The experience of meaningful rehabilitation as perceived by people with chronic pain: A phenomenological study.

BACKGROUND: People with chronic pain may seek rehabilitation to reduce pain and restore productivity and valued roles. Theoretically, a biopsychosocial approach makes rehabilitation more meaningful, however, the limited research on meaningful rehabilitation predominantly describes the perspective of therapists and researchers. The client's perspective of meaningfulness in rehabilitation is lacking. OBJECTIVE: To investigate the experience of meaningfulness in rehabilitation from the perspective of people with chronic pain. METHODS: Qualitative, semi-structured interviews were conducted with

Repetitive Transcranial Magnetic Stimulation Over the Left Posterior Middle Temporal Gyrus Reduces Wrist Velocity During Emblematic Hand Gesture Imitation.

Results from neuropsychological studies, and neuroimaging and behavioural experiments with healthy individuals, suggest that the imitation of meaningful and meaningless actions may be reliant on different processing routes. The left posterior middle temporal gyrus (pMTG) is one area that might be important for the recognition and imitation of meaningful actions. We studied the role of the left pMTG in imitation using repetitive transcranial magnetic stimulation (rTMS) and two-person motion-tracking. Participants imitated meaningless and emblematic meaningful hand and finger gestures performed

New device, old algorithm? Bridging generations in perioperative coagulation management.

Viscoelastic testing permits targeted correction of coagulopathy in bleeding patients. As new generations of viscoelastic testing platforms become available, research exploring similarities and differences with older devices can provide insight for institutions seeking to use the newer technologies. Care must be taken to ensure such studies are designed to produce clinically meaningful guidance for adapting existing treatment algorithms to accommodate the latest viscoelastic testing technology.

Healthy working environments are essential, but what exactly is HWE policy and who would be responsible for its success?

A fundamental shift towards healthy working environments is essential, and it requires meaningful collaboration between employers, workers, and policymakers. While this argument may seem clear in public health research and practice, its implications for policy are profoundly unclear. This lack of collective clarity and purpose will undermine meaningful progress. We identify the need to address three questions: what exactly is HWE policy, what policy instruments should be adopted, and who would be responsible for their design, delivery, and coordination? We use the example of EU policy innovati

Feasibility of the "Take Action" Cognitive-Functional Program During Active Chemotherapy in Women With Breast Cancer.

Women with breast cancer experience treatment-related side effects, including cognitive impairment, that adversely affect functioning and quality of life. Despite growing recognition of cancer-related cognitive impairment, few studies have examined rehabilitation during active oncological treatment. This study examined the feasibility of the Take Action (TA) cognitive-functional occupational therapy program during chemotherapy among seven women. Feasibility indicators, perceived cognitive function, daily functioning, and well-being were assessed at three time points. The TA program comprised s

Neurally adjusted ventilatory assist in neonates with congenital diaphragmatic hernia.

OBJECTIVE: To measure short-term outcomes of neonates with congenital diaphragmatic hernia (CDH) while on Neurally Adjusted Ventilator Assist (NAVA), and to measure the impact of a congenitally abnormal diaphragm on NAVA ventilator indices. STUDY DESIGN: First, we conducted a retrospective-cohort analysis of 16 neonates with CDH placed on NAVA over a treatment period of 72 h. Second, we performed a case-control study comparing NAVA level and Edi between neonates with CDH and those without CDH. RESULTS: Compared to pre-NAVA, there were clinically meaningful improvements in PIP (p < 0.003), Re

Ex vivo human placental imaging: Navigating modalities, scales, and analysis approaches to obtain fit-for-purpose data.

The placenta is one of the most accessible human organs for ex vivo imaging, yet it still remains one of the least understood. Recent advances in ex vivo imaging approaches provide opportunities to capture the complex anatomy of the placenta and its vasculature across multiple scales. However, alongside this it is imperative to consider how we can optimise workflows - from tissue sampling and preparation to analysis - to obtain meaningful quantitative data from imaging, including computational handling of large and complex datasets at scale. Indeed, such tools are critical to advance our insig

A 15-Item modification of the PSP rating scale to improve clinical meaningfulness and statistical performance.

Progressive supranuclear palsy (PSP) is a rare neurodegenerative disorder characterized by physical, cognitive, and behavioral impairments. The PSP Rating Scale (PSPRS) is a widely used and validated, clinical scale to monitor disease progression. Here we show the modification of PSPRS to improve clinical meaningfulness and sensitivity. A conceptual framework was used to select meaningful items. Datasets were analyzed to improve the psychometric properties, understand factor structure, and develop a scoring algorithm. Fifteen items of the PSPRS were selected based on whether the items reflect

Towards clinically relevant dose ratios for Cabamiquine and Pyronaridine combination using P. falciparum field isolate data.

