Circumstances of child deaths in Mali and Uganda: a community-based confidential enquiry.

BACKGROUND: Interventions to reduce child deaths in Africa have often underachieved, causing the Millennium Development Goal targets to be missed. We assessed whether a community enquiry into the circumstances of death could improve intervention effectiveness by identifying local avoidable factors and explaining implementation failures. METHODS: Deaths of children younger than 5 years were ascertained by community informants in two districts in Mali (762 deaths) and three districts in Uganda (442 deaths) in 2011-15. Deaths were investigated by interviewing parents and health workers. Investig

Microcyclosporella mali: a novel fungal keratitis in a post-penetrating keratoplasty patient.

An 86-year-old woman underwent penetrating keratoplasty for pseudophakic bullous keratopathy and presented with an acute corneal ulcer thereafter. Examination demonstrated a fluffy white infiltrate and epithelial defect with subsequent endothelial plaque formation and anterior chamber inflammation. The ulcer was cultured, and fortified topical vancomycin and tobramycin were initiated but failed to significantly improve the clinical course. Cultures were ultimately positive for fungus Microcyclosporella mali that responded well to topical natamycin with stabilisation of the ulcer after 6 weeks

Leishmania major, the predominant Leishmania species responsible for cutaneous leishmaniasis in Mali.

Leishmania major is the only species of Leishmania known to cause cutaneous leishmanisis (CL) in Mali. We amplified Leishmania DNA stored on archived Giemsa-stained dermal scraping slides obtained from self-referral patients with clinically suspected CL seen in the Center National d'Appui A La Lutte Contre La Maladie (CNAM) in Bamako, Mali, to determine if any other Leishmania species were responsible for CL in Mali and evaluate its geographic distribution. Polymerase chain reaction (PCR) amplification was performed using a Leishmania species-specific primer pair that can amplify DNA from L. m

Piloting an intervention to improve microbiological food safety in Peri-Urban Mali.

INTRODUCTION: Diarrhoeal diseases remain a major cause of preventable death among children under five years old in developing countries. Studies related to infant diarrhoea causation have demonstrated a higher level of faecal contamination in weaning foods than in drinking water. Many studies have examined the microbiological quality of such foods, but few of them have resulted in an intervention. The present study builds upon an experiment in which the HACCP approach was applied to preparation of two common weaning foods (moni and fish soup) and used to develop simple hygiene measures which m

Failure of mebendazole in treatment of human hookworm infections in the southern region of Mali.

Preliminary studies indicated that single-dose (500 mg) mebendazole gave disappointing results in the treatment of hookworm infections (Necator americanus) in Mali. A placebo-controlled, randomized trial conducted with the participation of 103 infected subjects (background hookworm prevalence > 50%) confirmed that mebendazole (Vermox) did not reduce parasite burdens significantly, as assessed through fecal egg counts. In contrast, a group of subjects treated with pyrantel (Combantrin) experienced a significant reduction in fecal worm egg counts (overall, both sexes combined showed a 75% reduct

[The causes of blindness in the 3rd region of Mali. Study of 101 clinical records].

A retrospective study of 101 blind patients seen in daily consultation has been done in Sikasso's Yeelen Center from January 1988 to October 1989. The results obtained show that the frequency of blindness is 1.3% and that more than 75% of the causes of losing the seeing are due to the pathology of the posterior segment. The optical atrophy represents the major cause of blindness (63% of the cases) with usually a glaucoma excavation. Corneal opacities and the chorioretinal scarrings take respectively the second and the third place with 13% and 10%.

[Epidemiology and pathologic results of chronic carriers state of hepatitis B in Mali].

The HB antigen of the hepatitis B (HB) virus, studied by counter immunoelectrophoresis shows a prevalence of 8.7% in 1,860 rural Malians and 11.3% in 764 blood donors from Bamako. Amongst 1,350 hospitalised patients, no correlation could be established between the HBs antigen chronic carriers state and other infectious diseases, malnutrition or genetic deficiencies. On the other hand, the prevalence of HBs antigen is particularly high in hepatic infections: acute and chronic hepatitis (53.5%), cirrhosis (31.5%) and hepatomas (25.3%). The study of the prevalence of hepatitis B by radioimmunoass

Outcomes of community-based nutrition support groups in Dioila Health District, Mali.

BACKGROUND: Malnutrition remains a major public health concern in Mali, particularly in rural districts where food insecurity, limited dietary diversity, and inadequate infant and young child feeding (IYCF) practices persist. The Koulikoro Region continues to experience malnutrition levels above the national average. To strengthen community-level prevention and early detection, the Ministry of Health and partners implemented Nutrition Support Groups (NSGs), facilitated by relais communautaires. Despite their scale-up, evidence on NSG functionality and effectiveness in rural settings, including

Attractive targeted sugar bait phase III trials in Kenya, Mali, and Zambia.

BACKGROUND: Long-lasting insecticidal nets (LLINs) and indoor residual spraying (IRS) target night-time indoor biting mosquitoes and effectively reduce malaria transmission in rural settings across Africa, but additional vector control tools are needed to interrupt transmission. Attractive targeted sugar baits (ATSBs) attract and kill mosquitoes, including those biting outdoors. Deployment of ATSBs incorporating the insecticide dinotefuran was associated with major reductions in mosquito density and longevity in Mali. The impact of this promising intervention on malaria transmission and morbid

Tracking Rift Valley fever: From Mali to Europe and other countries, 2016.

On 16 September 2016, the World Health Organization confirmed a Rift Valley fever (RVF) outbreak in Niger. Epidemiological surveillance was reinforced among the French Armed Forces deployed in Niger and bordering countries: Chad, Mali and Burkina Faso. On 26 October, a probable case of RVF was reported in a service member sampled in Mali 3 weeks earlier. At the time the result was reported, the patient was on vacation on Martinique. An epidemiological investigation was conducted to confirm this case and identify other cases. Finally, the case was not confirmed, but three suspected cases of RVF

Low coverage of central point vaccination against dog rabies in Bamako, Mali.

Canine rabies remains an important public-health problem in Africa. Dog mass vaccination is the recommended method for rabies control and elimination. We report on the first small-scale mass dog vaccination campaign trial in Bamako, Mali. Our objective was to estimate coverage of the vaccination campaign and to quantify determinants of intervention effectiveness. In September 2013, a central point vaccination campaign--free of cost for dog owners--was carried out in 17 posts on three consecutive days within Bamako's Commune 1. Vaccination coverage and the proportion of ownerless dogs were esti

Higher Tetanus Toxoid Immunity 2 Years After PsA-TT Introduction in Mali.

BACKGROUND: In 2010, mass vaccination with a then-new meningococcal A polysaccharide-tetanus toxoid protein conjugate vaccine (PsA-TT, or MenAfriVac) was undertaken in 1- to 29-year-olds in Bamako, Mali. Whether vaccination with PsA-TT effectively boosts tetanus immunity in a population with heterogeneous baseline tetanus immunity is not known. We assessed the impact of PsA-TT on tetanus toxoid (TT) immunity by quantifying age- and sex-specific immunity prior to and 2 years after introduction. METHODS: Using a household-based, age-stratified design, we randomly selected participants for a pre
