PURPOSE: Coronectomy is a valuable treatment proven safe for non-pathological mandibular third molars with an increased risk of inferior alveolar nerve injury. Coronectomy may also be useful for mandibular third molars with dentigerous cysts and caries, but this is not commonly performed due to the lack of well-designed, evidence-based studies. Here, we aim to investigate the safety of coronectomy for mandibular third molars with caries and dentigerous cysts.
METHODS: One hundred fifteen patients with an impacted mandibular third molar with a dentigerous cyst or caries underwent coronectomy o

PURPOSE: An uncommon, but serious complication of endodontic treatment is inferior alveolar nerve (IAN) injury warranting operative intervention for exploration, debridement, and repair. The purpose of the study was to evaluate outcomes of microneurosurgical intervention for endotontic-related IAN injuries in terms of achievement of functional sensory recovery (FSR) and pain relief and to identify factors affecting outcomes.
METHODS: A retrospective cohort study of patients who had microsurgical exploration and repair of IAN injuries sustained during endodontic therapy was performed. The prim

Objective To investigate the effects of low-intensity pulsed ultrasound (LIPUS) on the mandibular remodeling following inferior alveolar nerve transection (IANX) and to optimize the parameters of LIPUS in the treatment of nerve injury. Methods IANX was performed in male Sprague-Dawley rats. Four weeks after IANX,the effect of daily LIPUS (from day 1) on the transected inferior alveolar nerve was examined in terms of sensitivity to mechanical stimulation. Moreover, histopathologic changes of mandibles were analyzed by micro-CT,HE staining,Masson trichrome staining, and immunohistochemical stainin

BACKGROUND: Disorders of lower lip in facial expression are mostly caused by functional impairment of the marginal mandibular branch of the facial nerve. In a typical case, active exposure of the incisors of the mandible is not possible and the lip appears distorted towards the healthy side. The most frequent cause of a lesion of the marginal mandibular branch is iatrogenic injury during operations in the mandibular or parotid region.
DIFFERENTIAL DIAGNOSIS: Functional disorders of the platysma, congenital hypoplasia or aplasia of the lower lip muscles, and defective healing following facial

We present a case in which interosseous wiring of the mandible resulted in an A-V fistula and an injury to the mandibular ramus of the facial nerve. These two complications can serve as a reminder to all who desire to use interosseous wiring in the treatment of mandibular fractures.

INTRODUCTION: Mandibular fractures are common in maxillofacial trauma, often requiring surgical intervention for optimal recovery. One critical concern during such procedures is the potential for nerve damage, specifically involving the mandibular canal. This study aims to investigate whether the presence of postoperative mandibular canal discontinuity is associated with an increased risk of nerve damage.
METHODOLOGY: A retrospective analysis was conducted on a cohort of 72 postoperative mandibular fracture patients. In total, 58 patients had entire mandibular canal continuity, while 14 showe

The treatment dilemma provided by asymptomatic third molars in mandibular angle fractures remains controversial. This prospective randomized controlled trial was undertaken to determine whether there is an advantage to extraction or retention of the third molar whilst repairing a mandibular angle fracture. Sixty-four patients were allocated randomly to the two treatment groups. All underwent open reduction and internal fixation (ORIF) with standard postoperative care. The primary outcome measure was uncomplicated fracture healing. Secondary measures were surgical duration, malocclusion, wound

BACKGROUND: Evidence shows neural involvement in bone remodeling; regulating maxillofacial nerve repair modulates jawbone. Neural stem cell (NSC) therapy is limited by sources/ethics, but neural crest-derived dental mesenchymal stem cells (MSCs) like stem cells from the apical papilla (SCAPs) have strong neuroregenerative potential for NSC transdifferentiation. Schisantherin A (Sch-A), neuroprotective, enhances NSC proliferation/differentiation. This study explores optimal Sch-A concentration/duration for SCAP neural differentiation and effects on rat mental nerve repair/mandibular development

PURPOSE: Injury to the marginal mandibular nerve (MMN) can occur in numerous surgeries involving incisions near the inferior border of the mandible. Injury to this nerve can cause considerable cosmetic and functional deformities. A thorough knowledge and understanding of the anatomic pathway of the MMN is important to avoid permanent nerve damage. The purpose of this study was to determine the position of the marginal mandibular nerve in relation to several key mandibular anatomic landmarks and to identify variations of the nerve as it approaches the inferior border of the mandible.

