The sagittal split ramus osteotomy (SSRO) and intraoral vertical ramus osteotomy (IVRO) are two common orthognathic procedures for the treatment of mandibular prognathism. This randomized clinical trial compared the surgical morbidities between SSRO and IVRO for patients with mandibular prognathism over the first 2 years postoperative. Ninety-eight patients (40 male, 58 female) with a mean age of 24.4±3.5 years underwent bilateral SSRO (98 sides) or IVRO (98 sides) as part or all of their orthognathic surgery. IVRO presented less short-term and long-term surgical morbidity in general. The SSRO

The healing process at the osteotomy site can result in visible mandibular contour irregularities, compromising aesthetic outcomes and requiring secondary corrective procedures. This study provides a literature review on the risk factors for mandibular notching following bilateral sagittal split osteotomy (BSSO) while also proposing an original preventative technique.

This article presents a case involving a 16-year-old female patient in whom a pseudoaneurysm of a facial artery branch was diagnosed two weeks after initial orthognathic surgery. The patient presented with a sensation of pressure, accompanied by pulsations synchronised with her heartbeat, located on the right side of the mandible. Ultrasound imaging, followed by CT angiography, confirmed the diagnosis of a pseudoaneurysm within the parotid region. Endovascular embolization effectively stopped blood flow to the pseudoaneurysm, leading to rapid recovery without any lasting complications. A pseud

Large, benign intramandibular lesions are frequently removed by resection followed by extensive free tissue transfer or delayed bone grafting. We outline a protocol to remove benign mandibular lesions using sagittal split osteotomy (SSO) with virtual surgical planning (VSP) to mitigate risks involved with this effective, tissue-saving approach. Patients with benign mandibular lesions accessed by SSO with VSP during 2014 were included in this study. Computed tomographic data were imported into VSP software. Using VSP, the exact locations of mandibular lesions and the inferior alveolar nerve can

The objective of this study was to propose a treatment protocol for patients with lateral prognathism based on the unilateral sagittal split ramus osteotomy (USSRO). This was a prospective study involving 31 patients with lateral prognathism, who required a bilateral sagittal split ramus osteotomy (BSSRO). Two groups were formed using the proposed protocol, with specific inclusion criteria for each group: BSSRO (n=17) and USSRO (n=14). Occlusal parameters (dental midline deviation, overbite, and overjet) were measured preoperatively (T0), at model surgery (T1), 1 month postoperative (T2), and

Nonunion of the mandible after microvascular flap reconstruction of defects resulting from tumor surgery is a known but rare complication, occurring in 5% of cases. When nonunion results with the use of microvascular flaps, rigid internal fixation, and radiotherapy, other options to treat the nonunion carry a greater risk of osteomyelitis, osteoradionecrosis, and/or persistent nonunion. Although endosseous implants have been reported to cause mandibular fracture in some cases, our case proposes and supports the use of implants for immobilization and/or prevention of nonunion of the mandible in

BACKGROUND: Unilateral sagittal split ramus osteotomy (USSRO) is not widely used given the postoperative instability caused by the inevitable rotation of the mandibular segment during surgery. However, the influence of mandibular movement on the condylar morphology and position stability has not been completely explored. The aim of the study was to quantitatively evaluate the effect of USSRO on the condylar surface morphology changes and postoperative stability in patients with mandibular lateral prognathism and compare these findings with the classic bilateral sagittal split ramus osteotomy (

OBJECTIVE: This study proposes a new plate system, a sliding plate, after sagittal split ramus osteotomy (SSRO). The sliding plate was designed to adjust the spatial relationship between the proximal and distal segments during the postoperative period. This plate was used for mandibular setback surgery to minimize forward early relapse.
STUDY DESIGN: Twenty-six Korean subjects who had undergone mandibular setback surgery by SSRO were included in this study. Skeletal stability was evaluated with lateral cephalograms and 3-dimensional cone-beam computerized tomography.
RESULTS: Forward mandibu

