Success of surgical interventions for direct dental implant-related injuries to the mandibular nerve: a review.

To the best of our knowledge there are no guidelines regarding the surgical management of dental implant-related injuries to the mandibular nerve. This review aims to investigate the success of different surgical interventions. Neurosensory injury to the mandibular branch of the trigeminal nerve can occur during administration of local anaesthetic, elevation of the flap, preparation for osteotomy, and placement of the implant. Surgical interventions include extraction of the implant, external decompression, internal neurolysis, excision of a neuroma, neurorrhaphy, nerve grafting, and low-level

Revisiting the Topographic Anatomy of the Marginal Mandibular Branch of Facial Nerve Relating to the Surgical Approach.

BACKGROUND: The marginal mandibular branch (Mbr) of the facial nerve is vulnerable to damage during rhytidoplasty, surgical reduction of the mandibular angle, parotidectomy, and excision of the submandibular gland. OBJECTIVES: The authors sought to map the Mbr and determine the relationship between the number of Mbr offshoots and the course of the Mbr. METHODS: The Mbr was examined in 29 hemifaces from 12 embalmed and 4 fresh cadavers (10 males, 6 females; mean age, 73.7 years). RESULTS: The Mbr was located ≤5 mm from the gonion (Go) in 24 of 29 hemifaces (82.8%) and ≤10 mm from the interse

Transverse cervical and great auricular nerve distribution in the mandibular area: a study in human cadavers.

The angle of the jaw is innervated by the auricular branch of the superficial cervical plexus (SCP). Cervical cutaneous nerves of the CP carry the sensation from the antero-lateral cervical skin. It is clinically relevant to identify the cervical cutaneous nerve distribution and the nerve point using superficial landmarks but published studies describing the emerging patterns and cervical cutaneous nerve branch distributions in the mandible are rare. The overlap between the cervical and trigeminal and facial nerve distributions and anastomoses is highly variable. The objective of this study wa

The efficacy of mylohyoid nerve anesthesia in dental implant placement at the edentulous posterior mandibular ridge.

The aim of this study is to evaluate the anesthetic efficacy of mylohyoid and buccal nerve anesthesia at the posterior edentulous mandible versus regional anesthetic block to the inferior alveolar nerve in dental implant surgery. The study was composed of 2 groups. In the first group (group A), 14 voluntary adults (7 female and 7 male) received local infiltrations of 1 mL articaine HCl 4% with epinephrine 1/200,000 to the ipsilateral mylohyoid and buccal nerves. In the second group (group B, control; 9 female and 5 male adults), the inferior alveolar and the buccal nerve blocks were performed.

The use of mandibular nerve block to predict safe anaesthetic induction in patients with acute trismus.

Acute trismus can be caused by pain, muscle spasm, swelling or mechanical obstruction. Unfortunately, the cause is not always obvious during pre-operative airway assessment. In this pilot study, we prospectively evaluated mandibular nerve block as a pre-operative tool to identify patients with reversible causes of trismus, namely pain or spasm, in order to allow safe anaesthetic induction. Six patients with unilateral fractured mandibles and trismus received a mandibular nerve block before induction of general anaesthesia. There was an increase in maximal inter-incisor gap after the blocks (me

Mental nerve paresthesia associated with endodontic paste within the mandibular canal: report of a case.

The present study describes a case of endodontic paste (Endomethasone) penetration within and along the mandibular canal from the periapical zone of a lower first premolar following endodontic treatment of the latter. The clinical manifestations comprised anesthesia of the right side of the lower lip and paresthesia of the gums in the fourth quadrant, appearing immediately after endodontic treatment. The lip anesthesia was seen to decrease, with persistence of the gingival paresthesia, after 7 months.

Linear lichen planus in the region of the mandibular nerve caused by an allergy to palladium in dental metals.

We observed a case of linear lichen planus extending from the right cheek to the jaw, which was accompanied by itching and an unusual sensation in the mouth. The eruption was located along the mandible and aligned with the mandibular nerve. The right molars of the patient contained metal compounds and these were considered to be the cause of his disease. Patch testing with various dental metals showed positive reactions to palladium and platinum. A biopsy specimen showed a lichenoid reaction in the lesional tissue, and dermal contact dermatitis was noted at the palladium patch test site. His d

An upper cervical spine injury producing paraesthesia in the distribution of the mandibular division of the trigeminal nerve.

A case is reported of a 37-year-old man who developed paraesthesia in the distribution of the mandibular division of the trigeminal nerve following an injury to the upper cervical spine during a game of rugby. The central pathways of the trigeminal nerve are discussed and it is proposed that mandibular fibres descended beyond their expected level in the spinal trigeminal nucleus, and that it was these fibres which were compressed in the unstable dislocation at the atlanto-axial joint producing the paraesthesia described. Other remote anatomical explanations for the paraesthesia are considered.

Beyond the Mandibular Canal: A Narrative Review of Inferior Alveolar Nerve Variations and Their Clinical Significance.

The inferior alveolar nerve (IAN) is commonly presented as a relatively constant branch of the mandibular nerve that gives rise to the mylohyoid nerve (MHN), enters the mandibular foramen, follows a single mandibular canal, and terminates as mental and incisive branches. However, direct human anatomical evidence shows substantially greater complexity. This critical narrative review synthesizes cadaveric, microdissection, histological, imaging, and osteological evidence on proximal IAN roots, relationships with the maxillary artery, communications with neighboring mandibular nerve branches, MHN

Leiomyosarcoma of the infratemporal fossa with perineurial spread along the right mandibular nerve: a case report.