The selection and combination of dose regimens for antimalarials involve complex considerations including pharmacokinetic and pharmacodynamic interactions. In this study, we use immediate ex vivo P. falciparum field isolates to evaluate the effect of cabamiquine and pyronaridine as standalone treatments and in combination therapy. We feed the data into a pharmacometrics model to generate an interaction map and simulate meaningful clinical dose ratios. We demonstrate that the pharmacometrics model of parasite growth and killing provides a detailed description of parasite kinetics against cabami

Clinically meaningful change for the chair stand test: monitoring mobility in integrated care for older people.

BACKGROUND: Clinically meaningful changes in the five-repetition chair stand test are essential for monitoring mobility in integrated care for older people. Recommendations for the clinically meaningful change of the chair stand test are not well known. Our study aimed to estimate the absolute and relative clinically meaningful changes for older adults' five-repetition chair stand test. METHODS: We applied distribution-based and anchor-based methods in addition to receiver operator characteristics analyses to a population-based study of community-dwelling adults (SAGE Mexico study, n = 897) t

Large posttraumatic stress disorder improvement and antidepressant medication adherence.

BACKGROUND: Patients with vs. without posttraumatic stress disorder (PTSD) are more likely to have poor antidepressant medication (ADM) adherence but it is unclear if improved PTSD is associated with ADM adherence. We determined if clinically meaningful PTSD symptom reduction was associated with ADM adherence. METHODS: Electronic health record data (2008-2015) was obtained from 742 Veterans Health Affairs (VHA) patients using PTSD specialty clinics with a PTSD diagnosis and PTSD checklist (PCL) score ≥50. The last PCL in the exposure year after the first PCL≥50 was used to identify patients w

Improving UK data on avoidable perinatal brain injury: review of data dictionaries and consultation.

BACKGROUND: High quality data is important to understanding epidemiology and supporting improvement efforts in perinatal brain injury. It is not clear which data items relevant to brain injury are captured across UK sources of routinely collected data, nor what needs to be done to ensure that those sources are fit for purpose in improving care. METHODS: We reviewed data dictionaries of four main UK perinatal data sources and consulted a multi-professional group (N = 27) with expertise in neonatal/maternity care, statistics, and clinical negligence. RESULTS: None of the data sources we review

Human factors engineering validation study for a novel 0.1-mg epinephrine auto-injector.

Background: Anaphylaxis in infants and young children is increasing. Historically, epinephrine auto-injectors (EAI) were not available in a dose or platform designed for patients who weighed <15 kg, and, therefore, 0.15-mg EAIs were prescribed for these patients. Results of ultrasound studies indicate that currently marketed 0.15-mg EAIs have needle lengths that may strike bone in ∼29 to 43% of patients who weigh <15 kg and result in possible intraosseous injection. An EAI with a more weight-appropriate 0.1-mg dose and shorter needle length has been developed to potentially help minimize the r

Biomarkers and surrogate endpoints for drug development in propionic acidemia.

Drug development in rare metabolic diseases is frequently limited by the lack of validated clinical trial endpoints, particularly for chronic disease modification. Propionic acidemia (PA) is an intoxication-type inherited metabolic disorder with significant morbidity and mortality that begins in early life, for which there are no approved disease-modifying therapies. While regulatory pathways such as accelerated approval permit the use of surrogate endpoints that are reasonably likely to predict clinical benefit, their acceptance requires strong biological rationale and evidence linking biomar

Supporting the Primary Outcomes of the Mirai Trial for the Adjunctive Digital Therapeutic Rejoyn (CT-152) in the Treatment of Major Depressive Disorder: Meaningful Change Analysis in the Montgomery-Åsberg Depression Rating Scale.

BACKGROUND: Rejoyn (CT-152) is a prescription digital therapeutic (DTx) adjunct to antidepressive medication authorized for patients with major depressive disorder. To better understand the patient benefit of DTx and other treatment modalities, current regulatory standards support the use of modern psychometric methods to interpret the clinical meaningfulness of treatment effects for patients. In the primary analysis of Rejoyn from the pivotal phase 3 Mirai trial (NCT04770285), Rejoyn showed a broad risk-to-benefit profile as demonstrated on multiple clinician- and patient-rated scales, includ