BACKGROUND: The teaching of bilateral sagittal split osteotomy (BSSO) requires the utilisation of practical training methods to optimise the surgical result and prevent complications such as bad splits or damage to the inferior alveolar nerve. The objective of the present study was to evaluate a structured training program using a cost-effective, 3D-printed mandibular model to improve osteotomy and splitting performance in BSSO.
MATERIAL & METHODS: The study was registered in the German Clinical Trials Register (DRKS00034369, registration date: 20/11/2024). A lower jaw model was fabricated us

Objectives: The objective of this study was to conduct three-dimensional cone-beam computed tomography (CBCT) analysis of posterior mandibular anatomy in order to investigate its potential correlation with the occurrence of bad splits during sagittal split osteotomy (SSO). Methods: A retrospective review was conducted on 56 patients (112 hemimandibles) who underwent bilateral SSO between 2016 and 2022. Distances between the mandibular canal and the borders of the mandible, along with anatomical landmarks, were measured. Sites with and without bad splits were compared using chi-square and Mann-

Orthognathic surgery is a typical approach for treating maxillofacial deformities. However, orthognathic surgery results in positional changes in the condyles. In a previous review, the effects of orthognathic surgery on temporomandibular joints (TMJs) are not provided. Hence, in this study, we investigate the morphological changes in TMJs after mandibular and bi-maxillary osteotomies. The relationship between the morphological parameters of TMJs and symptoms of temporomandibular disorders (TMDs) is discussed. Finite element contact stress analysis is performed, and the results show that the t

BACKGROUND: Orthognathic treatment of patients with juvenile idiopathic/rheumatoid arthritis is still controversial. Mandibular procedures may result in relapse and further condylar resorption and pain. Maxillary osteotomies and genioplasty may be effective for the correction of malocclusion and for aesthetics.
PATIENTS AND METHODS: Five patients with juvenile idiopathic/rheumatoid arthritis with severe temporomandibular joint (TMJ) involvement underwent orthognathic surgery with a simultaneous Le Fort I osteotomy and advancement genioplasty. The age of the patients at the time of surgery ran

This study was to describe the use of inverted-L osteotomy of ramus and iliac bone graft for the management of mandibular deficiency in adult patients. From 2008 to 2010, 11 patients (aged 19 to 29 years) with mandibular deficiency underwent intraoral or extraoral inverted-L osteotomy of ramus and iliac crest bone grafting. Data were collected from the patients' records, photographs and radiographs. The height and width of the ramus were successfully expanded by inverted-L osteotomy and iliac crest bone grafting with minimal complications in all patients, resulting in significant improvement i

A 23-year-old female with hypoglossia, who had a narrow mandibular dental arch, was treated using the gradual expansion technique. Three lower incisors were missing and the right molar occlusion showed a scissor bite. Her speech was acceptable. Gradual unilateral expansion of the mandibular alveolar bone was performed. Orthodontic tooth alignment was performed prior to surgical treatment. A tooth-borne expander was devised using a hyrax-type screw to move the inclined right alveolar bone into an upright position. Alveolar bone osteotomies were performed under general anesthesia and the expande

A 33-yr-old woman with marked maxillo-facial deformities as a result of underlying beta-thalassaemia major was to undergo corrective maxillary and mandibular osteotomies. The placement of an endotracheal tube posed a problem in this patient because of anatomical deformities in her nasal passage, surgical constraints on using the oral route, and reluctance of the patient to have a tracheostomy. This case report describes the use of a submental tracheal intubation technique, and the associated anaesthetic difficulties encountered in patients with this pathology.

We prospectively studied nine patients with the obstructive sleep apnea (OSA) syndrome with polysomnography before and after combined (seven simultaneous and two sequential) uvulopalatopharyngoplasty and inferior sagittal mandibular osteotomy with genioglossus advancement as treatment for OSA. All patients had failed to tolerate nasal continuous positive airway pressure and had a respiratory disturbance index (RDI) > or = 20. All patients were found to have disproportionate pharyngeal anatomy with airway narrowing by cephalometrograms and/or fiberoptic nasopharyngoscopy. Following surgery ther
