[Mastoid bone graft in implantology. A case report].

INTRODUCTION: In implantology, autogenous bone remains currently a useful-material to rebuild horizontal and vertical osseous defects. Usually, it is the needed osseous volume that indicates the grafting procedure, intraoral one for small and middle volume and extraoral one for severely atrophied jaws. The originality of this case report is to show an alternative to usual grafting procedure borrowed from ENT surgeons: osseous mastoid graft. TECHNICAL NOTE: A 63-year-old patient with maxillary sinus pathology underwent during same surgical time maxillary sinus curetting and osseous grafting to

Mastoid subperiosteal abscess in children: drainage or mastoidectomy?

OBJECTIVE: To evaluate the management of mastoid subperiosteal abscess using two different surgical approaches: simple mastoidectomy and abscess drainage. METHOD: The medical records of 34 children suffering from acute mastoiditis with subperiosteal abscess were retrospectively reviewed. In these cases, the initial surgical approach consisted of either myringotomy plus simple mastoidectomy or myringotomy plus abscess drainage. RESULTS: Thirteen children were managed with simple mastoidectomy and 21 children were initially managed with abscess drainage. Of the second group, 12 children were c

Computerized assessment of the mastoid air cell system.

There have been many arguments on the development of pneumatization of temporal bone. However, a technique for direct volume measurement from high resolution computed tomography has never been reported. The aim of this paper is to develop a technique by using digital image processing to measure the volume of the mastoid air cell system. Forty three ears of 26 healthy subjects (13 males and 13 females) without a history of chronic or exudative otitis media, clear signs of Meniere's disease, severe sensorineural hearing impairment or malformation of temporal bone were eligible for enrolment in t

The facial ridge and the discharging mastoid cavity.

This study evaluates the causes for a persistently discharging mastoid cavity and attempts to weight the importance of each identified factor. It consists of a temporal bone dissection to evaluate the anatomy of the facial nerve and a clinical prospective study measuring the factors that contribute to a wet cavity. The factors measured were height of the facial ridge, the size of the meatus, state of the tympanic membrane, and size of the mastoid cavity. The height of the facial ridge and the kidney shape of the cavity were found to be the most significant factors contributing to the state of

Endolymphatic mastoid shunt surgery in unilateral Menière's disease.

One-hundred endolymphatic mastoid shunt operations in patients with classic unilateral Meniere's disease were analyzed. The patients were carefully preselected with a comprehensive protocol of audiovestibular and metabolic investigations. All patients had definitive electrophysiologic evidence of endolymphatic hydrops with an enhanced negative summating potential on transtympanic electrocochleography. The surgical results were analyzed both by the original American Academy of Ophthalmology and Otolaryngology Guidelines (AAOO, 1972), and the more recent modifications of the American Academy of

Effectiveness of prophylactic antibiotic treatment in mastoid surgery.

This article describes a prospective study of the effectiveness of prophylactic antibiotic treatment in preventing infection following mastoid surgery. Seventy-two patients who underwent surgery for chronic middle ear disease served as the basis for this study. Bacteriologic findings from middle ear discharge, showing aerobic and anaerobic bacteria, are reported. The patients were randomly classified into two groups, one undergoing surgery with preventive antibiotic treatment with clindamycin and gentamycin and the other undergoing surgery without antibiotic therapy. The early postoperative in

[Interdisciplinary Management of Acute and Chronic Mastoiditis].

Acute mastoiditis represents the most common complication of acute otitis media in childhood and, despite a declining incidence in the era of vaccine prevention, remains a clinically significant entity. Particularly in infants and immunocompromised patients, symptoms may be nonspecific, thereby complicating early diagnosis. The present review provides a structured overview of the etiology, pathophysiology, diagnostic workup, and therapeutic management of acute and chronic mastoiditis, with special emphasis on interdisciplinary aspects. The pathogen spectrum is age-dependent and subject to epide

The complexity of acute mastoiditis in Swedish children.

OBJECTIVES: Acute mastoiditis (AM) is a rare complication to acute otitis media (AOM) in children. Population-based studies are useful for establishing adequate treatment strategies and to evaluate the effects related to the introduction of the pneumococcal conjugate vaccine (PCV) in 2007. Previous reports however report on an overdiagnosis of AM, warranting the need for validity assessment. The aim of the present study was to assess the validity of ICD-10 diagnoses of AM and to address the differential diagnoses. Further, we aimed to map the incidence of AM over time and to specifically map t

[PATHOMORPHOLOGICAL CHANGES UNDER LATENT MASTOIDITIS].

In our opinion it is caused by incomplete awareness of the distribution of pathological process and the character of morphological changes in the patients. In this regard, we studied structural changes in the soft and bone tissue taken from various cavities of the middle ear system. Operating material was formed in paraffin after the fixation and proper processing. We proceed with decalcification of bone tissues in 15-20% nitric acid. We painted 5-6 mm thick peels prepared on the rotating microtome with hematoxylin and eosin. The microscopic examination revealed inflammatory-destructive change

Acute Mastoiditis Caused by Streptococcus pneumoniae.

Acute mastoiditis (AM) is a relatively rare complication of acute otitis media (AOM). The most common pathogens include Streptococcus pneumoniae, Streptococcus pyogenes, and Staphylococcus aureus. Pneumococcal vaccination and changes in antibiotic prescribing recommendations for AOM may change the incidence of AM in the future. Diagnosis of AM can be made based on clinical presentation, but computed tomography of the temporal bone with contrast should be considered if there is concern for complicated AM. Both extracranial and intracranial complications of AM may occur. Previously, routine cort
