Diagnostic Approaches for Measles Virus: Methods, Advances, and Ongoing Challenges.

Measles, also known as rubeola, is a highly contagious and potentially life-threatening disease caused by the measles virus. It classically presents with fever, cough, coryza, conjunctivitis, and a maculopapular rash. Despite the availability of an effective vaccine for decades, measles outbreaks continue to occur globally, largely driven by declining vaccination coverage and increased international travel. With no specific antiviral therapy available, rapid and accurate diagnosis remains essential for timely clinical management and effective outbreak control. Diagnostic methods have evolved f

Prolonged Maternal Shedding and Maternal-fetal Transmission of Measles Virus.

Prolonged measles virus detection in maternal saliva and blood was evidenced in 6 pregnant women. Maternal-fetal transmission was evidenced in 2 of 4 infants who were asymptomatic at birth, 21-24 weeks after maternal infection. Whereas peripartum congenital measles is severe, asymptomatic measles virus vertical transmission can occur earlier in pregnancy.

Severe acute measles pneumonitis: virus isolation in bronchoalveolar lavage fluid.

In the past few years, several endemic outbreaks of measles have been recognised, not only in children but also in adults, with severe and, occasionally, even fatal complications, possibly due to delayed diagnosis of the disease in adult medicine and decreasing vaccination rates. Furthermore, the treatment consists of supportive measures only. We present a case of severe measles pneumonitis in a 42-year-old man, a travel returnee, proven by direct virus isolation with PCR from bronchoalveolar lavage fluid. CT findings and pulmonary function testing revealed features of obstructive bronchioliti

Neurokinin-1 enables measles virus trans-synaptic spread in neurons.

Measles virus (MV), a morbillivirus that remains a significant human pathogen, can infect the central nervous system, resulting in rare but often fatal diseases, such as subacute sclerosing panencephalitis. Previous work demonstrated that MV was transmitted trans-synaptically and that, while a cellular receptor for the hemagglutinin (H) protein was required for MV entry, it was dispensable for subsequent cell-to-cell spread. Here, we explored what role the other envelope protein, fusion (F), played in trans-synaptic transport. We made the following observations: (1) MV-F expression in infected

Fetal growth restriction associated with measles virus infection during pregnancy.

We report on a 28-week infant with growth restriction starting after 23 weeks' gestation because of measles virus (MV) infection of the mother. Histological findings for the placenta revealed extensive fibrin deposition and necrosis of the villi, and MV antigen was demonstrated in the syncytiotrophoblast by immunostaining. The MV-specific IgM level in the infant was negative, but that of the mother was positive. Therefore, we speculate that growth restriction is not attributed to direct infection with MV, but to placental dysfunction due to a decrease in intravillous blood flow and oxygen supp

Decreased measles virus-neutralizing activity in sera from otosclerotic patients.

HYPOTHESIS: Any kind of depressed systemic anti-measles reaction can lead to the induction of a local immune response in the inner ear and possibly to reactivation of bone turnover in this region. METHODS: Different dilutions of sera were tested for neutralizing activity against a constant viral concentration. The ability of measles virus to infect and replicate in the cell monolayer was detected by enumeration of living and growing cells with a colored reaction. RESULTS: Virus-neutralizing activity in the sera of patients with confirmed otosclerosis was significantly weaker than in that of

Antisense-mediated resistance to measles virus infection in HeLa cells.

Endogenous expression of antisense RNA in transfected cells has been explored for use in blocking cellular gene expression and for its antiviral potential. Antisense strategies were used with the goal of blocking measles virus (MV) infection. A recombinant expression plasmid was designed to produce antisense oligonucleotides targeted to the 5' end of the MV nucleocapsid protein mRNA. This construct was transfected into HeLa cells. The transfected cell line and a control cell line expressing a random RNA comprising the same nucleotides were infected with MV and assessed for viral resistance by

Towards eradication of measles virus: global progress and strategy evaluation.

Despite an increase in global measles vaccine coverage from under 20% in 1980 to around 80% in 1990, measles remains a major cause of morbidity and mortality world-wide. This paper addresses a number of issues relating to efforts to control measles, namely, (i) at the global level, how might we assess the impact of measles vaccination on the incidence of infection and associated disease, and, (ii), at the strategic level, how can we utilise an understanding of the transmission dynamics of childhood viral infections to aid the design of optimal immunisation programmes? Based on WHO vaccine cove

Detection of measles virus nucleoprotein mRNA in autopsied brain tissues.

