carpi radialis brevis (ECRB) tendon graft via a dorsal approach. This technique is adapted from the modified Brunelli tenodesis, utilizing a portion of the ECRB instead of a flexor carpi radialis tendon graft to reconstruct the scapholunate interosseous ligaments and dorsoradiotriquetral ligaments. The novel aspects of this technique include the use of a trifold plastic sheet of biaxially oriented polypropylene to fac

Lunate Grafting Into the Radius for Lunate Fossa Reconstruction in Madelung Deformity.

A 40-year-old woman presented with Madelung deformity and severe arthritic changes at both the radiocarpal and the distal radioulnar joints. She was treated by using her lunate as an osteochondral graft into the radius allowing reconstruction of the lunate fossa. The scaphoid and triquetrum were removed concomitantly and a Sauve-Kapandji procedure was performed. Complete bone healing was achieved. Ten years later, an excellent functional result was maintained, with a pain-free wrist, an acceptable wrist joint range of motion, as well as a favorable aesthetic appearance. This procedure may be i

Bilateral Bipartite Lunate Treated With Proximal Row Carpectomy: A Case Report and Review of the Literature.

CASE: A right-hand dominant, 38-year-old man presented to clinic with atraumatic left wrist pain of 1 year duration in the setting of bilateral bipartite lunate. Both wrists ultimately became symptomatic and were treated with proximal row carpectomy. The patient had durable pain relief in both wrists after proximal row carpectomy. CONCLUSION: Bipartite lunate is a rare developmental anomaly that is usually asymptomatic. Operative intervention directed at treating involved osteoarthritic joints is warranted in the rare symptomatic patient. Clinicians should be aware of the pathology given its

[Diagnosis of unusual fractures of the wrist bones].

Fractures of the navicular bone are the most frequent fractures in the region of the wrist bone. Injuries of the other carpal bones are often primarily overlooked, since the examiner commands over less routine in diagnosing these rare fractures and the carpal bones are often superimposed over one another. We examined 9.723 x-ray films of the hand performed because of suspected fracture of the os naviculare, in order to find out how often lesions of the carpal bones - with the exception of those of the scaphoid bone - were discovered via the x-ray series of the os naviculare manus. For each ind

Finite element modeling of the influence of hand position and bone properties on the Colles' fracture load during a fall.

Distal forearm fracture is one of the most frequently observed osteoporotic fractures, which may occur as a result of low energy falls such as falls from a standing height and may be linked to the osteoporotic nature of the bone, especially in the elderly. In order to prevent the occurrence of radius fractures and their adverse outcomes, understanding the effect of both extrinsic and intrinsic contributors to fracture risk is essential. In this study, a nonlinear fracture mechanics-based finite element model is applied to human radius to assess the influence of extrinsic factors (load orientat

Bone Grafting and Unloading Procedures for Stage 1-3a Kienbock's Disease.

INTRODUCTION: Kienbock's disease is an avascular necrosis of the lunate. Although the etiology is not well known, the progression of the disease from ischemia to lunate collapse to arthritis is well documented. In the pre-collapse stages of Kienbock's disease, procedures for joint leveling, unloading the lunate, and revascularization are performed. The aim was to analyze the usual presentation of patients with Kienbock's disease clinically and radiologically to assess the ulnar variance, carpal alignment, carpal height preoperatively and postoperatively, and the outcome of the above-mentioned

[A clinical and experimental study of dorsiflexed intercalated segmental instability in a scaphoid fracture].

The purpose of this study was to clarify the pathomechanics of dorsiflexed intercalated segmental instability (DISI) in a scaphoid fracture. Twenty two patients with a scaphoid fracture were used for the clinical study. DISI was recognized in 6 patients. There was no statistically significant difference between the DISI(+) group and the DISI(-) group in total ROM, or in grip strength. In the DISI(+) group, however, ROM shifted dorsally (p < 0.05). Clinical factors for DISI were as follows. (1) pseudoarthrosis without treatment: 2 cases; (2) bone union taking place in a humpbacked position: 3 c

Mechanical Effects of Wrist Position at the Wrist Joint: A Finite Element Analysis.

PURPOSE: This study aimed to evaluate the effects of wrist joint angles during flexion-extension and radioulnar deviation on stress distribution using finite element analysis. METHODS: Eight fresh-frozen upper limb specimens were analyzed using computed tomography. Finite element models were developed to simulate grip postures in flexion-extension (five positions ranging from 30° flexion to 30° extension) and radioulnar deviation (eight positions from 15° radial deviation to 20° ulnar deviation). Stress distributions (equivalent stress, minimum principal stress, and maximum principal stress)

An Algorithmic Approach to the Treatment of Kienböck Disease.

The algorithm and rationale described is a reflection of our own surgical experience for this challenging disorder and can be compared with other publications. Our algorithm has evolved from treatment of a large volume of patients with Kienböck disease in a referral practice. However, it is limited to the management that we have found logical, effective, and within our scope of experience. The treatment guidelines for our specialty as a whole will evolve as our understanding of the etiology and our ability to quantify efficacy improves.

[Stress osteotomy of the distal radius- and ulna metaphysis (Illarramendi procedure): an alternative treatment method in lunate necrosis].

The etiology of Kienböck's disease is still unknown. Many operative procedures, usually based on biomechanical considerations, are recommended. Based on known effects of core decompression procedures on bone, we present a technique of core decompression of the metaphysis of the distal radius and ulna (MCD) as an easy, alternative biologic treatment. Ten cases with a mean follow-up of 10.8 years are analyzed. All patients had significant postoperative pain relief and returned to their previous activities. At the time of follow-up, comparative motion of flexion-extension of the wrist averaged 74

[Partial arthrodesis of the carpal bones in advanced carpal collapse in chronic scapho-lunar instability and following scaphoid pseudoarthrosis].

Degenerative arthritis of the wrist as a consequence of chronic scapho-lunate instability or chronic scaphoid non-union follows very specific patterns. Joint destruction occurs primarily in the radio-scaphoid and in the luno-capitate joints while the radio-lunar joint is preserved. Treatment by partial carpal fusion between capitate, scaphoid and lunate aims at reconstruction of correct length of the central column and restoration of normal carpal height by reducing the dorsiflexed lunate bone, thereby decompressing the painful radio-scaphoid joint. We have carried out the procedure in 21 case