Leiomyosarcomas are malignant tumors displaying strong smooth muscle differentiation. They can often develop within the GI tract and myometrium, but are particularly rare in the head and neck. Perineurial spread of head and neck cancer is observed in patients with neoplasms of the skin (squamous cell carcinoma, melanoma) or skin appendages (adenoid cystic carcinoma). We report the case of a woman who presented with diplopia and headaches. MRI showed an infratemporal mass lesion and faint enhancement tracking along the mandibular nerve into the wall of the right cavernous sinus. A nerve biopsy

Revisiting the anatomic relationship of the marginal mandibular nerve and the posterior facial vein: a cadaveric study.

In preparation for the donor and recipient facial vascularized composite allotransplant (VCA), the marginal mandibular nerve branch should be identified and co-apted to the recipient's nerve to allow for re-innervation. We describe a method to identify the marginal mandibular branch of the facial nerve in facial VCA donors and recipients. Through cadaver dissections, the posterior facial vein was traced posteriorly to identify the marginal mandibular branch of the facial nerve. In cases where we were unable to use this relationship, we found the posterior facial venous system to be diminutive.

[Mandibular third molar extractions with proximity to the inferior alveolar nerve canal: what are the alternatives?].

Throughout the evolution, modern diet became softer and led to less attrition and less mesial migration of teeth resulting in an increased incidence of teeth impaction. When a mandibular third molar extraction is indicated, damage to the inferior alveolar nerve (IAN) is one of the most important complications. The objective of this review is to provide an overview of the alternative treatment modalities to total removal for a third mandibular molar in cases of proximity to the inferior alveolar nerve. Among the discussed options are: Orthodontic intervention to slowly move the tooth apex away

Post-operative nerve damage and the removal of the mandibular third molar: a matter of common consent.

The aim of this study was to seek a consensus regarding written preoperative warning of possible nerve damage for patients having outpatient surgical removal of mandibular third molar teeth with local analgesia in Oral and Maxillofacial surgical units throughout the UK. The person first advising the patient is not always the one undertaking the operative procedure, and confusion concerning consent may arise in such circumstances. Questionnaires were sent to 140 consultants in Oral and Maxillofacial surgery units throughout the UK. Replies were received from 101 consultants. Of these, 29.7% rou

Changes in the inferior alveolar nerve following mandibular lengthening in the dog using distraction osteogenesis.

Distraction osteogenesis as per Ilizarov was used to lengthen the canine mandible. In this study, physiological and ultrastructural examination of the inferior nerve was performed. Mandibular body corticotomies were performed, and the mandible was distracted 7 mm. The animals were killed 4 weeks after the distraction was completed. Bone formed within the distraction gap in all dogs. There was no statistically significant difference in the jaw-jerk voltage between control and experimental sides. There was a significant difference between the distracted and control nerves in only one area of one

[Complications of alcohol injection into the mandibular nerve. Gasserian palsy and oculomotor palsy (author's transl)].

Total sensory loss, ptosis, ocular muscle palsy and pupil dilatation were noted after alcohol injection into mandibular nerve for idiopathic trigeminal neuralgia (in the right 2. and 3. division). The diagnosis was made that the injected alcohol induced the Gasserian palsy and oculomotor palsy. The oculomotor palsy was fully recovered except slight pupil dilatation after four months. The mechanism and prophylaxis of these complications are discussed.

Evaluating Sensory Nerve Outcomes After Horizontal Osteotomy for Mandibular Distraction in Infant Robin Sequence Patients.

There has yet to be a consensus on the appropriate technique for mandibular distraction in the infant Pierre Robin population nor is their sufficient data on sensory nerve outcomes. The purpose of this study is to validate the safety of the horizontal osteotomy by: (1) determining mandibular foramen location in infant Pierre Robin patients relative to the dentoalveolar plane and (2) evaluate long-term function of the inferior alveolar nerve in previously distracted Pierre Robin patients. Preoperative 3D Computed tomography (CT) scans of Pierre Robin patients < 1 year old were reviewed. A line

Anatomical relationship between mental foramen, mandibular teeth and risk of nerve injury with endodontic treatment.

OBJECTIVES: The objective of the present study was to evaluate the anatomical relationship between mental foramen (MF), including the incidence of the anterior loop of the inferior alveolar nerve (AL), and roots of mandibular teeth in relation to risk of nerve injury with endodontic treatment. MATERIALS AND METHODS: Cone-beam computed tomography (CBCT) images, which included teeth either side of the MF, were randomly selected. The anonymised CBCT images were reconstructed and examined in coronal, axial and sagittal planes, using three-dimensional viewing software, to determine the relationshi

Unusual facial pain secondary to inferior alveolar nerve compression caused by impacted mandibular second molar.

Symptoms of inferior alveolar nerve (IAN) compression are reported during endodontic procedures, placement of implants, third molar surgeries, inferior alveolar nerve block injections, trauma, orthognathic injuries, ablative surgeries or use of medicaments. Presented is a rare case of a 15-year-old girl who reported severe pain in relation to an impacted permanent mandibular left second molar, the roots of which had entrapped the mandibular canal causing compression of IAN. Timely surgical intervention and sectional removal of the impacted molar is indicated to relieve the symptoms and avoid p