By means of RT-PCR, a portion of measles virus (MV) mRNA encoding nucleoprotein (NP) could be detected in 11 (18%) of 61 brain tissue samples obtained from administrative autopsy cases, who apparently had not suffered from subacute sclerosing panencephalitis (SSPE)-like central nervous system disorders. Most of the brain-derived NP sequences showed significant asynonymous nucleotide substitutions when compared with wild-type MV isolates and SSPE virus. Our present results suggest that MV commonly persists in the human brain without causing apparent clinical symptoms, probably due to decreased

Growth of measles virus in various human lymphoid cell lines.

Neurovirulent TYCSA strain and attenuated Schwarz strain of measles virus and Halle strain of subacute sclerosing panencephalitis (SSPE) virus replicated in cultures of human lymphoid cell lines of the T-cell type, MOLT-3, MOLT-4 and CCRF-CEM. TYCSA and Halle strains grew rapidly, but Schwarz strain grew slowly in these cell lines. Furthermore, these three strains established persistent infection in CCRF-CEM cells but not in the other cell lines. In these persistently infected cultures an almost entire population of cells were shown to be infected and infectious virus was produced constantly f

Immunological response of subacute sclerosing panencephalitis patients to measles virus.

The immunological responses of patients with clinical signs of SSPE were examined by the direct migration inhibition, complement-mediated antibody cytoxicity and the lymphocyte-mediated cytotoxicity assay techniques. Peripheral lymphocytes migrated similarly to controls when exposed to measles antigen. Lymphocytes from SSPE patients responded to a SSPE strain of measles infected cells in a manner similar to lymphocytes from controls. A factor which interfered with the expected activity of sensitized lymphocytes to measles virus was detected in some cerebral spinal fluid obtained from SSPE pati

Measles-virus-specific immunoglobulin-M response in subacute sclerosing panencephalitis.

Measles-virus specific IgM and IgG responses in sera and cerebrospinal fluid (C.S.F.) of 20 patients with subacute sclerosing panencephalitis (S.S.P.E.) and of two control groups were examined by indirect radioimmunoassay. All S.S.P.E. patients, regardless of the stage of the disease, had high titres of anti-measles antibodies in sera and C.S.F. The antibody activity was associated with both the IgM and the IgG classes of immunoglobulins. In 7 (35%) of the S.S.P.E. cases the specific IgM response was more pronounced in the C.S.F. than in the serum, suggesting IgM production within the central

Cell-mediated immunity to measles virus in subacute sclerosing panencephalitis.

Four patients with subacute sclerosing panencephalitis were investigated for their specific cellular and humoral immunities against measles virus. Lymphocytotoxicity mediated by peripheral blood lymphocytes was evaluated with the colony inhibition test of target cells having measles antigen. The lymphocytes from two patients of subacute sclerosing panencephalitis (SSPE) specifically destroyed the carrier cells; however, no significant lymphocytotoxicity was observed in the other two patients. The result suggests the heterogeneity in cellular immune states in SSPE patients.

Thymic dysplasia, persistence of measles virus, and unexpected infant death.

A case of sudden infant death in an 18-month female infant is described which occurred 3 weeks after clinical recovery from a typical attack of measles. At post-mortem examination no obvious cause of death could be found apart from some terminal aspiration of vomit. Histologically, the thymus showed selective absence of Hassall's corpuscles with a normal number of cortical lymphocytes. There were many atypical lymphocytoid cells in the lymph nodes, spleen, and portal tracts, and measles virus was subsequently isolated from the lung and tonsil. There was a raised titre of complement fixing meas

Measles Virus Bearing Measles Inclusion Body Encephalitis-Derived Fusion Protein Is Pathogenic after Infection via the Respiratory Route.

A clinical isolate of measles virus (MeV) bearing a single amino acid alteration in the viral fusion protein (F; L454W) was previously identified in two patients with lethal sequelae of MeV central nervous system (CNS) infection. The mutation dysregulated the viral fusion machinery so that the mutated F protein mediated cell fusion in the absence of known MeV cellular receptors. While this virus could feasibly have arisen via intrahost evolution of the wild-type (wt) virus, it was recently shown that the same mutation emerged under the selective pressure of small-molecule antiviral treatment.