The surgical removal of lower third molars endangers both the lingual and inferior alveolar nerves. Patients sustaining an injury to either of these nerves must be managed correctly, and this requires a diagnosis of the injury type and regular monitoring of the recovery of sensation. Surgical intervention for a damaged inferior alveolar nerve is not usually indicated but may be undertaken: if the nerve is completely divided and the severed ends are misaligned; if a bony fragment has compressed the mandibular canal; or if the patient suffers from persistent neuropathic pain. In contrast, after

One of the most important concerns during posterior mandibular implant placement is avoiding the mandibular canal. Nerve injury can be very disturbing to the patient, possibly causing mild paresthesia to complete anesthesia. Explantation of a dental implant that gas violated the mandibular canal is the most recommended treatment. However, an osseointegrated implant that placed in the mandibular canal can be left if the patient shows no symptoms. In this case report, we describe a technique to maintain the apexes of the implants while partially removing fractured osseointegrated implants previo

OBJECTIVES: The facial trauma surgeon should be able to list indications for endoscopic treatment of mandibular condyle fractures and discuss the limitations and complications of the technique.
BACKGROUND: The ideal treatment of mandibular subcondylar fractures continues to be debated. Acceptable results are often obtained with conservative measures such as mandibular maxillary fixation followed by elastics. On the other hand, an open approaches result in potential risk injury to the facial nerve. These 2 arguments have cautioned many surgeons from open treatment of condylar fractures. Recent

PURPOSE: Injuries of the inferior alveolar nerve (IAN) related to endodontic treatment are being increasingly reported. However, consensus on the preferred intervention and the timing of and indications for surgical treatment is lacking. Here, we describe our experience with painful IAN injuries arising from endodontic treatment and requiring prompt microsurgical treatment.
METHODS: Seven consecutive patients with painful IAN injuries were referred to the Maxillofacial Surgery Unit of San Paolo Hospital in Milan. All patients had undergone root canals endodontic treatment in the mandibular mo

PURPOSE: The purpose of this study was to examine the role of preoperative computed tomography (CT) imaging of the inferior alveolar nerve (IAN) for patients at increased risk for nerve injury during mandibular third molar (M3) extraction.
MATERIALS AND METHODS: To address the research purpose, the investigators enrolled a sample composed of subjects who presented for mandibular M3 extraction and had panoramic radiographic signs interpreted as being associated with an increased risk for inferior alveolar nerve injury. All subjects had preoperative CT imaging studies done to ascertain the posi

A 26-year-old man was suffering from pericoronitis of his mandibular third molars. To determine the position of the mandibular canal in relation to the roots of the third molars, a panoramic radiograph was made. The radiograph revealed at the right side a bifid mandibular canal and the upper part of the canal seemed to be related to the third molar. Additionally, a cone beam CT was made, which revealed a bifid mandibular canal at the left side and a trifid mandibular canal at the right side. Anatomical anomalies of the mandibular canal may have clinical implications, such as an increased risk

PURPOSE: Lingual nerve damage is one of the main complications of mandibular third molar removal. In literature, the series published are heterogeneous so we have decided to study the importance of the lingual protection only during germectomies (wisdom teeth without root formation).
METHODS: We conducted a randomized prospective study in 300 germectomies, operated by the same skilled surgeon. These 300 third molar removals have been divided in two groups, one with lingual nerve protection and one without.
RESULTS: Lingual nerve injuries following germectomies, with or without lingual protec

When facial nerve paralysis complicates a mandibular fracture, it may be difficult to locate the point of injury, since the nerve may be injured in the fallopian canal by a secondary temporal bone fracture or in soft tissue by mandibular fragments. Following a review of eight previously reported cases, this paper presents two additional cases, one with bilateral, complete paralyses and the other with a unilateral paresis. In the first, the condyles were driven posteriorly, resulting in bilateral temporal bone fractures, a unilateral external canal stenosis, and a unilateral sensorineural heari
